Sir, Doctor said I have 'Lacrimal gland dysfunction' due to inflammation or covid and have aqueous dryness.... Is it possible that Lacrimal gland works properly again or I have to rely on drops ?

I found all of this useful, would all opticians in the US have all the equipment you have to test for these? I have been diagnosed with blepharitis and recently MGD but apart from slip line and blue dye added to eye, none of the equipment in your video was used for any of my tests in the UK. I try to tell people about this but it falls on deaf years. My aunt only got the care she needed after years of treatment with the wrong doctors. Sadly finding a good doctor involves lots of money and trying.

Your channel is a oasis of knowledge. You explain what ophthalmologists often don't. I even remember one telling me that astigmatism was just tired eyes and that treatment was aimed at resting the eyes.

I have had Sjogrens for more than 25 years and have had many of these tests, also including a lip biopsy. No one has ever explained things as well as you have. Thank you! ❤

You should consider to go into academics; your talent to share knowledge is extraordinary! I learned that when my eye Dr. Told me I have dry eye, the condition should be monitored and I don’t think is happening. I have eye drops as treatment, but in my last visit was not even mentioned. Thank you, I ‘ll be my own advocate.

I need cataract surgery & consulted 2 drs. The 1st said my eyes are dry but 2nd said he’s seen eyes like mine only in people with Sjogren’s. So I’m taking cyclosporine & the other drops & cleanser for 3 weeks prior. I wish other eye drs had told me-they said if my eyes didn’t bother me I didn’t need treatment. But I’m 63 & can see other issues over my life that were probably related to Sjogren’s but misdiagnosed. Now I’m terrified of cataract surgery when before I thought it would be as most people say-quick & easy recovery with great results.

This was a very comprehensive video. I went to a dry eye specialist who did none of these tests, not even the Schirmer tear test. He just looked at my eyes with the slit lamp and said I have stage 2 meibomian gland dysfunction. Ugh...

Every topic you’ve covered in this video was informative an valuable to me-Thank you. My last eye exam was 2018 when I was diagnosed having bletharitis, and astigmatism of the right eye causing double vision that worsen with more distant objects. I am naturally inquisitive so ask lots of question during my eye exams, but doctors are reticent about taking time to answer my question during an exam that feels like an “assembly line operation.” I suspect this is partially due to insurance company imposed time constraints for their coded procedures and doctor offices trying to operate financially in the black. Not meaning to reflect my opinion specifically on you or your practice; Generally speaking, I’m 62 and have observed over the years how insurance companies have gone from covers medical treatments to dictating how medicine is performed-it’s despicable.

My dr did the yellow stain. She told me to use pataday and oasis eye drops that are preservative free. I was waking up with syringy eye mucus that would make my vision blurry. Since starting this I stopped waking up and experiencing the mucus. She said mine was mostly allergy with some dry eye.

It's sad doctors around my area who say they are dry eye doctors don't offer extensive test/exams like what you've mentioned. I need help and relief and no one seems to have the knowledge behind it

Even though i had borderline dry eye test results, my symptoms were almost extreme. I had to treat myself for allergies with Rx drops before dr would give me Punctal plugs. I knew that it was a waste $$ and time. After 7:30 pm, i would use my drops every 10 minutes. He finally understood. I only have itchy eyes in May. It was August when i saw the dr. I got plugs in Oct

My Schirmer test was just under 1. He had to peal the paper off of my eye lid, it was really stuck, and dry. I was dx'd with Sjogren's Syndrome shortly thereafter (and SLE).

Hi Doc, amazing video! How do you test with both Lissamine Green and Fluorescein on the same day? Do you do them in a specific order? Do you have to rinse the eye with saline in between?

I have a lot of eye irritation and my Opthamologist said I have very dry eyes. Drops (not even the good hyaluronic acid kind) seem to relive the gritty, irritated feeling. I asked if this could be allergies but they said no because my eyes were not itchy. Xiidra irritated my eyes more. They did none of these tests you mentioned. How do I get them to do these or find someone who will?

My clinic does most of these tests, typically when I've gone in for LipiFlow or just a dry eye check-up eval, so while I already knew about basically all of this, this video is definitely well-organized and should be very informative to those who are unfamiliar with this.

