Great video. Thank you very much. In the 21st century a Total Hip Replacement should only be performed using the Direct Anterior Approach. The anterior approach is more technically demanding for the surgeon, which is why many surgeons have not yet switched to this approach. This approach is considered a minimally invasive surgery to replace the damaged hip while avoiding nearby muscles and tendons. This approach causes less pain and results in a quicker recovery. There are generally no hip precautions necessary after the direct anterior approach, so motion is not restricted. Patients can return to work when they feel comfortable. This typically takes about 2 weeks. As a 75 year old male and an engineer in the power industry, the surgery was performed as an outpatient and I was home that evening on January 9, 2024. Used a walker for the first two days and then a cane for the following two weeks mostly for protection from falling and to relearn how to walk without a limp. Was active and back to work the following week and playing pickleball at 8 weeks. Was cleared from physical therapy after 4 sessions. Was already doing the exercises and riding my stationary bicycle before the sessions with the physical therapist. My biggest recommendation is to do at least 2 or 3 times a day the associated physical therapy exercises for several weeks prior to the surgery and for at least 8 weeks post surgery or in my case until I got back to my normal fitness activities. The ankle pump exercise should be performed even more frequently. I still do them frequently throughout the day. Also ice pack pads will be your post surgery friend; they should be used frequently on the area of the incision for the first 2 weeks after surgery.

Can't thank you guys enough for making these videos. It took me practically 4 weeks to get physical therapy started and this program helped me to get going on my own. My surgeon and physical therapist, when I eventually got one, both commented on how well I was doing. The credit goes to you. Thanks again .

I have been following Bob and Brad for a few years. Initially it was for problems with my back, but later for other things. Now I'm back and looking for exercises for after hip replacement surgery. You guys have not let me down. This isn't my first hip replacement, I had my right hip done 9 years ago when I was in my early 40's. I had developed pain in my hip. An orthopedic surgeon told me I had some thinning of the cushion in the joint, but should be fine for another 5 to 10 years. I was back a year later because my leg kept buckling on me. The cushion was completely gone. It was just bone on bone. My left has now done the same thing. I am just over 2 weeks post surgery. A few days ago I had a visit with the surgeon and a physical therapist. Everything is great. I already had ditch the walker most of the time. I shared with them my need to strengthen my legs as much as possible before August. I am a teacher associate and will be working in a building that is 4 stories so I will be going up and down stairs ALOT. The therapist gave me some more exercises to add to what I was already doing. I came here to see if you guys had something more that I might add. I'm glad I did. You have all the ones my therapist had given me and more. I will be adding them into my rotation. Thanks again for all the time and effort you guys put in to these videos. You are so very helpful.

My comment is, after my hip replacement, the hospital therapist, had me walk over to his office, told me the fastest way to get back, was to walk stairs, he had me walk up an down on his mock up stairs in his office. I went home started walking up an down, the 18 stairs in my home on my 3rd day home, walked up an down the stairs each day. The home physical therapist, decided to discharge me from her care after 9 days. She said I made amazing progress. Im 66 years old, have been a welder since 18 years old. I never told her I wasn't doing her exercises. Which I thought was a waste of time. I attribute my recovery to walking the stairs every day once in the morning/afternoon/evening, that I'm 66, had a great desire to get back on my feet, and past work activity. I'm not an athlete, I'm not overweight, I'm not physically special in any way. Just an average guy, that wanted to get my life back to normal, asap.

Also, I used the walker, when not going up an down the stairs. But I suggest using a cain in each hand, every time you go up an down the stairs, for safety, for balance, and they really help the first 2 weeks with my right hip, that forgot how to move. I used the cains every time I went up and down the stairs for probably 5 weeks. You sure don't want to fall and fk up your new hip. 3 weeks after my hip replacement on 4-19-2024 I felt great. But I do not think I am able to go back to being a construction welder yet, its to much of a physical job on the legs. It is now October 5th that I text, but I can ride my bike and jog now, but I am still cognizant of my self imposed limitations, cuz I don't want to fall or fk up my hip. A hip redo is a big fking deal, more costly, and I figure much more rehad time. Looking forward to getting my left hip replacement. Osteoarthritis. is some bad shit to deal with.

Can you do these if you had a Calcar fracture during surgery? And do you do all of these the first week?

I am 16 days post anterior approach. My PT has me doing all the quad exercises, marching, and step ups with ankle weights. Full squats without holding onto anything. I have to take prescription pain meds, which I am running out of to get through a session. My knee is killing me. Very sharp pain upper inner knee cap. She keeps upping the difficulty every session bc I am doing ‘so well’. Except it doesn’t feel all that great. I can push through it, but I am hitting 7 on the pain scale doing some of this stuff, with Dilaudid and tylenol on board. I am then so sore that I just lay in my recliner for the rest of the day. Is this normal? I feel we are going too fast. Am I being a baby? My knee hurts with every step. It is the limiting pain, not the hip. How do I fix it? I can pinpoint exactly which exercises are making the knee irritated. I have no underlying knee pathology and it didn’t hurt before surgery. Advice?

So sad you were pushed without guidance on what to do after. Rule of thumb for pain is there's good pain and bad pain. So I've had the anterior hip replacement and the swelling due to it is messing with my knee joint. Always treat pain as Gods warning to do something to fix the pain. This reply is three mn out hope you've addressed the pain and are progressing well.

How did it go? I will be doing back to back hip replacements. I wanted both done at the same time but they said at 54 I was too old sadly. So right hip in 2 weeks, left one in 3 months. This time next year I will be sitting cross-legged under my favorite tree. I can't wait.
