Welcome to Dr. Matthew Boes’s Shoulder Replacement Rehabilitation video series. This video is designed to help maximize your recovery following shoulder replacement surgery. The goals of our Phase I program for shoulder replacement rehabilitation are to protect the shoulder as it heals and to start gentle motion and muscle contraction exercises. Remember the following as you engage in Phase 1 shoulder replacement rehab exercises:
Remove your sling after first 10-14 days at your physician’s direction and begin light daily use of the arm at home
After removing the sling, start motion exercises [Exercises 4-9 in this video]
Start with Table Slides, progress to Pulleys, and then perform Stick Exercises as tolerated
Perform 1-2 sets of 20 repetitions of each exercise
Use sling for comfort when out in public for up to 4 weeks
Avoid arm motion behind the back for 8 weeks after surgery
Do not push off with operative arm (i.e. - when getting up from chair or bed) for 8 weeks following surgery
Start Isometric strengthening [Exercises 15, 16, & 17 in this video] at 3 weeks post-op as comfort allows
Hold contracted position for 2-3 seconds to activate muscle contraction
The following shoulder stretching and strengthening exercises, which are demonstrated in this video, should be conducted during phase 1 of your shoulder replacement rehab:
PENDULUMS
STICK - SUPINE FORWARD ELEVATION
STICK - SUPINE EXTERNAL ROTATION
TABLE SLIDES - ELEVATION
TABLE SLIDES - ABDUCTION
PULLEYS - FORWARD ELEVATION AND ABDUCTION
STICK EXERCISES - UPRIGHT FORWARD ELEVATION
STICK EXERCISES - UPRIGHT ABDUCTION
STICK EXERCISES - UPRIGHT EXTERNAL ROTATION AT SIDE
ACTIVE ROM - ELBOW, WRIST, AND HAND
GRIP STRENGTHENING
SCAPULAR - SHRUGS
SCAPULAR - FORWARDS ROLLS
SCAPULAR - BACKWARD ROLLS
ISOMETRICS - FORWARD ELEVATION
ISOMETRICS - ABDUCTION
ISOMETRICS - EXTERNAL ROTATION
Icing is very important after each therapy session, so be sure to place an ice pack or cooling pad on the affected shoulder for 10 minutes to help limit any irritation from exercise. If the shoulder joint is stiff, try using moist heat for 10 minutes prior to exercise to help loosen things up.
This video should not be used as a substitute for regular physical therapy visits and guidance from your physician. Only progress from one phase to the next based on recommendation from your physician. Most therapy regimens, particularly after shoulder replacement surgery, are based on a healing timeline. Progressing too quickly, even though it may feel okay at first, may put you at risk for injury. Regular physical therapy visits are valuable for giving you feedback on your progress and helping to ensure you are doing exercises properly. Thank you so much had complicated left rotor cuff surgery where is massive bone spur removal. I think I'm still at stage one. Doing very well. Hopefully my insurance will approve a second 12 visits to PT. If not I have you! :-)

I was using the pulleys to get overhead movement at week 4 . however I am a fairly fit 37 yr old

Thank you so much Dr. Boes. I start these exercise at week 3 and 4. I am 70 years old and I have a total reverse shoulder replacement. At week five, I am doing almost everything. I take it easy.. I don't push to high limit. Thank you again and again.

@@skimpyimp2725 I didn't have any pain from the first minute I left the recovery room and until now. Maybe I was lucky. Also, I eat my full meal after one hour of the surgery. I stayed one day in the hospital. Of course, I had the nerve block and lasted for at least two days.. may be three. I took pain killer according to the anesthesiologist advice.. Called my doctor to let him know that I don't have pain.. he suggested to have the pain killer every 8 hrs, then every 12 hrs, then one pill every 24 hours. I stoped taken any pain killers after seven days. Hope my reply is clear and helpful.

This will help me immensely; my PT is not progressing as I have hoped; think I will save some money and just follow this regiment. I am at 9 weeks post total shoulder replacement and still can't raise my arms over my head without a pully. Hope this regiment speeds up my progress.