At 5:58 often when I look in the mirror I can see this - my oil layer floating on top of my eye. It looks exactly like in this video, floating up after I blink. Please can you tell me if this is good or bad!?

yes, but I have worked in clinics that don't have anything but fluorescein stain and I still felt I did a good job detecting dry eye. I think all the toys are great and help, but a doctor who listens to their patient and cares is what matters most. ✌

All my life i had perfext vision and no eye issues. I woke up one day a few months back, and my eyes were irritated. It got worse as the days passed. After about a month, i went and had an eye exam, they said i may have GLAUCOMA, and they daid my left eye vision was a little off. They gave me glasses, and sent me out the door. After another month, getting used to glasses, which i never did lol it was horrible feeling. My eyes were getting more irritated. I went to the e.r , they did another IOP check on me, and they told me my iop was high normal. Told me to relax , but said "you seem to have open angle glaucoma in your left eye' i was freaked out so bad. So they sent me to see a specialist , took me a couple weeks. Dueing all this, i lost weight, ti stressed and scared to eat. Depression, anxiety, couldnt work in front if a computer anymore. Had to stop due to burning eyes and dry. I started to see double vision a little bit , halos around lights ..i really Started to freak. Finally i made it to a specialist TODAY. They did all these test on me , diablated my pupils , iop tests, put some telescope things on my eyes lol and by the ed if the day.. I DO NOT HAVE GLAUCOMA!!!! In fact , they told me to throw my glasses to the side. And said "you have severe dry eye" thats it. Showed me the nirves in my eyes on the computer, it was crazy to look at Lol So after all these months of agony, scared to death...today i am a relief man. Depression, anxiety, everything left immediately. Told me to do warm compresses, and a chekup in 6 months. Guess what, i started warm compresses today..my eyes already feel so relaxed and relieved. Nomore glasses , i can see perfectly fine again. Still a LITTLE irritating, but thats it. Still a little bit of double vision reading white letters..weirdly only ehite letting. Love your videos!! Im the happiest man a live today. Im looking at ALL remedies to stop this stuff :)

The autoimmune informationn. I don't know if you have heard of Complex Regional Pain Syndrome? It is when the sympathetic nervous system has an abnormal response to pain. You're in pain 24/7 which causes inflamation in the whole body. I doubt my doctor even knows this. I need to bring this up because i don't know if i have dry eye disesse or if i have dry eye from CRPS? I wish i found this video sooner. But glad I did non the less.

I have chronic dry eye I'm currently working on correcting naturally with moist heat therapy and massages to the Meibomian glands in order to increase healthier oil production. I saw the Itears 100, but cannot find any opthalmologists aware of it in my area to prescribe it for me in order for me to also produce more healthy tears. To make matters worse, I have recurrent erosion in both eyes which begins to generally occur more frequently and more severely around Nov. and throughout the winter months due to dryer winter air in Pennsylvania. I've purchased two humidifiers to balance our home relative humidity between 55-60%, and while it does help to reduce the severity of the episodes, the recurrent erosion episodes still persist. In addition to dry winter air causing the recurrent erosion episodes to increase in both frequency and severity, and the humidifiers aiding in reducing the severity of the episodes, could the recurrent erosion episode frequency also be a result of winter barometric pressure being different than that of the warmer months?

First of all, I appreciate these kinds of videos that help arm the patient with more information BEFORE they seek treatment. Thank you for that! Second, I found the entire video helpful as I've started my own search for dry eye treatment. Which brings me to a question/suggestion for a future video. While researching dry eye and where to go for it in my area, I came across an office offering both Lasik and another corrective surgery called SMILE. Do you have a video on the differences, pros/cons, etc. of the two? If not, would you be willing to add that to your list of video topics to consider? Thank you! :)

I found out that lipid layer scan is done during meobian gland scan . I also have come to conclusion that maybe my dry eye is mixed , not only meobian but also lacrimal glands , because you said that Shirmer's test shows the work of the last one , but luckily both will get a great relief thanks to Omega-3

Absolutely loved this video. For people in Australia like myself we find it so hard to find dry eye specialists and the ones are very basic in treatment options. For us who are prepared to travel to Minnesota to see you what would these tests costs as a patient? I think most of us would be prepared to get on a plane and travel to get the help you offer.

Wow. Im speechless. Step by step. Im about t get this done on october 31. Just t see all those test that may my left eye get swollen. I been having problems with my eyes. I was on steroid drops for to long may my cataract progress rapidly. I ended having catarats surgery. But still my eyes feel like sand on my eyes, itchy and my left eye is very sensitive. I should have this test done right when I started like an infection on my left eye that it was swollen under my eyes very puffy for so long, then gave me steroids drops which make the cataract grow fast. Thank u for such an amazing video and very well explain. Congrats & lots of blessings

Hello…if I do warm compresses for my dry eye and it’s blurry after the recommended 10-15 minutes of compress time, is this a good thing? Does this mean that I am still producing oil in my existing meibomian glands?

