20+ Years as an OT doing occasional post op shoulder rehab. This is the best video on the subject. Always increase flexibility (and muscle tone) progressively WHILE decreasing pain progressively. IF PAIN is staying the same or worse increasing with ANY exercises (over weeks), your MD/Therapist should be fired. There's no need to hurry the process.

I am 6 weeks out of surgery which was on 12/7/2023. I have not started PT as of yet but am lightly and carefully proceeding forward. I was able to complete all that was presented in this video today with little pain and able to extend and retract with the full range of movement presented today. I look forward to other videos and my PT will actually start 1/30/24. According to everyone involved with this I am being told it is a process and do not rush this process. I have always said to myself if I try to be a Superman and push through this process I will end up a Stupid Man in the end.

My issue is with lymphedema in same arm as my reverse total shoulder replacement. The additional weight of my arm makes this so hard to do ; but hanging in there .

2 weeks post op typically you are just getting over the inflammatory phase (phase 2) of healing and going into phase 3 (proliferative phase). This is why exercise is more structured in post op rehab. We want the tissue to be ready to meet the demands of exercise without risking further tissue injury...2 weeks post op would be to soon for some of these exercises, some of them may be okay to start depending on your individual healing process!

I'm 8 weeks postop and actually doing well with the therapy. Problem is, the surgeon refuses to be part of the "team" and has given absolutely NO instructions except for NO RESTRICTIONS, since day 3 postop. Insulted my Physical Therapist, too. So we are winging it. Thanks for this video, I'm so glad you made these exercises available.

@@drbeddoedpt Yes, I think so. We have done some of these so far. I can use my cane to lift my arms up toward the ceiling and then extend back over my head. We haven't done the dowel in front and lifting up. I will show that to my Physical Therapist. Yeah, the surgeon is pompous and arrogant. I'm done with him. Thank you so much for responding!

I liked my surgeon but at 3 weeks out I e only talked to him 5-10 minutes total. The PA does all initial meetings and follow up.

Is the patient doing no PT for the first 6 weeks post-op. Shoulder would be frozen and PT starting at 6 weeks would be excruciating

Hey. I got a question for you. I really really hope you're going to reply cuz my cases pretty rare. I am a bilateral lower limb amputee. I'm missing my left leg above knee. I have the whole femur. And my right side is a hip disarticulation. I've hurt as a baby in a car train accident. Been very active my whole life several black belts eggcetera. Now that I'm 47 my rotator cuff and my right shoulder is completely gone. And doc says I need a reverse shoulder done. It's scheduled for January 7th with Dr Miguel Ramirez at Johns Hopkins trained doctor. Would you approach rehabbing a wheelchair-bound man difference then you would a normal man not in a wheelchair when it comes to this surgery and Rehab. I want to make sure I pick the right therapist around me. So I'm reaching out the others that might have encountered this issue already. I really look forward to your reply as this is something that's going to be a very hard recovery for me and I could use all the advice I can get. Thank you hope you reply

Hey, thanks for the question! The healing process will be similar to a non wheelchair bound individual. It will still be protocol driven due to tissue healing being similar patient by patient (unless other comorbidities are present) and the surgery you will be undergoing being performed relatively the same way every time. But later on into the recovery process we would want to address your specific needs being in the wheelchair and using your upper extremities and supporting musculature a lot more than that of a non wheelchair user. Any Physical Therapist worth anything will be able to address those additional adaptations to your specific needs! Best of luck with your surgery and with your recovery!

Alys consult with your physician prior to any new program, but these are the typical exercises one would be giving to a patient at 4-6 weeks unless their are other issues/impairments your physician has talked to you about!! But again, consult with your medical team!

tanya we are stretching our bicep tendon/muscle. We want to have our elbow at or close to the end of our knee so that we can extend our elbow as far down as we can and hold for a nice stretch!

After 8 weeks post op I am so disappointed in my progress. I was in a sling for 6 weeks...only allowed to do shoulder circles. I just can't seem to get below a 5 on the pain scale. I'm going to try to do some of these exercises in addition to what I am doing with PT. I also have a golf ball fluid sac on my elbow. Could this be impeding my progress?

Sorry your progress is going slower than expected. It sounds like you have elbow bursitis in the olecranon bursa. It shouldn't effect your shoulder rehab and progress. Some orthopedic surgeons will direct their patients on a modified program based off of how the surgery went or what type of "repairs" were necessary. Check with your ortho surgeons office to get more clarification!

I'm 70 yo & 3+ months post-reverse shoulder arthroplasty. My elbow is completely stuck! I can't make the "L" bend like "goal post" away from body at all whether laying down or standing. What should I do?

I had the reverse shoulder replacement, coming up on 3 weeks, I've been out of the sling after 3 days, and have been doing a range of motion exercise 3 days a week since week 2, going to guess the L shape you have most likely you left the arm in the sling a lot, per doctor orders, I'm guessing, Get the range of motion going, simply sitting in a chair taking that L shape bend and with your fist going to the floor straightening it out, stop at the point of pain, go back to the L, like a seated dumbbell curl with no weight, do it for 5mins, 3x a week , then ice, good luck

Ok! My orthopedic surgeon is totally worthless in my rehab. A fabulous surgeon but terrible rehab ! I have followed several orthopedic exercise specialist like you for each of the stages. I have just completed stage 1 or 2 and I will have six weeks in one day. Before surgery my left shoulder was in good shape and strong. So I am ahead of others and I am 74. I had complete reverse left shoulder replacement. Looking for week seven exercises! Thank you so very very much

I am Post Op from 2/10/21. Left Total Reverse. I have a Bump/Osteophyte on the top Clavicle/Acromion on the top of my Left shoulder. I asked him before the surgery, it hurts, are you going to remove this? He said, IT WILL GROW BACK???? He refused to do anything about it and now it is impinging on my PT ability with movement.? It hurts even when my bra strap lies on it? Did research for 2 years trying to find a "good" dr. well I don't know. I have pain and just diagnosed with Cellulitis this weekend 5/23/21-Sunday a Walk In Clinic, she took one look and said cellulitis, but I will confirm. Saturday morning large 2 inch X 2 inch round red hot bump on my arm. ? I am 64 was always very active, have a Total left Hip, 6/16/16, CLICKS and EVERYTIME I COUGH iI COULD FEEL THE PROSTHESIS. IT DOES NOT FEEL RIGHT. SAID BONE ON BONE NOW RIGHT HIP.. I AM A HOT MESS.

Sorry to hear about your pain and everything you are going through. The bone growth (osteophyte) can be considered "common" but it depends were the formation is at to determine if it is impedeing movement and causing disfunction. The clavicle-acromion joint doesn't move a lot, and if the formation is anywhere but the acromioclavlicular joint surface it should not cause to much, if any, disruption/pain with movement (but get more imaging if needed to determine location). The Cellulitis is a concern but most likely unrelated to your clavicle problem (consult with your PCP on this). With the total hip replacement and clicking I, or another competent DPT, would need to assess you in person to determine if it is effecting function or not. but strengthening in and around your hip and pelvis is most likely a solution! Please consult with your physician or health care specialist before trying anything new. Sorry dear, I hope that you are able to find the solution your are looking for. Please see a Doctor of physical therapy in person for a better assessment and I hope you are able to get back to the functionality you are looking for!
