Hi and thank you for your compelling video about this condition! I have one question, please. If low progesterone (due to lack of corpus luteus genesis) is the cause of the increased frequency of GnRH secretion through the physiological negative feedback, doesn't that already imply ovulation not happening? I think I didn't get the primum movens of this vicious cycle correctly. Is it the hyperinsulinism? Thanks :D

@@Cryptokite019 In the normal menstrual cycle, when the graffian (antral) follicle ovulates, the egg is sucked up into the fallopian tubes whilst the remainder of the follicle at the ovary becomes the corpus luteum. Normally, the corpus luteum produces progesterone which exerts negative feedback on the hypothalamus such that GnRH pulsatility is decreased to reduce LH/FSH levels and prevent further ovulation. However in PCOS, the follicle does not ovulate such that the corpus luteum is not produced - the follicle remains as an arrested pre-ovulatory follicle in the ovary thereby owing to the polyCYSTIC aspect of PCOS. As there is no corpus luteum, there is NO progesterone produced to negatively feedback on the hypothalamus - as a result GnRH pulsatility increases. As fast GnRH pulses favours LH production, there is a high LH:FSH ratio. The high LH leads to increased androgen production via theca cells which cause hirsutism and acne, alongside insulin resistance which also amplifies further androgen production as it has co-stimulatory effects with LH. On the other hand, low FSH prevents complete follicle maturation as there is no inter-cycle rise/fall such that there is no selection of a dominant follicle. This is why PCOS women present with irregular menstrual cycles and anovulation.
