It’s interesting that there is concern regarding side effects of female hormone replacement yet little to no concern among the medical professionals to prescribe statins, blood pressure, osteoporosis, anti anxiety medications, etc. many of which come with serious side effects. This country is out of control with prescription medication usage. It never stops.

How would this work for women with estrogen dominant breast cancer?

It depends on the cancer. We work with women who have estrogen positive cancers and come up with unique plans that work with their oncology teams. It’s a challenge but so common that we have to try to find a compromise between breast cancer risk and osteoporosis fracture risk.

Would love info on Creatine and Taurine. I have seen others saying that there are studies showing positive results with bone density when taking Creatine and Taurine. What are your thoughts on this?

I’m an advocate for creating and taurine but haven’t looked at evidence specifically for bone health. I’ll have my team take a look.

I'm a 57 year old man with osteoporosis. I fractured 2 vertabra in Aug 23 and that's what it took to convince an endicronologist to treat me (relcutant prior to the fracture because of the limits one can be on drugs). So I am on Forteo. On my own I am taking testosterone (which was low) and nandrolone. I am confused by 2 things: 1) If Forteo increases bone density AND testosterone and nandrolone also do this - do I get triple the density if I am on all 3? And what do I have to stop any of these drugs in a period of time? FDA allows Forteo for more than 2 years now. Even if bone density is increased only a percent or two a year, shouldn't that continue on these drugs year after year - assuming I use all of them indefinitely? I don't see why any of them would stop producing bone or why I should stop any of them.

I agree with taking testosterone to supplement deficiency as needed BUT nandrolone and Forteo are not natural and long term data is not available on safety.

I’ve always had an interest in the trilogy of hormones for menopause symptoms. I thought now being past menopause, hormone treatments were more likely to cause negative effects. How does age, or time past menopause, change recommendations for HRT?

The risk goes up with estrogen and the 10 year is the threshold for many doctors. I'm willing to go past 10 years but only on a case by case basis. Testosterone and progesterone though have very few restrictions based on age!

Is progesterone appropriate for post menopausal women without a uterus?

In my practice we do recommend it because there are progesterone receptors all over the body, even in bones. It also helps with sleep when taken orally.

What is your opinion on Geranylgeraniol or ‘GG’. There have been some studies on this supplement that shows some results on increasing bone density. Can you discuss this?

Is Micro Crystalline Hydroxy Apatite (MCHA) the same as Milk Bovine Protein (MBP)? Is colostrum the same thing as MBP? Or are these three all different? Thank you and God bless you for the work you do.

which treatment would you recommend someone who has osteopenia and is recovering from a long lasting eating disorder? I've been suggested starting hormone treatment but I am reluctant due to possible side effects. I would appreciate your reply. I really value your opinion!

Hi there. Most people can be on HRT without fear of complications. Side effects are not that common and easily managed. HRT is great for people with eating disorders as well! Come join our masterclass and bone foundations course for more on these topics.

Super interesting. My questions are - 1. What percentage of male patients do you have, 2. If you have these patients, have any been on long term trt therapy, and 3. What percentage develop side effects inimical to the continuation of therapy? Maybe another video diving into the nuances?

Hi there. We are about 1/3rd male last I checked. Not many are on testosterone when they come in but most need it! Testosterone deficiency is a common cause of low estrogen which results in increased bone loss in men!

That's interesting. My doctor said my testosterone was okay but he also said my vitamin D was OK but my nutritionist said it (vit D) was to low. So what is an acceptable testosterone level over 50?

I don't believe you can simply say there is an acceptable number, because biochemistry is so complicated and there are many nutritional and lifestyle factors at play. I would say if your libido is ok and you can put on some muscle when you want, you are probably good. You cannot really go wrong with more vitamin D. Accepted current opinion is anything from 1000 to 5000 units per day. The winning combination seems to be vitamin d, vitamin k and magnesium. If you still feel excessively tired, bad mood, low sex drive and you are eating ok and not drinking much, that would be the time start asking real questions.

Is there an age limit when you use progesterone with estrogen? My Dr said you don’t use progesterone past 10 years after menopause? But is this really true?

The 10 year arbitrary cut off is an archaic approach to HRT. We use it as long as we want the benefit of it.

