Seattle Fire responds to a fatal multi-vehicle collision

At 3:28 PM on February 7, 2022, the Seattle Fire Department responded to a motor vehicle collision at Aurora Avenue N and N 96th Street in Seattle, WA.
Engine 31 from Northgate was first on scene and found the two, later determined to be three, vehicle collision. Additional units beyond the standard large response were requested.
Units on scene found four patients: two in an SUV, one in a landscaping truck, and one in a forklift. Three patients required extrication, which were all successful. Here, reports differ. What is confirmed is that a female in the SUV was transported to the hospital where she unfortunately died, a male also in the SUV transported in serious condition, and a male in the landscape truck transported by AMR in stable condition. What happened to the forklift driver is different in press releases from scanner audio. The audio states he was transported by AMR in presumably stable condition, though press releases state he was uninjured.
It was later learned the forklift operator caused the incident when he crossed the highway while under the influence. He has been arrested for DUI and now charged with vehicular homicide.
Rest in peace Jessica Valdez, mother of three, gone too soon.
