PrEP is only conditionally available on the NHS in Scotland. the nurse explained the reasoning to me as risk management. The thought being that, whilst PrEP is usually effective, it presents complications to the disease in cases where it is not effective (significantly harder to treat). Is this something Brandon is able to shed any light on?

I was a little disappointed at the opening 'bit' ... I love a little levity, but it sort of equates not being on PrEP with "Dying of AIDS", which seems like a disservice to all the hard work the LGBTQ+ community has done to reduce the stigma associated with being HIV+. It hasn't been a death sentence in years... PrEP is something I would really encourage everyone to discuss with a physician, though obviously that isn't always the easiest route. My previous GP wasn't even aware PrEP was "a thing" and my only resource for information was the internet.... which no offense internet, but you're a seriously awful resource for factual information, especially about PrEP. Some additional tips I would offer for anyone considering going on PrEP: (1) It protects against HIV-1 ONLY. This is the most prevalent strain of HIV in the United States, but other strains have been encountered (This is why they still recommend using condoms) (2) Truvada-Resistant subtypes of HIV exist, and PrEP will not protect you (This is why they still recommend using condoms) (3) Everyone's body reacts differently to ART, and at the end of the day that's what Truvada is (4) Before starting Truvada you will need to have a Kidney / Liver test, as well as an HIV test. (5) It's one pill every day. Not one pill when you remember, or one pill when you're about to get lucky. Every. Single. Day. (6) You can take Truvada with or without food, but studies in Europe suggest that food helps increase its efficacy. Food can also help reduce some of the side effects. (7) Speaking of side effects, upfront they can be pretty awful depending on how sensitive you are to the medication. Expect that first week to give you a nasty headache and likely make you have to poop a lot as those are the more common problems. (8) There are SERIOUS AND LIFE-THREATENING SIDE EFFECTS to Truvada. Your doctor will inform you of these but it's important to pay attention to how you feel and be proactive if you're exhibiting any of the dangerous signs (Tea-colored urine, clay-colored stool, etc). (9) You need to have been taking Truvada a minimum of 2 weeks before you are considered "protected". Plan in advance.

Also, if I might add a couple things: 1. PrEP is a preventative measure, but if you believe you've been exposed to HIV, PEP can be used. 2. While you can't die from HIV itself, you would die of complications.

PEP is another troublesome beast; at least insofar as access to it. I've heard so many terrifying stories of men and women who feared HIV exposure outside of a medical environment, and when they went to ER for PEP were met with a resounding "What is that? That doesn't exist..." One of the biggest benefits to PrEP might just be the protocols associated with maintaining your prescription. Physicians can only write a 90-day prescription and before you can be authorized a refill you have to have an HIV test completed to confirm you haven't become HIV+ while on Truvada. It's a great way to encourage awareness of ones status, and create a culture where regular testing is routine.

I'm just going to have a moan on our country's situation with PrEP (the UK - more specifically - England). Basically there was a medical trial before PrEP was supposed to be introduced by the NHS (so basically free) and everything was fine and then the NHS said they weren't going to provide PrEP because it "wasn't their responsibility" (but that of local authorities). Then a court case was taken up against NHS England and the outcome was that they are in fact responsible. Then there was so much backlash and bigotry in the media, saying that it's an excuse for people to have unprotected sex and that the funding could be used for people with "more serious" problems like heart conditions. They didn't realise that it costs more to provide treatment for someone who is HIV+ their whole life than to prevent it with PrEP. It was bloody horrific to see that people were so against it. It's insane that we haven't got PrEP here available on the NHS, it's shameful. There was an amazing story about this one ordinary guy, Greg Owen, who was basically solely responsible for a 30-50% decrease in new HIV infections in London in 2016 because of PrEP. There's a Buzzfeed article on him, really worth a read.

Based on how it's been explained to me, if someone has an undetectable viral load, their chances of transmitting the virus are so small that they're almost nonexistent. I could be wrong though...

Even if you are undetectable . . . meaning your blood has an undetectable viral load. . . .you can still maintain a very high viral load in your semen or anal/vaginal fluids. On the flip side. . .you might have a high viral load in your blood but almost none at all in your semen and anal/vaginal fluids. Regardless of the scenario. . . .even if there is only a 0.000002% chance of getting/transmitting it . . . . .it simply isn't a 0% chance. . . .it only takes one time to get HIV and it doesn't matter how undetectable, healthy, or what drug a person is on. When you get it. . .you wont just get 0.00000002% of HIV. . .you will get 100% HIV all the way and for the rest of your life. I don't know why people are so comfortable taking the risk and so comfortable justifying it to others.

If someone is pregnant and HIV+, there is a strong chance their baby will be born with HIV, but it's not 100% guarenteed. Using the right medications to lower the virus level in the positive person's body can greatly improve the likelihood of giving birth to a HIV negative child.

I love your videos and I endorse all the things you talked about in this video, but if I can be critical at all, I have to reiterate Robert Combell's first point. I know from watching previous videos that your intentions are never negative, and I understand that in order to promote some kind of protection, you have to talk about the risk. But I'm concerned about inducing too much fear around HIV/AIDS because it would enhance the discrimination against people living with HIV and let the stigma persist. So, I would appreciate a fact check video about HIV/AIDS on this channel :)
