The fastest way to get a hernia in the gym is to perform the one armed dumbbell row with your knee up on a bench. In this video, I’m going to explain to you why this is the case and more importantly, what you can do to fix that once and for all. I will discuss the anatomy of the groin and why men are much more likely to incur an inguinal hernia than women when working out in the gym.

It starts with a bit of a discussion about the anatomy. When we are young, the testicles descend into the scrotum through the inguinal canal. In most cases, this canal closes off once this has occurred. In some cases however, the canal never closes off and you are left with a hole through the tissues in the lower abdominal region that compromises the integrity of the area moving forward.
Even if this closes properly, you still have the duct for the spermatic cord which passes through down into the testicles that creates a hole or compromise in the abdominal wall muscles that you cannot avoid. The stronger your adjacent muscles are in your abdomen, the more tension is applied to this area around the hole that is already present for the cord. This can lead to a high incidence of additional herniation and breakdown.
Many people think that the main cause of this is lifting heavy weights on exercises that typically would cause you to strain like the squat, bench press or deadlift. This is not the case. In fact, it is more so the positioning of the lower extremities during the lift that makes the one armed row a bigger culprit when it comes to causing hernia and inguinal hernias specifically.
Because of the way the exercise is normally performed (with one knee up on a bench and the other leg on the ground) you are asymmetrically loading the pelvis and creating a torque on it. The heavier the weight you attempt to lift from this position, the more torque is applied to the pelvis and the work must be done by the muscles of the core to stabilize and control the descent of the weight. As you pull from the lowest position of the lift, your pelvic muscles are in their weakest and most vulnerable state.
Add to the fact that you are likely placing an additional tensile stretch on this region by partially squatting down during the rep to gain a little bit more momentum and it makes this worse. The key is not to keep your knee up on the bench but rather keep both feet on the floor at all times. All that is needed is to straddle the bench rather than lean on it. With both feet on the ground you are not only more athletic instantly but you have applied ground reaction forces through the legs on both sides. This acts to stabilize the pelvis more and decrease the amount of torque applied to it tremendously.
Perform all reps of the one armed row on one side and then move onto the other side. Be sure of course to keep all other elements of proper row form in place like keeping a flat back, chest out and pulling your arm into extension on every rep rather than rotating to ensure maximal activation of the lats.

Hey Jeff, I normally do the same exercise by standing on both feet on the outside of the end of the bench and place my hand on the bench, instead of crossing legs over the bench. I simply use the edge of the bench for support. I have noticed that this also allows me to incline the head of the bench to change my support to the hand, as I bend over to pull the dumbbell. It is working for me but I'd ask you for your expert opinion.

I wished I'd seen this 9 years ago. I was in the gym everyday. I was doing this exercise twice a week. Got into great shape! Guess what? BAM!! Hernia. Got it fixed. Few months later, BAM! Another hernia on the other side!! After watching this I can almost guarantee this exercise caused it!! THANK YOU FOR MAKING THIS VIDEO!!

To prevent hernias you need to make your core stronger . Also try to avoid certain exercises that increase your risk of a hernia .

I have had that hernia repair along with a full stomach repair due to prior surgeries. I feel so much stronger now but have the constant fear of repeating the injuries somewhere else not repaired. I am glad to hear this and keep on making sure that I do my workouts correctly. It is AMAZING how many times i look at someone at the gym and laugh inside watching someone else do it wrong! THANK YOU for all these tips! I take them to heart and to the gym constantly!

Thanks Jeff! The timing of me seeing this video could not be more perfect. A buddy of mine at work literally just had a hernia in that area,. And as part of my dumbbell exercise routine I have been hitting that particular one arm row excersise hard so this is perfect timing, it will allow me to adjust my workout before the problem strikes. Man I just wanna personally thank Jeff for all of this advice he's given. I implement a good 60 percent of what he's advised in my workouts and not have I received noticable gains, I also got rid of alot of nagging pains I was experiencing due to my form. This is one of the best videos I've seen. Correcting form in a minor way, but that will impact so much. I recently thought I had a hernia and after watching this, I can confirm that it's true. I'm working through it opposed to surgery, and this adjustment should help me so much.

