did tony not realize she was basically like a priest to him, washing away and condoning his sins? he could never tell her the truth about things he did but always expected sympathy. psychiatry actually helped tony in becoming a better criminal. the show made it look like tony was oblivious to these facts but i like to think deep down he knew and he was trying to manipulate melfi as he did with people in his daily mob activities

I think that was the point, in movie they use this technique to show different side of the person through interaction with some trusted friend and instead of monologue we know whats going on in protagonist mind .but main point is to manifest hypocrisy ,family values and other bs.

Carmela falsely confesses to the priests. Tony falsely confesses with Melfi. He tells Melfi his affairs, his crimes, even talks about murders. All to alleviate his guilt. And Melfi takes his money and even gets a thrill out of treating a mob boss until the penultimate episode. Melfi treats him even after he drives her to alcoholism.

I've been covering a ton of clips with James Gandolfini in The Sopranos and one peculiarity is his heavy breathing in many scenes, this scene here it was included after the psychiatrist tells him, "Get Out of my Life!!!" This man [actor] was laboring on the set. He apparently had an underlying serious health issue. Furthermore, people need to take it easy with calorie/carbohydrate intake and ration out their portions over a weekly setting. All you see in movies like this are Italians eating red meat, pasta, cheese, white breads, and gulping down bottles of wine every meal. Whatever happened to a diet high in protein and fiber? I think James Gandolfini gave us more than just superb acting. He also enlightened some of us to a tell tale sign of serious health issues.

Durins Bane Gandolfini was put on many sets eating lots of capicola, strombolis, red meats and high carb dinners. This dietary lifestyle [or lack thereof] could have poured over into his life off the set, or perhaps he ate like that his entire life. In any event I miss him a lot.

Melfi demonstrates why psychotherapy is just a huge crock, a big game. She earlier tells Tony that He can't blame himself for Gloria's suicide but then blames herself for her patient's suicide for not being there for her. In my opinion, clinical psychologists are the prime cause of suicides.

Watch the scene where she and Tony discuss Gloria Trillo's suicide: she (Melfi) regrets being unable to have helped a patient but knows Gloria had multiple problems and "slipped through everyone's grasp", including hers despite her "giving [Dr. Melfi's] patients everything [she's] got." Also, when Tony first tells Dr. Melfi she should "leave town" and says he'll "take care" of the guys who could "clip" her, she says, "Oh my god...people are going to get murdered, aren't they?" In this scene Tony may be thinking she blames herself for his committing murder; her reaction is outrage at his ignoring just how badly this has hurt her practice, including a patient's tragic delusion that the only alternative to meeting with her psychiatrist was to commit suicide. Finally, in this scene, she says, "Walking away is a choice. Ruing my practice? It's a choice." Part of what psychologists do is guide their patients to take responsibility (the mob's decisions have led to her having to lam it, resulting in a patient's suicide) or realize any fallacy in blame (Gloria was not suicidal because of Dr. Melfi or Tony--she even asks Tony, "Why are you so quick to blame yourself?").

***** That would explain Melfi's anger at having her practice compromised. She helped Gloria as much as she could, knowing Gloria was so unstable as to be unpredictable. This unnamed patient who committed suicide may have been honest about her suicidal tendencies, and her relationship with Melfi was a lifeline, as you put it, and telephone calls are not always as good as seeing someone face to face. Melfi could have kept returning to her office, but had she been killed she would have been lost to all her patients. Her practice, and this patient's lifeline (i.e. support system) was compromised because Tony sought psychiatric help when he must have known how the other gangsters could react.
