The guy also had 2 felony DUI’s, 3 additional misdemeanour DUI’s totalling 5 since 2007. He spent time in jail for getting DUI’s while on probation for previous ones. In 2019 he was charged again for violating probation and refusing to attend court ordered AA meetings. It would have been criminal to issue him a license, his student license should have been revoked and his reckless indifference is mind numbing and made his outcome inevitable.

Revoking a piece of paper is meaningless at this level of self-destructive behavior. Regulations are for people who obey them. You can't legislate proper behavior and there is no cure for people who never left their childhood behind. Well, actually there is a cure. A close call that scares them straight. Souls on board have to survive to reflect on the terror.

Fat wallet, big ego syndrome. Would have been easy for him to hire a commercial pilot who would fly him around, but he opted to go to the danger zone instead. Tragic outcome but not a surprising one.

Had a similar thing happen to me. I was flying my husband and myself to Vegas in a 182, just like these guys, when we ran out of fuel shortly leaving the airport out of New Mexico (don't remember the city). Gary died as a result of the crash. Sad. But it didn't stop me from wanting to get back in the clouds. Now I'm remarried in an even better marriage. Almost like the crash was a blessing in disguise!

The greatest compliment I ever received was when, just before my PP check ride, one of the CFI's where I was flying shook my hand and said "Your one of the most cautious Students I've ever seen. You should have a long career ahead of you!" Made me feel like I was indeed LEARNING and understanding the responsibility that a Pilot takes on.

Back in the early '90's I knew some guys from Piper and asked about all the Malibu crashes that seemed to be happening often. They said "Guys buy a million dollar 'All Weather' aircraft and think they can fly through storms". More money than experience or competence.

Well said. We call them Dr and lawyer killers. Train in a Cessna 150/172 get their license and then buy an expensive sleek sexy fast airplane. Malibu’s are excellent airplanes and safe if you fly them by the numbers. But if you get behind the plane there is no margin for error and you can’t fix it quick enough. Your skill and decision making need to be up to the task.

Was going to buy a Malibu but I learned that the landing profile was a bit different and because it's high performance I didn't feel as in control. So I just stick with my Arrow 3 which is fast enough, reliable, and basically an extension of me.

@@Thinks-First100% - smart move - I have owned an Arrow III and flown a Malibu. The Malibu is a very “restricted” plane - so many “tight”limitations - given its ability to fly in flight levels. I would take the Arrow III any day. 👍👍👍

@ an arrow is a great airplane. Have that main spar checked and Eddie current tests done. Flew in a 140 from Florida to Oshkosh and upon return the annual was done. In one year the left side of the spar box was so corroded it crumbled like paper in your hands. They’re getting that old.

Chad almost crashed on my house! His plane crashed less than a quarter mile from where we live, we heard the crash and thought it was thunder from the storm. We were the ones that provided the FAA with the security camera video of the plane . This is such a terrible tragedy, this man ended up leaving his kids without a mom and a dad.

A pilot for 40 plus years and still consider myself a student. Each debrief is an appreciated lesson. I stopped flying in IMC a dozen years ago because I was not flying enough to maintain an acceptable (to me) level of competence. I'm continually astounded that with all the weather information now available we pilots still find ways to refuse to face reality and defy the laws of physics.

You can bet that this pilot had never flown under the hood so probably had no concept of what techniques apply to flying in turbulent weather. There is nothing more sobering than unusual attitude recovery with partial panel under the hood (unless you are hardball IFR) during training. Get there -itis is a killer. One can have an enjoyable time stopping short safely, not that this guy was safe in any way. Thank you, Hoover, for this debrief.

Pilot for 25 years and I feel the same way. I still hire instructors if I haven't flown in more than 6 weeks. And I bring safety pilots on board to keep my IFR skills up to par. I'm SEL and Rotocraft rated.

I'm 57, i always said "the only way i would fly is if i built the craft and pilot it myself". Just from what you said i'd fly if you were the pilot. But still i have to build the craft. which you most likely wouldn't want to even get in it , let alone fly the thing. But this is a first for me

In Chad's case I think something like this was pretty much inevitable because 'the rules don't apply to me'. The rules and safety mean nothing if they are ignored. This is yet another tragedy for which there is no excuse.

One of my fav stories is a kid lived next door to a commercial airline pilot. The pilot would take him flying on the weekend once in a while in a Cessna. One day they are at the airport and the pilot gets the plane completely ready to fly and at the last minute he says “I don’t like these clouds, maybe we will go up next weekend” and parks the plane and takes the kid home.

