Hi, may I ask you how long it took to rehab your ankle? I didn't have surgery; (fractures below ankle joint) - cast and boot 9 weeks. Started PT at home 2 weeks ago and my ankle's not doing well. How long did you have to rehab it before you could go without a boot or brace for support? (p.s. I'm not able to tolerate crutches).

What excellent videos! Wish I had seen them soon after I broke my ankle in Dec. In retrospect, I think I had poor medical care with my ortho P. A. and now I have some complications. There is now displacement in the distal fibula, I still have pain and swelling, and possibly facing a non-union fracture. My CT scan report looks nothing like the “everything looks good” “you won’t have arthritis or need surgery” he gave me on my last appt. I feel as tho I’m starting over and wasted all that time. The videos are a great learning tool. Thanks for posting!

Hi Cy S. We are sorry to hear that things are not going as well as you had hoped. Certainly when you break a bone around a joint there can be long term issues that relate to how the bone has healed as well as due to the severity of the original injury. A non union of the fibula is very rare but not unheard of. If you are uncertain of the quality of the care you received, a second opinion can often be helpful. Good luck and thanks for the positive feedback! Spread the word

I got myself a long oblique distal fibula fracture while skateboarding in a skate park for the first time on the 7th of June. Surgery was on June 12 (a plate and 10 screws out of which 2 are syndesmosis screws) and my post op is tomorrow (June 25). I’ve been wearing a moon boot and taking meds, pain pretty much went away on day 3. Hoping to not get a cast but that’s inevitable. I'm watching orthopedic lectures on KZbin, and it seems that surgeons are migrating from syndesmosis screw fixation to tether fixation. There seems to be an understanding that screw fixation: 1) gives the surgeon a higher chance of malreduction, and 2) gives the patient reduced dorsiflexion. Furthermore, my perception is that there seems to be an unspoken hope among surgeons that their syndesmosis screws will break within the joint after ligament healing, thus restoring ankle flexibility without needing removal. (However, I have not seen a discussion about complications of this, for example how a headless screw might get in a surgeon's way if that area was reinjured.) My question: if we formalized the unofficial understanding of syndesmosis screw fixation, would it still be an ethical procedure when a tether is available instead? In other words: is it ethical to use something which you know works worse, and which you hope will break at the right time to work better?
@TalkingWithDocs8 ай бұрын
So there have been studies looking at the broken hardware. One published by Dr Weening and it did not affect outcome. There is a trend toward the tether but as far as we know it has not been shown to be superior yet but maybe someday

I’m at the end of week 12 post op for a right bimalleolar ankle fracture and I tore two ligaments. Now I’m sporting a plate, screws and two tight ropes. Non weight bearing for 6 weeks. Check up at 2 weeks, xrays at 6 weeks which I was then allowed to bear 50% weight. Further check up at 12 weeks and cleared to start bearing 100% weight. As an adult it is so very difficult to manage walking again. Takes time. Definitely need to build up the strength in the right leg and ankle. Glad I live in Australia and have health insurance and was able to get great medical care.

I am an 80 year old woman who fractured an ankle. This is what a radiologist said: "There is a transverse fracture of the medial malleolus, at the level plafond, with moderate medial displacement of the distal fracture fragment. The tibialplafond is mediallysubluxed relative to the talar dome. There is irregularity of corftex at the anterior tibial plafond, which could be related to the medial malleolus fracture or could represent an additional, distal tibia fracture. There is a mildly displaced obliquely oriented distal fibular fracture proximal to and at the level of the tibial plafond. The talar dome is smooth. " This was as of 5/23/20. Since then I have been wearing casts. Last one I wear with a cast shoe. The radiologist's report as of 7/7/20 is "Bones/joints: The joint spaces are preserved. No bony lesions identified. Bimalleolar fractures again noted. Partial healing." My concern is about putting pressure on my ankle. Unlike most people from what I have heard I have not had pain. I have been told to walk on my foot until I feel pain. Since that is not happening what should I consider when practicing walking? I appreciate your videos and advice and suggestions very much.
@TalkingWithDocs4 жыл бұрын
Hi Janice. Ankle fractures like yours can take a long time to heal and sometimes even require surgery. Our best advice is to listen to your surgeon. Start partial weight bearing and then get reassessed with an xray to see if the fracture is stable. Hope that helps. You are very welcome for any help we can provide. Thanks for watching and subscribing if you did.

