I love all your videos but I must say this video defines why I think what you are doing with these videos is awesome. You show ALL of what goes into emergency medicine. So much of what we do is tedious and likely viewed by some as "not important" in the grand scheme of things. These are the things that most in the field can learn from but will rarely see presented in this format. Thanks again!

You undoubtly are a fine doctor, but a lousy mechanic :) :) At least you got the ring off, but I couldn't even start a surgery :) :) About the GEM cutter : allow the machine to rev up at max rpm, only then start cutting without pushing down the blade, allow to rev all the time. Use a little water running down the place to cool easily.

I'm a jeweler and have removed more rings than I can count. One trick that I've found is, once you've cut through the ring and opened it up some, is to gently pull the skin back through the ring as you work the ring off the finger. This helps reduce the amount of swollen tissue that bunches up around the knuckle. I've also had the person soak his/her hand in ice water to help reduce the swelling before trying to work the ring off.

That GEM cutter is an excellent piece of kit, I have never seen such effective cutting before. It has to be said that the operators are just as sharp!

+papalevies: Incorrect, they only used the wrong cutting disc (recommened for soft metals like Gold, Silver, Brass and Copper) until swapping to the diamond disc.

i thought the same. even with the proper cutting disk, the motor was just weak. it might be a crappily implemented "safety feature", though, to prevent too much heating. but that tool still wasted way too much time.

Sorry doc, But I've got a side grinder with a diamond blade- that will easily cut through stone , granite, metal , etc... And I could cut through that ring in about 2 mins- taking my time the last little bit as not to cut his hand. Could have used cool water if needed to reduce heat transfer from the friction. Perhaps they should have called in one of the experts from the maintenance dept.

The ring is made of siladium. It's extremely difficult to cut and cannot be resized. I'm a Professional Bowler and have many rings. Some are Gold but most are Siladium. That is how I know.

I had the same accident. Went to sleep and boom the next day my finger had swollen and i couldn't get it off. I tried using soap and water with no luck so i went to the doctor in a hurry and he tried this method they did on this video in the beginning and that didn't work. The doctors told me to go to the hospital really quick as my finger started turning blue. I came to the hospital and was taken in to the emergency room right away. There were 4 doctors there who tried with no luck and my finger turned more and more blue. They told me to go wait in the waiting room and 10min later i saw 7 firemen walking in and pointing at me to follow them while they carried a tool box 😅 They had a saw machine and cut my ring in half. My finger was stone hard and dark blue but after a while it went back to a natural colour and softened up. Thank you for reading :)

Interesting point, your suggestion sounds reasonable. Pushing fluids up, however, may put more fluids right at the source of the problem. Also, the main area being compressed was the PIP joint and getting edema fluids removed from that area is really what we wanted to accomplish and would work wrapping either way.

Friction is how the blade cuts. I can't understand putting KY jelly on it to "reduce friction" when you're trying to cut through something!

Dr Mellick, just a suggestion. When you're trying the string trick, start wrapping the string at the top of the finger. This helps push fluid back. This works for me.

Great video! The gem is designed to do minimal damage if a slippage were to occur(my guess). I can see that feature hindering you guys in this video. The pt was a trooper, though. Thanks Larry!

I have a pair of bolt cutters that would have snipped that thing right off of there.Have removed two rings with them in past couple of years ,no damage to finger.

Larry Mellick : Another time if you get the same problem you can try my suggestion. You don't have to cut the ring, you just need to smear his finger with soap suds and then pull while twisting the ring out. If necessary, soak his finger into the water 10 minutes before you pull the ring out to make sure his finger skin softer.

I could not find my original comment to reply back to you, but dremel tools are rotary tools that can be purchased at any home depot. From my experience either a cut off wheel will zip right through metal, or for safety of your patients, maybe a diamond carbide burr would work very well. That finger guard would work perfect with a dremel tool, or maybe a popsicle stick or tongue depressor would suffice for a single use.

when i was little i got a ring stuck on my finger too, my mom then took me to the fire department which was nearby where i lived and they had this little hand saw, similar to this video, after a few times dipping my finger in cold water because it got hot it was open and i was the happiest 5 year old in the world! great job you guys as well

Just dont sleep with it on. When you sleep your fingers swell called the rejuvenation process. So he had his ring on and it swelled up and couldn't go back down because of the ring.

When I was young, I slipped a thick metal washer onto my ring finger. For some reason, my mom took me to the local fire department lol. They eventually ended up cutting the washer off with a double handled hacksaw. That son of a gun got real hot, and I was crying the whole time. These were really good dudes though, and I still feel super thankful to this day.

If they go too fast the ring gets VERY hot and will burn his finger- they have to keep the power low and do a bit at a time. If you have ever cut a screw head or something metal with the Dremel and accidentally gotten a bit of the metal flakes flying off on your skin, then you know how badly that can burn....

