Welcome to Dr. Matthew Boes’s Shoulder Replacement Rehabilitation video series. This video is designed to help maximize your recovery following shoulder replacement surgery. The goal of our Phase 3 program for shoulder replacement rehabilitation is to improve strength and functional use of the arm. Remember the following as you engage in Phase 3 shoulder replacement rehab exercises:
Conduct stretching exercises until you achieve full shoulder motion (as compared to opposite side)
Stretch 2-3 times per day as needed
Progress strengthening exercises from use of exercise bands to dumbbells
Conduct 1-2 sets of 15 repetitions each exercise, 3-4 times per week
Start light, functional sports activity if applicable [Exercises 16 & 17 in this video]
Release to full activity per your physician’s approval
The following shoulder stretching and strengthening exercises, which are demonstrated in this video, should be conducted during phase 3 of your shoulder replacement rehab:
STRETCHING - WALL SLIDES
STRETCHING - CROSS BODY
STRETCHING - EXTERNAL ROTATION
STRETCHING - INTERNAL ROTATION
DUMBBELL - ELEVATION & ABDUCTION
DUMBBELL - EXTERNAL ROTATION
DUMBBELL - INTERNAL ROTATION
DUMBBELL - BENT ROW
DUMBBELL - BENT SIDE RAISE
BANDS - DIAGONALS
BANDS - PUNCHES
SCAPULAR EXERCISES - THUMBTACK EXERCISE ELEVATION & DEPRESSION
SCAPULAR EXERCISE - STANDING DUMBBELL ELEVATION
SCAPULAR EXERCISE - THERABAND RETRACTION & DEPRESSION SHOULDER LEVEL
SCAPULAR EXERCISE - THERABAND RETRACTION & DEPRESSION WAIST LEVEL
FUNCTIONAL EXERCISE - TENNIS SWING
FUNCTIONAL EXERCISE - GOLF SWING
Icing is very important after each therapy session, so be sure to place an ice pack or cooling pad on the affected shoulder for 10 minutes to help limit any irritation from exercise. If the shoulder joint is stiff, try using moist heat for 10 minutes prior to exercise to help loosen things up.
This video should not be used as a substitute for regular physical therapy visits and guidance from your physician. Only progress from one phase to the next based on recommendation from your physician. Most therapy regimens, particularly after shoulder replacement surgery, are based on a healing timeline. Progressing too quickly, even though it may feel okay at first, may put you at risk for injury. Regular physical therapy visits are valuable for giving you feedback on your progress and helping to ensure you are doing exercises properly. I had a reverse total shoulder replacement in July 2022. I wasn't ever sent to physical therapy so there are things I still can't do. I will be doing these exercises praying that I will be more independent and able to do the things I used to do.

Heading to PT in a second actually but I'm in my tenth week post op-reverse total shoulder replacement and feeling good about where I am! My instructors said I was ahead of the game right off the bat because the surgeon held the bar high from the minute I came out of surgery! He had me doing tons of stuff and it did pay off as much as it hurt and still will as I'm not fully healed yet of course but yeah, great video for sure!!!

Are the exercises in these series also appropriate for a reverse total shoulder replacement? I had a proximal humerus fracture 5 months before surgery. It didn’t heal properly. Thanks!

The shoulder replacement is all about eliminating the pain and lifting the arm above your chest and head. I had a rTSA six months ago due to severe osteoarthritis and frayed rotator cuff damage. The year before the surgery occurred, I had been limited in exercise and weight lifting, and lost quite a bit of mobility and strength. Since the surgery, I continue gaining strength lifting light weights, but the shoulder will never be like the original equipment before the onset of the osteoarthritis and loss of cartilage. The general rule on lifting weights with the effected shoulder is no heavier than 50 pounds based on my doctor, physical therapist, and what little I could find on the internet - this is primarily due to avoid ripping the screws out of the scapula (I assume). I also avoid lifting heavier weights above shoulder level including no pull ups so far. So final word, the surgery is about the pain and mobility. Create a due diligence checklist to ensure for the best outcome in regard to hospital, surgeon, physical therapy, etc. good luck.

So I read your comments I had shoulder replacement do to car accident my shoulder replacement don't have screws.but now I'm kinda worry about things I love to do are pull ups chin ups safe to do It sucks because I was big time weight lifter.love to work out .

@@patiorio1538 Assuming your shoulder surgery was probably the standard shoulder replacement surgery, while my surgery was the recently FDA approved rTSA (reverse Total Shoulder Arthroplasty), and I am in the old man category. You don’t have screws, but pegs are used to fits the artificial socket into your natural bone. In any case, I would talk to your surgeon on your risk factors and research scientific surveys and even antidotal stories such as mine. I made a decision to lift light weights, and focus on other forms of exercise. Repairing our shoulders the second time around, at least in my situation is a real challenge, so I am being more cautious. As I said in the previous post, the original equipment was better than the artificial joint. Also remember, if it hurts, don't do it. Good luck.

So the one exercise that this lady shows me PTA it really hurts do you have to go through that pain to get better.?

@@patiorio1538 that question is not in my pay grade - you should talk to the physical therapist (PhD) that set up your rehabilitation that the PT Assistant is suppose to be following - and the PT is usually given instructions or the rehabilitation protocol by the surgeon. Ask questions always, and research from professional sources.

Whenever you make instructions for therapy, make an indication which limb had surgery. Put a bandaid on the man’s shoulder or an X made from some adhesive tape, or SOMETHING! It would surely help!

I know this video old I'm in 4 month shoulder replacement I feeling pretty good I was wondering can I do push ups not now obviously. I'm talking about year and half from now to much stress on the shoulder replacement ? I If you can give me your feed back I would appreciate it

I have had a Total Shoulder Replacement. These are good exercises. Unfortunately, after this surgery you will never have the strength you had before the surgery. It should be a LAST resort. Put it off as long as you can.

Be carefu lwaiting too long. Bone on bone wears the joint and can affect the success of the surgery. This joint is smaller than the knee and the hip. Less mass to work with.

I agree. had shoulder replaced 8 years ago. Pain free, pretty good mobility, but no strength.

I am 63 years old now, and living in The Netherlands. My right shoulder had surgery in march 2019, my left shoulder in may 2020. Both are totally replaced. Had long recovery, but now I am working out in the gym again, wit plenty of strength and motion. Sometimes there are litle pains and stiffness, but I am completely happy with it.