Straight forward and detailed information is what works best for me! This was very helpful. I am trying to find a KZbin Video that can tell me what might go wrong if I am putting some weight on my ankle because I'm not feeling any pain. In other words, am I doing serious damage versus the possibility that my injury is healing well and it may be okay to put partial weight bearing on my ankle as tolerated.
@TalkingWithDocs5 жыл бұрын
Hi neets1234. Thanks for the positive feedback. When it comes to increasing weight bearing in the setting of an ankle fracture this is always best done under the supervision of an orthopedic surgeon who can base that decision on the xray findings and the timeline of your injury. But usually less pain means the fracture is healing. Hope that helps. And thanks for watching.

I broke my ankle February 2003, I'm told it was a compound fracture (I was run over by a car trailer) I was trapped beneath one of the wheels, and also hanging over a pit. The mudguard ripped a chunk of skin from the outside of my lower leg. Both ends of the bone were visible above the skin. I was given a tetanus injection as a precaution, the trailer has recently been used on a farm. In surgery later that night the bone was stabilized, and I was in a back slab, so nurses could check progress. Two days later I was given a skin graft to cover the skin ripped off by the mudguard. It healed beautifully. My only complaint is the site, sadly it's in an awkward position and plastic surgery won't make any difference. The ankle itself is still very painful. I do have osteoarthritis in the joint now. And it's constantly swollen not to mention painful. My Dr had talked about fusion but I'm not in favour of the idea.

Great video. A quick question regarding treatments. I have a lateral oblique malleolar fracture with either mild or minimal displacement. The doctor decided to have me in a hard cast for 4 weeks with an aircast for a further 6 weeks. My first question is, what is a mild/minimal displacement? And, when is it possible to recover fully without surgery in that case, assuming there are no other fractures.
@TalkingWithDocs4 жыл бұрын
Hi I V. So the amount of displacement is likely 1-2mm or a very small amount. Typically the bone will heal in about 6 weeks and patients can often return to normal activity around 3-4 months but every patient and fracture are different. Thanks for watching and good luck to you.

An excellent orthopedic surgeon. In a cast immobilized for six months. And six months of physical therapy that truly worked well. I am walking quite well. Healed beautifully This ankle was so very swollen ( just about 2.5 - 3 times it’s size) that I had fracture blisters and had to wait three weeks to do surgery. I was a real challenge. why do doctors always push surgery for these ankle breaks? My mom broke her ankle, and we can't afford surgery, so the orthopedist said he would do a cast. I am wondering if you skip surgery and do the cast for a 65 year old, what is the difference? Is the recovery longer? What is better?
@TalkingWithDocs4 жыл бұрын
Hi Ivonne. Surgeons do not typically push surgery however there are specific indications where surgery is better for ankle fractures and this would include displaced fractures, bimalleolar fractures (so medial and lateral) or Weber C type fractures (or fractures that are higher up on the fibula but have an associated syndesmotic ligament rupture). The purpose of the surgery is to allow the bones to heal in a proper position and reduce the chances of have a stiff painful ankle or developing arthritis in the ankle. Hope that helps. Hi Docs! I fractured my fibula 3.5 weeks ago. It's considered a clean brake, no surgery needed, no ligaments involved. The ER doc told me that recovery from this type of fracture is 100% back to normal; no arthritis down the line, no pain in the area from running etc. You seem to say different. I did see an orthopedic doctor and got a fiberglass cast for 3 weeks and will be reassessed. I'm a runner, will there be any repercussions in the future?
@TalkingWithDocs6 жыл бұрын
Hi Mitshy. So we will commonly tell patients that you may not get back to 100% but that doesn't mean that you won't. Some people won't but others will. It depends on the patient, the fracture, the healing, etc. Usually 5-6 weeks in an aircast or fiberglass cast then some physio. Many patients will return to running. Hope that helps.