Being 80 years old and breaking the hip, wrist, arm and several breaks in the shoulder at the same time these beginning exercises are way to advanced at that stage of the game. Just the pain is beyond control at first, the fear of another fall, the flashbacks the helplessness all play a roll in recovery. I was in rehab 3 weeks, could walk 80 steps with a cane, could not use walker due to the reverse shoulder replacement, no support from the right hand or arm at that time. Anyway I make at least a baby step everyday but no way am I going to ever be able to accomplish what that body builder displays. It's almost an insult to me personally when I'm working so hard at my pace. Please don't minimize the pain because taking some medicines can also be addictive and become an added problem for some. Be a bit more realistic if you really want to help. If you have not experienced this surgery get the facts from those who have. I was almost a full time volunteer in my community before this happened, so I was a very active motivated old lady and pray I will be again. Lee

had my ovo partial shoulder angioplasty feb 18th it has been almost 8 weeks and im still struggling. I do think these videos should say week 2 to 6 instead of week 0 to 6. this video is very good i am using it religiously 2 times a day i cannot for some reason get my arm over my head with an anterior lift. ill keep doing these for a few days and then will start phase 2. I underestimated the amount of time to rehab this is the hardest work i have ever done and ive had 2 acl repairs and a partial hip replacement

I agree, the rehab for shoulder replacement makes my hip replacements look like a walk in the park - ain't arthritis grand!

Pete, I hear you. I had an easier time with my first reverse shoulder. I did start this guys programs about four weeks post op and quickly advanced. By my fifth or sixth week post op I could get closer to 180 degrees. Now my second reverse replacement is going slower. Started phase 1 at week 5 post op and I cannot get past horizontal with that lifting w/stick exercise, also the table slide I have to stop much earlier. So I go slower, do not push past that discomfort point and BE PATIENT. I was playing softball with senior citizens five months after my first shoulder. These are great programs if you proceed cautiously and listen to your body. Ice, tylenol, rest, hot infrared sauna are great. I also got cold laser therapy from my chiropractor. Helped my recovery alot.

I had a complete shoulder replacement on my dominant side 6-months ago brought about by 47-years of construction and 25-years of competitive rowing. I'm 71, an active hiker and very assertive with my hiking poles. No overnight lasting discomfort. I'd like to start rowing again as well as do pushups. I've been released by the surgeon. He was favorably impressed with my strength (purple bands on 1st day of PT) Any restrictions that come to mind? Thanks

i did all of this at 64 but i always made sure to take my pejcoset before doing them , cannot imagine what I would have done without this medicine! and i did not get addicted , it was only used for pain and then i didn't nee it anymore.

Thanks for the information . Would I to know if I would be able to workout again because of my job, working in the hospital field they said I would have to look for another position after my surgery due to the type of work I do.

its slightly unbelievable since the person doing the therapy, isn't making the faces I make during my stretches. I think I overstretched because now my shoulder clicks when doing the pendulum

I do construction and i"m 58 years old my doctor is recommending a full shoulder replacement, and is basically saying once this is done I can't go back to do my job, because most of my work is overhead........so now what

Are the rehab exercises for Reverse Total Shoulder Replacement the same as for a regular Shoulder Replacement? I would think there might be some different restrictions or limitations. Please clarify. Thanks.

I don't know but that's what I'm trying to find out by googling it. Google tells you it's for reverse shoulder but then when you actually get to the videos, it simply says "shoulder replacement"

Definitely different!!! Everything is reversed...... muscles will be working differently. Give yourself time and listen to your doctor!!

@@sandrasatterfield4432 I’m having my 1st reverse replacement surgery. The same shoulder has had 2 replacements. I’m finding that my insurance doesn’t seem to want to cover the PT needed. I guess I’ll be watching these videos for my PT. Do you agree with the exercises shown? I hope you see this.

