You completely disregarded the dietary aspect of this inflammatory condition. I have been to 6 dermatologists for over 11 years of suffering with this condition. Every single one of them told me the same exact script, saying this has nothing to do with diet. I had to figure it out myself by doing elimination diet. Turns out it was all about PROCESSED SUGAR. I know others who completely treated PD by going gluten, dairy or soy free as well. Mine went away in 2 weeks time. And every time I eat sugar it comes back. so thats how I know thats what is causing it. I feel really let down by western medicine. Western doctors are like drones, say the exact same script and sell you the same cream and make you their permanent customer. Remember FOOD IS MEDICINE first and foremost. I hope someone sees this comment and chooses to discover what causes their "leaky gut" and inflammation that then causes PD.

Wow. I’m sorry that your doctors treated you this way :( I have had PD for about 6 years now, and my dermatologist was very transparent about the fact that there are many many possible reasons that one can develop perioral dermatitis; it is different for everyone. To disregard food as a possible trigger is absolutely ridiculous! Everything we put into our body has an affect on us!! Im so glad you were able to finally determine what your trigger was, and that you stuck to your guns about it being food related!

@@erinmarieee23 Thank you for this comment! Your doctor is right - there are many reasons. The phrasing doctors use can be very different. Your doctor said "there are many many reasons", my doctors said "we don't know why, it just happens". Your doctor communicated that there is something to look for and discover about your body. My doctors told me to just give up and put some cream on for the rest of my life 😅

@@mrsfrogleggs03 None. I literally just stopped eating processed sugar and it cleared out in 2 weeks. It might not be processed sugar for everyone. The point is - often PD is caused by inflammation in the gut. What causes inflammation - you have to figure out ourself. For some people it can be soy or gluten etc. I'd recommend you research elimination diet and see if it's something you are wiling to try. If you will be doing that diet, make sure that you first clear up the PD on your face to the point where you don't need the prescription cream anymore. Then, when re-introducing different foods, do it gradually. Give it like 4 days. Elimination diet suggests 1-2 days, but I found that with PD it takes a while to show up again, so give it time. But make sure not to use the prescription cream, or else you will not see what's triggering it. Hope that helps!

It’s awful I know, I found that using retinal helped. But it’s a constant battle and can be very depressing. I find stress kicks mine off. Gentle cleanser helped too as I think it’s partly a breakdown in skin barrier.

I was using strong toothpaste and mouthwash. Changed to a very mild supermarket toothpaste tiny amount. Took antihistamines for a few days. All gone. Thanks for the info.

I experienced perioral dermatitis after 3 weeks of retinol use. Get a prescription for metronidazole. After just a few days my rash is almost gone. I'm thrilled with the results.

If you are at the initial stage of it.. Just keep your face clean.. Use face wash if you've used any product such as sunscreen or moisturizer and then apply Surfaz SN ointment.. It is available at the medical store.. Worked for me

yeh, you can try also some cream with zink, however my favorite is Cicaplast Baume B5+ from La Roche-Posay.

I hope and pray this helps many of you suffering for a long time. I suffered from this since November of 2023. It first started small then overtook half the bottom of my face. It burned, itched and tingled. I tried almost every natural remedy which may have made it worse. I also was prescribed metro-gel for my family doctor which made it even worse as well. The only things that worked were plan greek yogurt overnight on my face to stop the itching and I purchaed canestan external cream (2% clotrimazole cream) which is an Anti Fungal. My doctor told me that if its not caused by bacteria then its caused by a fungus. It has been 3 full days and my PD is almost Entirely healed (90%). I expect it to be fully gone by day 5. Please if any of you are frustrated then try it. I apply it 3 times per day God bless.

Had a rash on my upper lip that probably wasn’t too noticeable because of my heavy stubble until one night it became very inflamed, itchy and somewhat painful - noticeable. Heard that equal parts apple cider vinegar and water helps. As I had it on hand I gave it a try (why not) and it did a great job relieving some of the Ill itching and soreness. Next morning I got some Gold Bond with 3% salicylic acid (marketed for Psoriasis) and the day after some Azeliac acid off of Amazon. Don’t know which worked better because I used both but my rash in less than a week is almost gone. It’s pretty rare I get rashes or even sick but the couple of times I’ve had a rash it was due to certain fragrances and keeping them in contact with my skin for too long. Not sure about this time?