My supervisor used to take me flying on our lunch break. He rented the plane for that time, I made arrangements for someone to cover me for the extra time. Once when we got to the airport he looked at the windsock, talked to someone to get a wind report, and decided not to fly. We were both disappointed, but I'm here to tell the story.

Great stories. I took my 6 year old to the airfield to fly years ago and there was a minor tech issue with the aircraft that I wasn’t going to accept. When i told him we weren’t going flying his eyes welled up and I so I shared with him the words that I’d heard from wise old pilots…”better to be down here wishing we were up there than up there wishing we were down here”. It’s an oldie but a goodie

As a pilot you must be thinking ahead of the airplane at all times! Example; even though your flight is going according to plan you must be thinking of irregularities such as where will I land if the engine quits or were will I land if the weather goes bad ect .

100% correct. Im sure you've met them too. This is his profile. A short, high- strung contractor that drives a new F250 that has never anything in the bed. He spends the majority of his time lurking around the halls of government buildings, dropping names, and trying to schmooze any official that can benefit or rule in favor of his business. Thinks everyone believes his fake BS and thinks every secretary wants him. Not an unforseen end.

As a young CFI I was asked to give instruction to a pilot who was on probation by the FAA because he had flown into conditions where he got stuck on top of a cloud layer that closed in all around him. He was a VFR pilot and should have checked the weather before he got trapped. The FAA said if he got instruction and obtained his instrument rating they would let him continue to hold a pilot's license. After talking to him I felt that his anti-authority attitude was too strong for me and said I wouldn't fly with him. Another instructor in our club took him on and he eventually got his instrument rating. a year or so later he crashed in WV due to fuel exhaustion in an area of widespread very low IMC , killing himself and his two daughters. Unfortunately, there is no test for "arrogant malcontent" so guys like this can slip through the cracks.

I do believer your story, but it seems very unconventional for the FAA to punish someone they way you explained for getting trapped on top. At one time or another, most all of us have made a mistake of equal value, and were lucky enough to learn from it and fly again. Can you elaborate a bit more on how his situation unfolded? I'm super curious about this one, as I've seen bigger "violations" overlooked.....thanks!

I am 80 a stopped flying in 2015 after open heart surgery. Did not need the FAA to pull my license. Common sense is uncommon in this world. Fly within your limits and be safe.

That's a terrible story there. As you indicate in your comment, a pilot certificate or an intstrument rating is not a solution for unsafe behavior, it just opens the door for even less safe behavior in the future.

Hoover, you missed something very important. I’ve been a GA pilot 50 years and in every accident of this “tragic” variety there is one constant. MANY OTHER PILOTS AND AIRPORT REGULARS PREDICTED IT. The problem is… Pilots are reluctant to report illegal and reckless behavior of other pilots. And sometimes when one does report it, the local FSDO just pats themselves on the back for giving some advice and remedial training and “keeping the pilot flying”. This is nuts. We pilots who are at airports, the mechanics who work on planes, and others who just hang around must actively report illegal and reckless piloting. America is not a “snitch” society, however, certain bad actions are serious enough that citizens are obliged to report them. Errant pilots should be regarded with no more respect than a child molester. A danger to society and to innocent others. Please add the admonition to your Debriefs of this type; “Report errant pilots before they kill someone, or its as much on you as on them.”

I have always believed that the aviation industry should make their aircraft records public and public. Rather than allowing the people who fly the aircraft to operate in a vacuum, and the relevant technology should be made public. There shouldn't be too many secrets and there shouldn't be too many power gaps.

Why I watch your videos one might ask, I have flown for years, cert. in serval formats now have had some health issues leaving me unable to pilot any longer. Watching your videos makes the decisions that were made to stop come to life to be safe for all. He was not smart enough to get a weather briefing. When talking to the weather people he would have shown his ignorance because they would have asked him specific questions which would have shown how inept he was.

I’m a CFII, and flying for 40 years, still active. I have 3000 hrs plus dual instruction given. I have had at least 3 students, just like this guy. While I get most comments here, I took the opportunity to try and coach/teach these folks about their issues and anti-authority attitudes. What works as a Boss, will get you killed in a plane. I have noticed that most of these arrogant people think they have it “figured out”. I had one that would not change and prove he knew that it was a problem……I basically found myself “not having schedule time for him”. I refused to be part of it. It should NOT be an issue with an instructor to be transparent and honest when safety is an issue.