In my case I had surgery after the double fracture right ankle accident beginning of december 2001. But I had to wait for about 2 days before surgery took place and in the meantime I was not allowed to eat or drink. I only had ice cubes. It was a hard lesson for me which left me with a lot of trauma.
@TalkingWithDocs7 жыл бұрын
Hi Christina. An ankle fracture is a serious injury and certainly can lead to some degree of permanent dysfunction due to the damage of the cartilage at the time of the injury. It is not uncommon at many centers to wait a day or two for surgery and this typically will not affect the outcome in a negative way. If your ankle has become arthritis as a result there are some both operative and non operative treatments to try and reduce your pain and improve your function however this should be discussed with you by a qualified foot and ankle surgeon. Good luck

Hi! I love your channel! This video has helped me out a lot, I’m just curious though, I had fractured my fibula about 3 weeks ago, it’s painless enough that I haven’t been taking medicine, however, I’ve been noticing some pain behind my knee lately (in the last few days) when I go to stand or if I’m upright for too long, it goes away almost immediately when I lift or elevate my leg.. is that normal? (I’ve had blood clots before in shallow veins so I want to make sure it’s not a cause for concern) Thanks in advance!!
@TalkingWithDocs5 жыл бұрын
Hi Nicholas. Thanks for the positive feedback. Blood clots are always a possibility with any lower extremity injury. If the pain goes away when you lift your leg it is less likely related to a clot. If the pain is constant and increasing and you have lower leg swelling you should definitely be assessed. Often the leg will be sore when hanging down. That will improve once you are walking more normally. Good luck and thanks for watching!

I broke my ankle 3 weeks ago . I waited a week before going into Urgent care because we had Pandemic starting with COVID 😩It was broke and it was reset in ER . I went to ortho four days later .I did not want surgery as can’t afford it . So they put a cast on and set me up to be checked four weeks later . But cast became to loose and I went in in two weeks . The Dr took the cast off and decided to put a boot on . She is chief of podiatry. She said not much has healed yet . But she wants me to treat boot like cast and no weight baring and back in four weeks . I’m 65 but have always been fairly active and like riding my Harley . The Harley didn’t take me out ! A wet bathroom floor did in the middle of the night ! Dr said my blood work showed good calcium for my age and good vitamin D . I am taking supplements too , to try and help . I don’t smoke or drink . But take meds for anxiety and depression . Is it unusual for bone not to heal in two weeks? Also I’m having panic about blood clots !!! I don’t just lay in bed I move around on a scooter . I can’t stay in bed ! I’m totally freaking out about blood clots now !! Can you talk to me . Yes I’m health phobic too . I was a weight trainer in my forties and ran 20 miles a week . Before my break , I went walking 2 to 3 days a week at six mile stretches each time with my dog . I’m scared now about hearing of blood clots .
@TalkingWithDocs4 жыл бұрын
Hi Pamela. So it is very common to not see healing on an xray at two weeks. Don't despair. With respect to blood clots, try to stay as active as you can and consider taking 81mg ASA if that is ok with your family doctor or specialist. Once the bone has healed (typically 6 weeks) you will wean out of the boot and do some physio and you will be on your way. Best of luck and thanks for watching.

Thanks for the question. So it is difficult to give a specific amount of time for how long stiffness would remain after surgery without knowing the exact injury and type of operation. Typically stiffness can be improved for months after surgery. Usually the largest gains are appreciated early on as swelling is reduced and physiotherapy is started. But you can continue to improve for a year after the injury. Hope that helps

What sort of strength from a ORIF fib can you expect from the bone post healing? Does the bone remain more prone to break or less prone with the plate/screws?
@TalkingWithDocs7 жыл бұрын
That is a difficult question to answer and every case would be different. Generally speaking, if the bone heals properly, the area where the plate lies would be as strong or stronger than your bone. however if you fell or suffered further trauma, the area above and below the plate can be prone to fracture depending on the scenario. Hope that helps.

Hello Doctors. First I’d like to thank you both for such a thorough video. As a patient who is unfamiliar with my injury this answered so many questions. I Fractured my Fibula and was in a cast for 5 1/2 weeks. Doctor has taken another X-ray and there is still a slight area in my Fibula that hasn’t healed. Doctor has put me in an air boot and said I can put weight as tolerated. It hurts a lot when I try to put weight and afraid of re-injury. Should I be worried? I’m thinking to seek a second opinion since I’m not happy with my doctors lack of information. Thanks you
@TalkingWithDocs4 жыл бұрын
Hi Chris P. You are very welcome. Often when you first start weight bearing the pain can be significant. Xrays at 6 weeks will also often not show complete union of the fracture however it typically is very stable at this point. It would be up to you to consider a second opinion. Gradually the pain should decrease. If it doesn't you should then consider being reassessed. Good luck and thanks for watching.