I’m not sure whether I had perioral dermatitis. I applied Aloe gel for 5 days and then shifted to Vaseline. In one day, most of the red patches disappeared and counting. Anyway, I wish no one would go through this disease.

Mine appeared after using my curology treatment I’m so upset I should’ve never tried curology again. Now it won’t go away it’s extremely noticeable and the dryness is horrible 😭 I had clear glowing skin all through my pregnancy and even postpartum

Also, are Doxycycline and Metro Cream safe to use long term? I’ve been dealing with perioral dermatitis for two years and it always comes right back when I finish a round of Doxycycline 😠 Thank you so much for sharing your knowledge!

It’s frustrating, as it can be resistant. I suggest a simple consistent skincare regimen. You may need to add other prescription topicals for maintenance to prevent reoccurrence but sometimes it’s just resistant so periodic doxy is appropriate but again it’s important to talk to your doctor. Another option is azelaic acid in addition to metro gel. Happy to do a consult if you are not able to see your Dermatologist

Regardless of what initially triggered it, Antibiotics are a major cause and perpetuation of this, because they can alter the microbiome allowing yeasts to proliferate on the skin like malassazia and candida which is usually the cause. Antihistamines play a huge role too because they affect H1 receptors that also control sebaceous glands which then don't secrete properly causing further skin drying and damage, that allows yeasts that feed off the fat of your cells and microbes to enter deeper into the dermis, then the immune system wants to react. It's a horrible viscous cycle mostly caused by psychotic MDs.

I developed this during my second trimester pregnancy, I am in my third now, it will not go away. 😦 Derm diagnosed it at Seborrheic dermatitis. The ketoconazole made it WORSE.

Mine was triggered by Vitamin C skincare. It went away at first so I tried a different brand and it's never gone. I'm sick of it... I've just started a topical antibiotic gel.

@@stellameii You're welcome, I thought it might help someone, Vitamin C is pretty strong. I hope you get rid of your dermatitis soon, I'm still using the antibiotic ointment and still getting new breakouts, probably because I ignored it for so long.

Hi doc My PD was caused by an overuse of steroids for my atopic dermatitis. Once I stopped the steroid for 3 days the rash around my mouth has noticeably reduced. So do I need to use any additional treatments to treat it or will it naturally clear up now that I've stopped the steroids ?

Hello Dr - this condition is new to me - I am almost 70 so not typical age for onset. I used steriod creams since I was misdiagnosed by my primary care - she tried but now of course it's worse than originally. My derm gave me metronidazole today so I will start that. Two questions: Can you use both metronidazole and azelaic 10 at the same time? Also, for years I did progesterone only bioidentical hormones , since fall I added back the estrogen/testosterone component. Though not high concentrations is it possible this is what brought it out and my treatments made it worse? I'm wondering if I should stop the add on of est/test, though I don't want to. Thank you, I look forward to your response.

Yes azelaic acid can be used with other topicals like metronidazole. But always double check with your doctor. There’s definitely a hormonal component meaning hormonal changes and fluctuations can certainly trigger it

Is perioral derm. exacerbated by humidity? I lived along the Pacific side of Central America when I first experienced itchy red patches around my nasal fold line area. Then, last summer i moved to the Caribbean and was fine all through dry seaaon but now that its wet season/hurricane season the perioral derm patches have returned.

Hi, I've been dealing with this for 4 years, and it usually goes away pretty fast, but it's the worst it has ever been. Im 16 weeks pregnant it always gets bad when im pregnant but goes away after a few weeks. I've tried everything you've suggested in your video, and still no relief. It's spreading to my chin now, and it's so painful. What do you recommend????

If you have some issues - gut problems, hormonal dysbalance, use some steroid medication, your dermatitis will not dissapear, even when using either crem with metro, or oral antibiotics.

Once I take an oral antibiotic then I have to fight a yeast infection. So I trade itching redness and burning around mouth and eyes for redness burning and itching in the vaginal area. Then I have to take an oral med for yeast infection and that med cause liver damage. This is just a horrible treadmill you just can't get off of.

Is it okay to put the azelic acid on the area in which is effected? For example my nares and around my nose is effected the most. Is it okay to put that directly on it? Should I put it on before or after my moisturizer? Thank you Dr!