The families are debriefed by the FAA and investigated for months on end as well as choosing to get a copy of the report when available, they have no need to watch youtubers do true crime reports of their own relatives.

Sadly, also another dead wife. My husband is a pilot and I have taken lessons. I would Never get in an airplane without seeing someone's license and asking questions (like how many hours in This airplane). Also why we don't go on island hopping little airplanes or go on sightseeing helicopters or airplanes. Just like bad doctors the bad pilots just keep moving on.

Physician here. The medications are not likely to be contributory. Fluoxetine is actually an acceptable FAA antidepressant. Trazodone is an antidepressant but almost never used in makes for depression, but in low doses for sleep. My strong suspicion is that there were likely other potential diagnoses present that the fluoxetine was not necessarily useful for treating. All addictions are impulsive, it takes effort to get to two felony DUI! He should never have driven any vehicle other than a bicycle. Sad.

Interestingly enough, the man had 5 DUI’s. As the good doctor said, Trazadone is sometimes prescribed off-label for insomnia. The biggest prescribers for such use? Doctors at drug and alcohol rehabilitation centers. Nearly every resident is prescribed either Trazadone or Seroquel (can’t remember the generic name) for sleep. Makes one wonder if Chad had recently visited one.

im a old AF C-130 FE , Ive been crew on C-130s for 24years, then 10 years on DC-8s , DC-10s. Combat flying in C-130s with people shooting at me didnt scare me as much as bad weather flying. We prepared to bail out one time due to extreme icing !

Instrument rated fly Grumman Tiger with lots of IFR in IMC, low overcast or stratus. When t-storms are forecast or may develop I either drive a car or fly commercial.

An airline got a brand new plane type. They scheduled a VIP ride for the mayor and many other prominent people. The CEO of the airline was on board as were 2-3 reporters. At the last moment the pilot canceled the flight because something wasn’t making sense to him. A reporter told the CEO “you gonna let him just cancel us like that?” The CEO said “We don’t question our pilots when it comes to safety”

I wonder if his wife knew that the flight was illegal, that his medical was denied, that he was not adequately trained to fly solo. Extra heartbreaking if he’d lied about it, so she was not able to make an informed decision regarding this risk to her kids.

If my husband had been a pilot we would not have flown together. I will never understand why parents would fly together even if there is only a "one in a million fatal crash" Risk. I feel it may be because most folks don't realize the danger those psych drugs themselves can cause as far as judgment and behavior. Few docs even know how they will interact with each other, yet they still prescribe them like candy with little follow up. Their side effects could explain much of the weird behavior we see in general society now. I wish someone would track the rise in misbehavior with those kinds of drugs. Not sure if this guy even took any, but chances are he did from his background info.

Meds get a bad wrap. When used judiciously, as prescribed, and follow the “12 hour from bottle to throttle “ rule (longer if its a really long acting med, mine aren’t and while CAN cause issues, been taking them for so long, no effect on me, and I even DRIVE like I fly…..SAFELY, even more so with passengers)

For what it's worth, the fluoxetine and trazodone have *nothing* to do with him being an egotistical blowhard who knew better than anyone who told him "No". They're *antidepressants.* Depression and risk-taking behavior are almost polar opposites, unless it's someone intentionally planning suicide. The trazodone is a little concerning for being sedating, but had nothing to do with him being an idiot. The 5 DUIs say 1000x more about his personality than the antidepressants.

@@afriendlyfaceinthecrowd confidence is bolstered by training and experience. Choose your CFI’s carefully and vett them as you would a physician. A good CFI will teach you the skills that will improve your ability and therefore your confidence. Experience can be garnered by flying more often by yourself and to unfamiliar airports. You can also fly with others, watching them and learning, it all adds up in your experience column. Join and participate in various pilot clubs, groups and organizations. Sign up for the “Wings” program and work on the various stages. Hope this helps.

As an av geek, and aspirant pilot (12 hours on a 152, solo on a Falke SF 25 motorised glider, solo on a glider and 2 hours short of my HPPL - couldnt complete due to Covid shutting down the school and destroying my savings), I really enjoy your debriefs and try to learn from every single video - please keep it up! As I couldn't finish my HPPl and now cant afford too, I have started gliding (most affordable) to maintain and improve my airmanship - I just want to learn as much as possible and be the best, safest pilot I can be

Nobody cleared him. That's the point. He never received a medical. He never received sign off to fly solo (which requires a medical). He never received sign off to carry passengers (which requires a full license). The system worked to not give someone unsuitable permission to fly. He just did it anyway.