I thought you're supposed to use dental floss because it compresses better? Also, that tool is a POS! Run to home depot and get a dremel. So frustrating to watch if medical tool prices are anything to go by. That thing is outrageously ineffective. Like 1 inch lb of torque?! LOL. There are 20 dollar rotary tools that cut better than that with 5 dollar diamond encrusted wheels. Need some mechanics in the ER LOL.

starting the elastic string at the distal end of the finger and wrapping towards the ring might help force fluid from the finger into the hand and help compress the finger better?

The String trick would have worked if u guys used thinner material (ex: 3 ply thread), whatever stuff u used was WAYYYYYY TO THICK! I work at a Jewelry store... Don't matter how thick the ring is or how swollen... works everytime!😊😉

You're right, the grinder isn't really strong, but the thing is, to be able to use it in that kind of situation, it has to be compact in size and thus limiting its power tremendously. The bit itself is also a factor. If you had something such as a dremel, you might think it is more powerful, but you are failing to see that because they need to ensure that the bit does not break and further injure the patient, they are cutting a pretty thick grove into the ring while a dremel can use a diamond seeded thin bit that can cut through metal like this is just a couple of minutes.

Have u measured the torque of both tools? No. So please stop telling doctors how to do their job. Tell u what. If u ever get a ring stuck on your finger, you get your handy dandy dremel out and cut it off yourself.

Ring was on over night for the first time. The finger had already demonstrated edema or swelling. Eventually, ischemia and ultimately strangulation would have occurred probably in a day or two more.

We had the same patient in A&E. It was also a big ring and the ring cutter didn't work. We used the string from the oxygen mask and it worked!

KY on skin around the ring and as much under as possible followed by progressive compression of the digit with elastic band (red rubber drain, pediatric Foley, rubber bands, whatever. Wait a few minutes. Remove compression and then slide the ring off. Usually works and easy.

Install the metal sliver between the ring and skin then produce the slot in the narrow part of the ring. Take a pair of diagonals or end cutters and place the tips of either tool and squeeze the handles together. This will expand the slot providing the extra diameter. If the ring is gold or silver the ring can be cut as it is softer material. After the ring is cut use both tools to pull the ring apart. This has worked very well for me in the Past. Joe Carter

Two different approaches by two different operators. I start proximally or close to the ring thinking I can squeeze the edema fluid distally where the finger is smaller or thinner. Starting distal might bring edema fluid up to the ring.

That's basically what I was implying through engineer speak. Perhaps I should have been more clear. I agree completely with you. I think the actual cutting must be from the mechanical force of protruding bits on the cutting bit striking and breaking off bits of the ring.

If I saw it correctly, both times the string was wrapped from the ring downwards. Shouldn´t the string be wrapped from below the joint upwards and given a minute or two to press fluids out of the finger as much as possible (hence the tight wrapping and elastic material)?

It’s so funny how the world works. I’m a carpenter. All these guys r probably light years smarter than me. But I could have got that off in probably half the time. They were pressing the grinding wheel down causing it to sieze up. You don’t need pressure let the wheel do the work

you know best,when I get a ring stuck on my finger I found the pulling made it worse whereas turning as you gently pull it is a lot easier.thank you.Pi

Couldn't you remove some of the excess fluid with a needle? When I ruptured the fluid sack in my left knee they stuck a huge needle in the side and pulled out four shot glasses worth of arthritic joint fluid. Don't see why the excess fluid in his finger could not be drawn out the same way.

I am a big guy much like this patient and i tend to get water retention did you ever consider treating his water retention vs cutting the monster ring? He def... looked like he was retaining water.

why didnt you wrap from the distal end of the digit to proximal, as to push the edema or blood up instead of trapping it beneath the ring? Would that have made a difference?

Good question. My thought was the distal finger was the smallest and pushing the edema away from the ring was my goal.

I really enjoyed this and the other videos of yours that I watched. I believe Roberts and Hedges' recommends wrapping distal to proximal and then unwrapping proximal to distal. Probably wouldn't have a made a difference in this case though.

Looks like something you might be able to make the local fire dept. do. They have the manpower and have to do some similar (although usually larger) extrications with industrial/manufacturing entanglements. Maybe you could sell it to the FD supervisors as training opportunities.

Doc, they make a tool in the auto industry that would work after the ring is cut ,They are alot tougher then the spreader pair in you kit. i see if i can get up a pic..

I had my wedding ring stuck and did the string trick. I used dental floss, along with dish soap to help. Got to the knuckle, had to start wrapping the finger again and continue. Finally came off!

Maybe the lubricant helps to reduce the friction between the sides of the blade and the sides of the cut groove, thus allowing a better rotation speed? I'm no engineer though.

I understand some tricks never work but wrapping it starting distally would get a better result as it would have pushed the fluid out of finger. Using elastics with some decreased temp would decrease swelling and edema.

The ring cutter needs a little more power- I know that the faster it goes, the more friction and heat it would produce- but those are controllable. A good powered rotary tool would chop right through that-

I had to cut one of these off in our ER two weeks ago. We didn't have the diamond wheel however so I just used a dremmel and took my time, still took about ten minutes