Extremely informative video Dr. Allen! Like you said, I don't think my eye care center did all those tests for my dry eyes. I have tried all the treatments without any improvement so now I have just started wearing scleral lens. I am having fogging issues with my left eye so when I went back for a follow up the doctor used the flourscein sodium I think to help with changing the fit of a new lens. The scleral lenses have made me eyes feel a lot better when before they would be irritated and red with blurry vision. I am wondering now that since the scleral lenses are not treating my dry eyes, will my MGD get worse? I have stopped using the eye drop and nasal spray treatments since receiving the scleral lenses.

I love scleral lenses! For some people, they are like magic. Lenses do get fogged up from time to time, it could be the fit, it could be the MGD (oils get trapped on the lens), it could also be the lens itself. Glad you are working with some docs who are willing to work with you and solve problems. The scleral lenses do a great job at trapping fluid behind the lens and helping the cornea stay hydrated but since there is a lot going on contributing to dry eye, it may still be beneficial for some people to have other treatments simultaneously.

Thank you for the video. I will ask my Dry Eye Specialist about the Corneal Sensitivity Testing. We have been treating my dry eyes for 2 years with only have minor success. Since I have other neurological issues this may give some answers.

My grandma damaged her sight (something about the cornea being damaged) due to sewing when she was younger (plus she had an eye infarction at least 15 yrs ago so she's blind from one eye); she has mentioned that her sight decreased in the past few months (from seeing 2 squares away to only 1) and the doctor she went to said it couldn't be fixed with glasses nor surgery (it'd help a very little amount so it wasn't worth to do it until she went almost completely blind), is there really nothing that she could do to fix it??

Hey thanks for asking! That's a higher level question for sure! Studies have found it to be good and comparable to oral medications such as oral doxy. It used to be pretty expensive, but now there are some compounding pharmacies which are making it more affordable. I like the idea of topicals more than orals as oral antibiotics can change the gut flora.

Doctor my eye going dry a year ago , after cataract lazik surgery and intracular lens Alcon vivity IQ UV toric ,special right eye and light pain , My vision is amazing open range day or night but my eye are now very sensitive to the light for out I used the best sunglasses I tried Maui Jim Peahi Grey ,blue , bronze HLC all are excellent , I need to used drop every 4 to 6 hour thera tear extra dry the best I tried or systene

Dear Dr. Allen, I have dry eye symptoms for more than 3 years now and it started after wearing contact lenses for ~ 3 months, though, unfortunately, I also wore them sometimes while sleeping. The symptoms are far more pregnant on my right eye, causing the tear film to rip apart within less than a few seconds, sometimes immediately (left eye can easily go over 10 seconds) - I'm noticing it because then it's like fog going all over the eye and it burns. Raising my head and looking down instead of up helps a lot. I've been to many doctors but none could find the true reason. I've had LipiFlow a few months ago and one week ago when I signed up for a manual gland expression, the doctor told me that the oil is coming out as it should be and none is clogged up. I still got the issues though on my right eye and I have the feeling that not the meibomian oil is the problem, but rather a physical or allergic problem or decreased watery tears since I found out whenever I get reflexive tears to be produced (e.g. wind exposure) it's way better and how it used to be. The doctors did not recommend me plugs because then the inflammation would probably increase since the water could not flow off. Spider veins are present and I had Limbitis as a kid that were treated with Cyclosporin A - due to the heavy itching coming from the bubbles that developed on the eyelids (mainly upper), I just HAD to scratch it. I'm wondering though why there were no symptoms for more than 10 years but after starting wearing contact lenses. I'm also currently testing all sorts of vitamins and other things like Sjögren's syndrome, hormons... Unfortunately I'm not living in the US and I can't easily visit you, but if you could tell me your overall meaning to my situation above from pure reading and knowledge, I'd be really happy! PS: Great content you do on KZbin!

My dry eye started after wearing contacts as well. Contacts just never felt right, and were sometimes unbearable, but I do work full time on the computer. I've had dry eye for 12 years since. Xiidra is at least preventing bad flare ups for me most of the time, although it seems my vision has been blurrier... Best of luck in your journey

Got any recommendations for dry eye clinics in central Georgia line Newnan or Warner Robins? Eyes constantly full of eyelash like irritation, sore eyelids (from the constant rubbing) have ptosis affecting vision because I can't stand to open my eyes fully because of the irritation.