Hi! I love your explanation. I have perioral dermatitis. First it started as small pimples arround the lower face area and I thought it as adult acne. But since July I started to get red patches arround the mouth with scally and sensitive skin. I fell like my skin is burning all the time. I went to a doctor and she prescribed me clindamicin as oral antibiotic for 8 days ( 4 pills every day) and topical eritromicin (every night). I am in the 3rd week after starting the treatment and it only shows a small improving...And the thing that scarries me the most is that some days it is better but other days it gets worse, and then it improves again and then it gets worse again. Can you please be so kind and help ? Is it normal this cycle.... ( it is summer in my country thats why she did not put me in any tetracyclines)

Did you listen at al what speaker explained? It depends. For example, if yor PD is caused by steroid drug, you will have it until you will stop treatment. If you have some gut problems as the cayse, your PD will last until you will get rid of your intestinal problems and so on. You cannot get rid of PD fast, if the cause is still acting within you.

My came on after I got off birth control and I was in the middle of moving and under a lot of stress at the time. I went to see a dermatologist and the stuff he gave me made it tremendously worse and extremely painful. I stopped everything, but the antibiotics and the burning and inflammation went down a little, however the red bumps and dry skin remained and it continued to spread more around my chin and mouth area. I had to go back on birth control and that was the only thing that helped keep it at bay. Years later I went off birth control again and tried different treatments with my doctor. The only thing that worked for me is the Clindamycin Phosphate 1% topical solution. I apply it at night time around the time of my month comes, I start to get little red dry bumps that will pop up around my chin and I know exactly what it is. Mine is definitely hormonal triggered as well as hereditary I believe, because my Dad had it when he was around my age and same with my sister.

Mine started after my daughter was born 😒 my skin was the only think that I felt confident about after I had her. I gained all my weight after her birth but at least I had good skin…until month 4 and then all hell broke lose! Still struggling with it

I had started changing my skin care routine afterwards too and that has helped immensely ❤️ we are waiting to see how things go after another week or so, but then I’m more then likely going to have to end up on the antibiotics like you said. Hello Doctor! I’ve been on doxycycline since July and my doctor has prescribed fucidin h ( antibiotic and hydrocortisone) for the last stage. I’m aware it comes with hydrocortisone which I’m scared of. What are your suggestions?

@@SkinByDrAzi hello , I have a few concerns about my skin . . . I went to a new dermatologist where we are stationed . My skin was okay for the most part at that time I was 6 months postpartum dealing with a little acne scaring so the doctor or nurse practitioner put me on a melasma cream for dark spots after a few months of use that’s when I started developing this redness around my chin area and month . Then once I started wearing makeup it didn’t burn or anything but it kind of made it a little more inflamed … they told me I had hormonal acne and gave me products that’s giving me dermatitis … now I’m taking mino 100mg and mupirocin . My skin was honestly not perfect , but I wasn’t expecting a melasma cream to cause this much trouble …. Is it a steroid ?

Can tacrolimus cream/ointment make Perioral Dermatitis worse? I find mine helped a bit but it made it go red every time and now there’s dark discolouration 😢

Hi Doc, I've been having the same symptoms as Perioral Dermatitis for the past 3 months. I've been to 4 different dermatolgist and 2 of them diagnose me with a Perioral dermatitis. The last one give me a mupirocin ointment, is it really effective to use? Hope you reply on my message doc

I'm suffering from periorial dermatitis almost 2to 3 years and it's around my nose skin turn red and start itchingand slowly its expand all over the face but it started from nose side . I meet lots of dermatologist but it's condition was same as before after i finish my medicine it come back. I'm using The Ordinary Azelaic Acid, i think its my 4th day....but everytime I applied it, it makes my skin itchy...is it normal?? Do i need to apply moisturizer? I'm currently using Oil Free Fragrance Moisturizer by Neutrogena. By the way, i'm in my 3month suffering from PD. I am also using Sulfur soap for my PD. Is Saline spray a nasal steroid? I have a perforated septum so have to use that. I have no idea why I got this perioral dermatis. It sucks so bad

Try applying a little bit of CeraVe healing ointment at night and see if that helps after a week or two. It’s been helping my nose area. It was extremely dry and flaky all the time and it’s the only thing that seems to help me the most with the dry and flaky skin caused from this condition. Best of luck.

