If you haven't heard All Too Well yet, you have to! Please, do me a favor, though...experience it solely for its lyrical content first. She wrote and directed an incredible music video to accompany the 10-minute version of the song that deserves to be seen as well because it really does add something. (And it's just cool to see Taylor envision a video for the song and see how she brings it to life.) BUT the video is so engrossing, that it's almost impossible to properly pay attention to the lyrics while watching it. And, as someone who obviously appreciates good lyrics and production, you'd be doing yourself a disservice if you didn't experience the song on its own on your first listen.

If you look at the credits, it says "Spiral Alice" which is actually "capillaries" jumbled, so you are correct, that's her heartbeat! A great example of her little easter eggs and puzzles she likes to give us. Jack Antonoff, one of her main producers/friends, really blew everyone's minds when he let the world know on IG that they recorded that song in december of 2021, rather than more recently which most people assumed .

Idk if anyone has said this yet but since you early on caught the exhale/heartbeat segment and you also love wildest dreams. She samples her heartbeat is in that too. Truly legendary behavior 🫶✨ To add on the exhale is also supposed to represent the exhaustion, a sigh, to stop and take a breath before you’re about to deal with something or the last respiration before dying. It’s all so detailed. And that’s why at the end. It stops and there is no pulse. Flatline. 😭

In terms of attention to detail, its her heartbeat as many have said. But Marjorie off of Evermore (Folklores twin album) is about her grandmother who was an opera singer. She uses her grandmothers voice in the backing of the song and she did it at the concert too. The hauntingly beautiful voice in the bakground was her grandmothers. Its also a tear jerker of a song so heads up if you listen to it.