I love hockey and have difficulties with impact activities such as running. My question: with all the health benefits that come with hockey and the risks you know, such as arthritis, DVT, or any other factors not discussed in this video, is it advisable in your professional opinion for me to continue playing hockey at this point of my life? At what point is an ankle/leg fx so severe that amputation is considered? I have a really bad pilon fx from a fall from 2 stories. I was told my ankle was a bag of bones and nothing was holding my foot to my leg but skin. An ex-fix was put on initially and then I just had surgery to attempt ORIF repair but amputation has been mentioned. Thankfully, not by the surgeon himself yet.
@TalkingWithDocs3 жыл бұрын
Hi OffTheLineFox. Sorry to hear about your injury which is certainly severe. A pilon is a hard fracture to fix because often the bone is in many pieces and the soft tissues and blood supply are not very good. The decision to amputate is often more related to neurovascular supply or chronic infection that cannot be cured. Even with ORIF, the plan would be to try and restore bone stock and if it healed, that may allow an ankle replacement in the future. Every case is very different though. Have a frank discussion with your surgeon. Best of luck to you during your recovery. Thanks for watching and subscribing if you did.

Thank you for all your info. I am 13 years post op for a fractured calcaneus with ORIF surgery. I have had continued pain with swelling since day one. My walking/standing time is reduced because of the pain. I've seen multiply Doctors over the years, and most of them suggest a fusion, but with no promise of pain reduction. Since the original surgery was so traumatic to me, I really would like to have all the hardware out and see where I am with the pain. My last visit to a surgeon, suggested that I have the metal removed and he was very critical of fusions. He felt fusion generally have poor results. I am so confused as to the best step. I prefer removing the hardware, but have nightmares that after 13 years it will be enmeshed in the tissue and difficult to get out. What is your opinion? I want to have a life again.
@TalkingWithDocs4 жыл бұрын
Hi Barbara. Sorry to hear you are having a difficult time. Calcaneal fractures are very difficult fractures to treat. Removing the hardware can be difficult and may not relieve your pain. There is a real chance your pain is related to post traumatic arthritis and that is why the other surgeons have suggested fusion which is not guaranteed to be successful. It is a complicated decision for sure. If you do not want a fusion, having the hardware removed is reasonable especially if you have found a surgeon that could do it. You are entitled to want your life back. We wish you the best of luck on your journey. Thanks for watching.

Husband just fractured his medial malleolus. At the ER the attending Dr. said air cast with no weight bearing for 6 weeks, bone was not displaced so thought that would be all that was needed and will be referred to an orthopedic surgeon within a few weeks for check up. He left and then I was allowed to come in and by then it was just a nurse that seemed very unsure of instructions and without consulting with Dr. and sounding very unsure of her answers, she said he was allowed to take the boot off anytime to shower or even to sleep and when questioned by me thought it would be fine even from the first day forward. I am not really sure this is a good idea and can't get a hold of anyone on the long weekend here. Generally speaking do you allow your patients with similar to take off the aircast boot at all in the first couple of days/weeks and if so what parameters do you give them? Thank you.

My surgeon has sent me to an ankle specialist for further care and am now having surgery on Wednesday to remove hardware, scope the ankle and lengthen the Achilles tendon as that seemed to become a big problem. The specialist said the first repair was done very well but my injury was serious. He told me at some time I will likely need an ankle replacement surgery due to the extent of the injury but go wait until I'm older for that. Both surgeons have mentioned trauma arthritis. How common are replacements needed years after an injury like that?

Hi Mari. Thanks for your question. Unfortunately after a Pilon type fracture arthritis is almost inevitable due to the damage caused at the time of the injury. I experienced a trimalleolar fracture 3 years ago. Surgeon was pleased with outcome. About a year following surgery at times when I stepped I'd experience a horrible sharp pain with total loss of stability. I would break my fall by using a counter, wall or nearby person. I did go to the Dr. , x-ray was taken, told bones and hardware looked fine, didn't see any cause, did see some arthritis. It started as a intermittent issue and with-in a couple months became more and more constant. After falling often, I could no longer rely on my ankle and started using a walker placing weight on my arms. Determined to walk, I used trial-and-error, and finally found a way to step with my foot turned (very) outward and the ankle was much more stable. Is this something that is considered normal arthritis after a bad fracture?

It would be very unlikely to have both injuries. When bones break, they bleed and this blood can track both up and down and cause bruising. If you were unable to flex your ankle that would be an indication of an Achilles rupture however this would be very uncommon. Hope that helps!