Hi Dr Azi, thank you so much for this video! Im currently taking Oracea- it has only been 11 days and im seeing good improvement. Each day was noticeably better than the last, however, this morning I woke up with a few new bumps. Is this to be expected while still in the early stages of taking an antibiotic? Or is a more linear healing process typical? Trying not to get discouraged by the new bumps popping up. Thank you!!

I’ve been using skinoren and it’s been helping, but I got prescribed Acnatac (tretinoin) today for it because it hasn’t gone away and I wonder if I should do that instead? The doctor said I could choose whichever. I thought tretinoin made it worse?

I sticked with Skinoren and it all went away! I never switched to the Trentinoin because I was scared. I treated it with skinoren both in the morning and nighttime and it went away completely after about a month. A tip: do NOT use skinoren on wet or moist skin, if will burn and itch so much, so make sure your skin is completely dry. It might itch a little but nothing intense. Good luck 💐

But I spoke with my doctor and stuck with the skinoren and never started tretinoins or oral antibiotics because I had acne years ago and those meds was hard on my gut so I wanted to stick with skinoren and see if it helped. And it did but I know it’s different from one person to the other of course!

Just found this video and I’m so greatful for this. My 25-year-old daughter is really struggling with her face and I’ve searched everywhere for what is going on with her skin. She’s going to see a derm but in the meantime we’re going to try things from your list. One question I have is would Botox help her? I get it myself for regular maintenance but I’ve been wondering if it would help since people get it for the sweat glands I’ve heard? Thank you!

Yes, Tea Tree oil kills fungal infections. Regardless of what initially triggered it, Antibiotics are a major cause and perpetuation of this, because they can alter the microbiome allowing yeasts to proliferate on the skin like malassazia and candida which is usually the cause. Antihistamines play a huge role too because they affect H1 receptors that also control sebaceous glands which then don't secrete properly causing further skin drying and damage, that allows yeasts that feed off the fat of your cells and microbes to enter deeper into the dermis, then the immune system wants to react. It's a horrible viscous cycle mostly caused by psychotic MDs.

@@asherasator I went to a dermatologist and I was prescribed two creams. They are for yeast overgrowth? Ketoconazole, Tacromlimus ointment. If this doesn’t clear up then we have to treat the inflammation.

Hi Dr. Azi, I have had this for years and always pops up when I'm under prolonged stress. Doxi magically makes it disappear but usually comes back when I finish the cycle. I was wondering if fasting or the elimination diet would work. Since it's an inflammatory response wouldn't an anti-inflammatory diet help? Thank you

@@khaddisag I like Naturasil Sulfur Lavender but other good brands are Keep It Clean and Heritage Store Hot Springs. Wash once or twice daily. I would not moisturize because microorganisms like bacteria, fungus and Demodex thrive in moisture. I used a high % zinc diaper ointment to soothe dryness. You will absolutely get dry and flaky as the skin repairs. Another awesome sulfur option is DeLaCruz Acne Treatment/Sulfur Ointment. It's used as a mask, left on for 5-10 minutes and rinsed off. That was the number one most important thing in my routine, in my opinion. I still use sulfur soap daily and do a weekly mask. It's been 16 months clear! 🙂

are you kidding me? antibiotic course for lyme disease is typically around 21 days, serious BV - a week of antibiotic treatment - why would you suggest 4-6 weeks for something that comes and goes on its own and is mostly hormonal triggered? Antibiotics for that length of time can do a lot of damage to 1)the immune system 2)gut health - the two interdependent things that are crucial for recovery from any malady !

It's typically a low dose antibiotic where the primary affect is anti inflammatory, it isn't acting as a strong antibiotic. And yes it does take at least 4 - 12 weeks to clear. Trust me I've had this for 5 months and it hasn't gone on its own, im now on week 3 of antibiotics and its still there. I make sure to take a probiotic everyday

@@asherasator thanks for all the info. I do wish I'd never taken the antibiotics, even though are a first line 'treatment' for it. I've seen some improvement by just leaving it alone for the past 3 months, but it is still dry especially the perioral areas. I don't take antihistamines anymore but I'm now taking the combined pill as it keeps my cystic acne away, but I know this decreases sebum in the sebaceous glands, do you think taking the combined pill is also a bad idea?