@@kennyhiggins7806 I just had the total reverse replacement surgery 4 weeks ago. These are the exercises I am doing now. The first couple weeks after surgery you need to heal and do very low key activities such as isometric wall presses, maybe the pendulum, and keep moving your elbow to prevent it from freezing up. My therapist moved my shoulder for me. My homework this week, starting 5th week after surgery, includes pushing the shoulder blades back, laying on my back and clasping my hands to raise my arms up as far as I can (non -surgical side does the lifting), the video cane exercises, and some of the wall isometrics. I have done the pulley exercise during therapy as well. The therapist puts heat on my shoulder for 10 min. before I start and then ice for 10 min. at the end of the session. Good luck.

I am at the 4 week stage following total reverse replacement surgery. My therapist has me doing the exercises twice a day with about 10 reps of each. He said I can add another session if I want.

I am in week 5 and can lift my arm above head with the help of the cane or intertwining my hands together and raising. My right hand does all the work for the left hand. I started this a week ago. I can NOT lift my left hand (operative side) alone, nor would the therapists let me! I am 65 and in OK shape.

ive just had a partial replacement. why is there no videos from people who have actually been through the surgery ? a model athlete who is clearly less than 30? this is just not real

You wanna know exact correct posture you trying to achieve.. Obviously you can't just raise your hand completely in 1 st try, but that should be the goal..

Im am a 30 athlele, just got my shoulder replaced, i am actually pleased to not see a 80 year old model doing these exercices for once. If anyone has gotten their shoulder replaced and is 40 years old or less , Id like to get in touch. Or get a forum recommanded.

I disagree with much of this. Doing overhead with the bar in the first six weeks (assisted) is a pipe dream for most people - even laying down. If you're a 22 year old triathlete, then perhaps you could get away with this - everyone else....naahh. This information needs to be presented by someone who is not a weight lifter and body builder - preferably someone who has actually DONE this.

I'm 66 years old and was able to do the overhead extension without assistance at 4 weeks. Maybe you're doing your patients a disservice to say they can't do these unless they're a 22 year old triathlete.

I am 65 and I started using the cane to lift my arms over my head at 4 weeks. My operative side could not do it alone, nor am I allowed that movement, but with the right hand leading the way, I have no trouble doing this. I raise as far as I can and hold for a few seconds. Repeat 10X. I am only in OK shape.

sad. that seems very pessimistic. Most people want total recovery after replacement so they can resume the physical activities they enjoy. If you don't do the therapy, then you will be like my husband's friend who can't even use a hammer

you're correct...the timing of 0-6 weeks is pure fantasy and possibly dangerous...I'm 10 days after surgery ad the is no way in hell I could do any of these things except the pendulum swing and part of the table slide...bure BS

I'm working with a 70 year old woman right now who can do all of this 3 weeks post-op. And supine bar flexion is far more about range of motion than it is strength so I don't think the weight lifter comment applies.

This doctor is nuts. I am 54 years old and in excellent shape. I had a total shoulder replacmnt 17 days ago and other than the pendulum swing to help with circulation none of these should be done until at least 4 weeks and that is if you are in great shape and even than go slow. This video should e removed from You Tube!!

I just had a total reverse shoulder replacement 4 weeks ago. I definitely agree that these exercises are geared more for 4 weeks following surgery. My in-house OT after surgery consisted of a few isometric exercises similar to the video and elbow bending to keep the arm moving. The therapist also did several types of movement with me to loosen my arm. I started out-patient OT last week and the exercises in this video are very close to what I am working on. My therapist wants me to raise my arms as high as possible with the cane, but I am not near where this video is showing. But I have enjoyed this video and have watched it several times to help review exercises I can now do. I appreciate the help! Thank you.

I had rotator cuff surgery at 65 and I can’t come close to doing half those at 4 weeks out. He’s nuts. No way should people flow his advice.

this whole video is LOL...I had a total shoulder replacement 10 days ago and I promise you, there is no way in hell a person could do most of these exercises so close to surgery...maybe (maybe) a few weeks in...I'm not a slug by any means, but there is no way in hell I could do what this guy is doing on the video other than the pendulum swings