Fluoxetine is Prozac. It's an anti-depressant. It "can" be used for anger and irtrabiliity, but this had to be put in the video for dramatic effect. The pilot seems to be an a-hole, drunk, anti-authority, and not qualified, etc. This med is an SSRI which the FAA is now allowing under SI. To imply that the reason it was prescribed was for that is irresponsible.

Nobody was able to stop this guy from driving under the influence multiple times (aside from the time he spent in jail), nobody would have been able to stop him from taking off. At some point, people have their own responsibility to not do stupid things, and some people will insist on doing stupid things anyway, you can't stop them all the time. Granted, more oversight would still be useful, if there are enough obstacles in the way, some people will choose some other stupid thing to do instead (that is hopefully less lethal).

I’ve always wanted to learn to fly. Your simple (in a good way) debriefs of these GA mishaps have really helped me come to terms with the fact that my disability means that I should never be a pilot. Me too I'm intellectually fit enough and physically capable but my vision, though correctable, is just not good enough on it's own. Not even to be a truck driver in my country. I still found an awesome and fulfilling career for 50 years in healthcare. And enjoy an independent retirement, often traveling by air. (Love going on the A380).

Man, I don't know where to start with this one. Arrogance, complete lack of judgement, operating an aircraft illegally, no relevant experience, total lack of regard for the safety of his family and others. This guy was headed directly for disaster from the outset of his aviation journey. The outcome here was a foregone conclusion. The tragedy is that he killed his wife and orphaned his children. I sometimes say that accidents like this are preventable, but in the case of this guy I'm not so sure, it was only a matter of time. As an aside, and it's not important now, but I do have to wonder how in the world he was able to insure a Malibu given his complete lack of credentials and documented experience. Great job Hoover, I hope that viewing this video might save some lives going forward.

I really like your work here. This clip makes me so upset for so many reasons. One thing you may want to mention to people is that pilot info is public. They can check if they are active and what license, rating and medical they have. Allows the public to confirm what they are told.

I remember the first time I rented a 152 and flew the pattern alone. This was just after my instructor signed me off to solo. I brought my girlfriend and she SAT ON THE TARMAC AND WATCHED. Sneaking her into the passenger seat never even crossed my mind. Not an option. Play by the rules. I knew eventually she would be able to come with me. Just be patient.

Absolutely agree, ATC could have been more involved, but had zero requirements to be and since it was a trainee, definitely not part of the problem.

WHY not point out THE FACT that air traffic control in the U.S. the past few years HAS MANDATED DEI training and hire. MOST could NOT PASS the exam so it was changed... AND CHANGED again.... till they were able to pass. GOOD LUCK.

The Cessna 182 is considered a complex a/c. It's not a retractable but they did at one Time make it with retractable gear. But it has a constant speed propeller. It is used for IFR training but not student pilot training because it requires specific training and a sign-off from a CFI for type. You should have passed your Private pilot written exam and then pass the Private pilot check ride before even using it as a training platform for Private pilot.

One slight correction. The Cessna 182 is not “commonly used for flight training.” It is considered high performance and requires an endorsement. If you said 172, I agree. In fact, you can train AND get a PPL in any aircraft, including a P-51. Legally, yes, but we know that it is practically impossible.

Yes, that stood out to me, too. It's commonly used for training on complex aircraft but not for ab initio training. He definitely wasn't ready for it.

@@thomasdalton1508 I don't think it was the airplane that killed him, It's pretty obvious that he didn't understand the dynamics of unstable weather. Some people think that because of rain it is stable, and that is far from the case. Rain creates lots of turbulence, even moisture gathering and a cumulus cloud creates lots of turbulence and most pilots don't know that, and for sure most students don't know that whatsoever, and that's a fact

@@christophergagliano2051 Absolutely. The fact that he was flying planes that were too advanced for him is just evidence of his poor judgment. It didn't really impact the flight. The fact that he was going faster than he would have been in, say, a Cessna 152 may have meant he had less time to realise he was flying into bad weather and less time to do something about it before it was too late, but I doubt a few extra seconds would have changed anything.