Hey dr. Allen! Will it be okay to let curettage be delayed for 1 to 2 years only if not troublesome then? or it could even cause eye cancer or anything which is harmful for eyes?

Hey thanks for commenting! And for watching! by curettage, I imagine you mean for a chalazion right? In the case of a stye, surgery is always optional unless a doctor is suspicious for basal cell carcinoma, otherwise if a chalazion is not bothersome, it is fine to leave it alone. There is a possibility it will resolve on it's own over time.

I had a Lasik 10 years ago and my myopia has come back, I also have dry and sore eyes too often especially in case I lack sleep or I’m overstressed. Dr do you recommend another lasik? Thanks 😊.

Unfortunately My eye center doesn't have any such eye testing tools. They have only few basic equipment fr all kind of patients. More unfortunate thing is my eye clinic doesn't hv any dr like dr.allen who explain the eye issue so well elaborated...

I have so many questions. I am from south Africa and the only test they do here is the TBUT. Can you base your diagnosis solely on TBUT only? I went to 3 different doctors and each have a different diagnosis 🤐

Sir I want to ask you. Recently I noticed that my vision goes black and returns normal in less than half a second like if someone turns off and on the light very quickly. I should mention that i've been suffring from visual snow since 2020 and i deal with a lot of stress. My biggest fear is going blind and I went to an eye specialist last saturday for a full eye exam and he told me that everything is fine with my eyes. I also went to a neuroligist and she didn't suggest the MRI. I'm really concerned.

I have been diagnosed with Sicca. I also have been told I have dry eye. I have never been offered exams such as those you have spoken about. How can I find a physician in Massachusetts who would be as thorough as you. Or can you suggest a treatment. I do use proper eye hygiene. Can you help?

That really was comprehensive! Are all those tests done on the same day or are they spread out over multiple visits? How long would you estimate all those tests take to complete in total?

Hey thanks for asking. They are usually all done on the same day. The testing is all done usually within 15 minutes and then myself or another doctor comes in after our technician to do the assessment and discuss treatment options.

My upper right eyelid twitched for over a month. Then it stopped but always is tender and I feel a warm heat feeling in it. Not the eye just the eyelid. Eye doc said I have mgd and bleph. Gave me doxycycline but not getting improvement in the pain and warm feeling in that eye. Have you seen this? What is your experience with doxy? Thanks.

Warm compresses don't do anything for me. Also been taking vitamins and a lot of omega 3. I've had this issue 3 months after I had covid. I wonder if it's nerve damage? My doctor never did scans or anything just a dilation which came back normal. Throwing antibiotics at me which make me feel like crap. I also got floaters at the same time. It's brutal.

Hey thanks for asking. The warm sensation is interesting to me because lately I have been getting these feelings like my eyes are warm too. Eyelid twitching is usually caused by anxiety, stress, fatigue and over stimulation (caffeine), but dryness and allergies are thought to make it worse or more likely. Doxy is great at it's full dose for infections, but it is often prescribed at a lower dose as it can help with flora overgrowth as well as inhibits some matrix matalloproteins (mmp) and lipases. Inhibiting the MMP is good because these can contribute to corneal thinning and stopping lipases helps with the oil secretions and reducing inflammation. Issues with doxy - concerns over GI upset (including possible GI flora change) - Doxy can cause photosensitivty - dairy products inhibits doxy activity if taken at the same time - taking right before laying down the doxy can cause serious esophageal irritation (Bone, teeth, and some other concerns in young children) But otherwise it has been shown in studies to be good for MGD, various infections, even rosacea. Are you planning to follow up with your local doctor?

I don't know if il follow up I've been to 3 eye doctors nobody can find what's wrong with me. Burning pain and warm feeling for 5 months ruined my year. I've tried everything I've seen in your videos and what they have told me nothing gets rid of this. It's only my right eye.

I was DELIBERATELY heavily EXPOSED in a CT to my face, eyes and thyroid at a hospital in north Carolina. I am having dry eyes, blurred vision, facial burns, oral issues and no one will do anything about it or the women who did this to me!!!!! HELP!!!!!!

My corneas have zero sensation. I can literally tap my corneas with my fingers and feel nothing. cochet bonnet don’t even register. All due to smile refractive eye surgery three years ago. My a schirmers are also close to zero as is my quality of life. The surgeon told me I was a great candidate and dryness always goes away. His lie ruined my life.