I am almost 72. I finished 2 years of Forteo last September. I improved 16 percent in my spine, but it is still osteoporosis. I hate jaw death and sudden femur break. I don't know what to do now.

Medicare should cover some options for HRT. We prescribe it commercially for those that can’t afford compounding. That said, I prefer compounding for my patients. It allows better flexibility in dosing and dialing in blood levels.

Hi there. Calcium supplementation or calcium through diet is probably not related to most bone spurs. We verify that patients are not getting too much calcium in diet or through supplementation but not generally going to be related to bone spurs! There is no natural bone spur removal through supplementation or otherwise.

I've been weight training for years and still snap bones easily. Since I began to get too muscular, (like a linebacker) I now only visit the gym twice a week. On the plus side since I'm a retired senior the gym is my only social life on the down side, I devour food after the gym. Oh well, thee are worse things in life 🤷‍♀️

Just a suggestion. if you aren't drinking whey powder with 1/2 Tablespoon of extra virgin olive oil in it, try it. Maybe a Tablespoon of extra virgin olive oil in the whey powder. . Kion and Equip are high quality Why Isolate powders. Check out "Rebuild Your Bones" by Dr. Jayson and Mira Calton

Hi there! Liftmore is a good start as indicated by @skitchyjohnson BUT remember that in that trial they also used impact in addition to resistance training. Let’s not be confused about the role of resistance training. It’s for muscle which secondarily improves bone.

@@Dr_DougLucas Thank you for saying this. Not a popular opinion. IMHO there’s a cost to living longer and it’s quality of health. We need a balance. Who wants to take 60 pills a day?? Edit: pasta granny videos on amazing older (mostly Italian) women making him made pasta. Find the one the 101 yo. Still mobile and sharp! Same with some friends Nonna’s and Nonno’s. Low carb? 😂👌

Is it dangerous to take a vitamin D2 50,000 unit? My new Dr wants me to take 1 pill a week for 4 weeks and take Calcium 500 mg D3 400 IU twice a day everyday. I need to get lab work again at the end of taking D2

We prefer D3 instead of D2. It's not dangerous unless taken for too long. Vitamin D toxicity is real but not that easy to get to.

@@Dr_DougLucas thank you. What are your thoughts on an injection of Prolia. I am a bit concerned about the side effect of the inflammation around the heart. I am on a carnivore diet and when you run my labs for cholesterol it is high. My concern is if I take the Prolia and my cholesterol goes higher my Dr will want you to put me on a statin

@@PattyC1211 Common question! Some people react to a higher dietary or sat fat diet with elevated cholesterol. This may increase your risk of heart disease but current evidence suggests that it doesn't. I ask my OB if she would give me hormones because she said I'm losing bone Because of Menopus , and she refused to prescribe it to me Because of my age , and her Fear of cancer . it seemed A sensible solution to bone loss to me , not so much for The doctor .

I am still studying on the hormones but i asked for the Combipatch for hot flashes, sleep, mood issues & it has really helped. Also my migraines dramatically decreased. Hormone balance is very important. I also have been diagnosed with osteoporosis/osteopenia in my spine & hips. I wonder if I had been diagnosed earlier & started supplementation & hormones earlier, my bone loss would not have advanced to this point. I will be looking into going to a gyn & coming off hormones to get properly tested for my hormone levels & asking about testosterone cream. I am 56 yrs old & wish someone had told me about this stuff years ago.

I'm taking calcium from all kinds of different sources Microcrystalline , I'm getting that from bone up by jarrow Bone stiulatinging peptides Long bone Bovine , from Grass fed cows living bone by ancestral, algae , I'm also Going dairy with Kefir , and a cup of blue berries . antioxidants and other vitimins , fish oil I'm doing the big push for 6mo until my july scans and I will adjust my calcium down When I get my results back , Plan to join master class All they want to do now is Push Prolia at me , Oh I'm doing Melatonin 6 mg and taking Estradiol For menopause issue , Haha they said no hormones but here I am Only because of bladder infections , so I'm hoping that will also help ! I think my OB just gave me wink and a nod ! I'm not making this up ! Haha

Hormones can be beneficial for bones. No question. There is a risk:benefit conversation to be had BUT we are generally able to start most women on HRT with osteoporosis. Consider a second opinion.