I think the main takeaway is that it really doesn't matter what airplane you're in, what matters is thinking ahead two or three hours before your flight even begins. Specifically you have got to understand that mother nature, I'm talking thunderstorms must be avoided at all cost. Even the airliners avoid them. I doubt if his instructors even got to the point of avoiding bad weather because he was only training to be a private pilot. But it's most important to emphasize to students that these weather systems must be avoided at all costs, and that's a fact

For sure. I worked with a guy who was just starting to rise in the regional motorcycle road racing ranks. He and I rode sometimes with other coworkers. When his second child came along he and his wife agreed he would hang up his racing and just do road bike riding. He hated leaving that progress on the table, but understood how dangerous it could be to continue into the even more competitive ranks. I admired him more for having that good sense than his track accomplishments.

I check the weather before I take a road trip in a car. I can't understand why anyone flying wouldn't bother to check when it is literally a life or death matter.

As a CFI, I had to checkout a guy one day, who wanted to rent a Cessna Cardinal the next day and fly it from Belgium to Spain. He showed up in shorts and sandals and didn't bring any charts or preparation with him. When asked if he had previously flown this type of aircraft, he said he had several hours in the type, but when asked for proof in his logbook, he could not produce it. During the preflight, he had no idea where the fuel strainers were located on the aircraft, so I got more and more suspicious about the guy. I told him that we would fly to a nearby airport to perform some touch and go's. Immediately after takeoff he took his feet of the pedals and put them on the floor. When I asked him why he did that, he said he never used them in flight. Long story short. We never got to the airport, because he got lost and we almost ended up in military airspace. That's when I told him to fly back, but that didn't work out either. After landing I told him that I was not going to sign him off and that he could not rent the airplane. He got really mad and told us that we were only interested in getting his money. Sometimes you just have to stand your ground when you encounter reckless people like this guy.

He lacked self discipline and control. We all need to be able to take orders and follow instruction in our workplace sadly Chad felt that he knew it all. So sorry for the children and families left behind.

Hi Hoover! Great reporting. Thank you for your excellent work. Jack here, CFI for 27 years. GA aviation pilots are self policing and not all are able to do that. There is also a syndrome where many people in aviation surrounding rule breakers won’t report irregularities because they are afraid of retaliation from the denounced. I’ve seen for the past 27 years numerous wannabe pilots who are just unfit for this. Being a pilot caries a great responsibility, that said, for ab initio pilots, prior to being admitted to training, the FAA/TC might start looking at tightening up the rules regarding the attribution of a student pilot permit. Further more, for each flight outside 25 nm, if one is talking to ATC and if a proper flight plan hasn’t been filed with a valid pilot license number, the flight gets reported right away and the pilot flying put on notice. I know this wouldn’t prevent fraudulent pilots from circumventing the rules, but it might dissuade some and maybe save some lives in the air and on the ground.

How many cases like this do you know? Why making again more work and more bureaucracy for the extreme rare cases? I guess there are also pilots who fly under the influence of alcohol, would you then suggest all planes mandated to have a breathaliser?

@ Yes… true… the industry is already regulation heavy and it’s impossible to cover all angles. Somewhere, we still need to rely on the assumption that the majority of pilots will abide to regulations (and common sense). A year ago, a plane crash occurred in our area involving a solo student pilot performing unauthorized manoeuvres over his friend’s house over a residential area. He plunged into the backyard. He and is unauthorized passenger were severely burned and are crippled for life. I fly 4 times a day (and night) teaching at an aviation college and it gives me great sorrow when I learn about plane crashes even more when students are involved. I understand and agree that adding more rules and fail safe mechanism would be impractical.

I wish I could learn to fly but I know I would never get approved for a medical certificate and it's way too expensive just to take lessons without any path forward. It's a shame, but that's the way it is and the rules exist for a reason. I'm taking up aviation photography instead. It's so unfortunate that people get in over their heads and overcommit by buying aircraft when they aren't fit and/or proficient, and ultimately very selfish. While Brandy is not to blame for the accident, she should have known better than to fly with him. It's sad they didn't stop to consider their children first.

There is ultralight and light sport that do not require a medical. While limited to smaller airplanes and a single passenger it is still quite fun! Provided you can comply with the following, copied from the FAA website, this might be an option for you: Sport pilots may not use a current and valid U.S. driver’s license in lieu of a valid airman medical certificate if they know or have reason to know of any medical condition that would make them unable to operate a light-sport aircraft in a safe manner.