Yep got an inguinal hernia doing this one. I was only 18. Was out for a while waiting for surgery and then after the operation. I’m 100% now and have been lifting a lot safer. My main problem was I wasn’t breathing at all while lifting. Thanks for covering everything Jeff!

This is great for taller people as well. At 190 cm I often feel that most benches are too low. If I kneel I get some really weird angles. Standing solves that cause I can just raise the backrest thingy. Thanks again for saving me from unnecessary injury, Atlean-X!

Great Video.. As an OR nurse I see at least 6 Inguinal Hernias a day. Most guys don't even know they have a hernia or they know and think it will heal itself. Out of the 6 at least 1 of them is a lifter. Will try the advice. Thanks Jeff.

It will literally heal itself though as long as you can sort of push it back in and just continue to strengthen your core muscles. I have had (and have) both naval and inguinal hernias for about a decade now but manage to keep them both under control and mostly symptomless with the right kind of exercise routine. Only a fool would get an unnecessary surgery.

Nuclearcx I got a naval hernia in 2013, still have it. Gave up lifting and it never really got any worse, so decided to start lifting again and it seems to have gotten a bit better. I think it’s all about removing what caused it in the first place, and avoid making the mistake again. I don’t think they’ll heal completely, but be mindful of them and they shouldn’t get any worse

I did not know this information. But whenever I was doing the wrong form something always felt wrong. I switched to the correct position by myself and I found it to be much better and non straining.. Now I know why..

I wish I knew this before doing that exercise for more than 2 years. Now I have back pain everytime I train hard and I'm only 24 years old. I can't understand how personal trainers at the gyms continues to advice doing this exercise. Those people should have enough knowledge to distinguish a good from a dangerous exercise because they have a great responsibility for the other people safety. I would appreciate very much if you could make an entire video about exercises which can easily provide injuries and for that reason should be avoided 👍

Thanks Jeff for this video and great information. I do dumbell rows wrong and luckily avoided getting a hernia even with 120 lbs dumbbells. I will now do it the right or safer way as you demonstrated in your video.

An observation about deadlifts: many disc herniations suffered while deadlifting do not reside from making a "round back" or doing the exercise otherwise wrong. It is from anterior pelvic tilt. If you have anterior pelvic tilt, from a certain degree on deadlifts are a no-go until you fixed that, no matter how correct you conduct the exercise. While "round back" rather leads to a L4/L5 disc herniation, anterior pelvic tilt rather leads to a L5/S1 disc herniation. The herniation Jeff describes in the video is L4/L5.

Thanks! I had surgery 5 weeks ago and it was from crazy ab workout that I did because it caused me pain, thinking pain=progress. Anyway, just thinking your rows should be done facing the bench, with a hand on it. That way, the bench doesn't get in the way and you are completely symmetrical. Love your videos.

I find I sleep better, wake up without as much neck pain, without a pillow. I messed something up when I flipped off my 4-wheeler last year and heard my neck crack about 6 times.

Everytime I sleep on one side with my body resting on one shoulder I frequently wake up with shoulder pain and it lasts few days. I try to sleep on my back and with no pillow. Its really hard to fall asleep if not super tired but I wake up fresh and rested. I suggest you try .

Thanks, Jeff! This exercise has always felt awkward to me. The straddle thing makes more sense and feels better, especially when I'm using a slightly declined bench to get more lower lat activation.

I think you have explained a pain I have had on and off for years. I am now 56 and back in the 80s I use to weight train. Maybe once or twice a year I get that shooting pain right where you seem to be pointing. I didn’t train hardly at all for twenty years or more but have been training now for a few years with part weights and body weight. I can’t remember the last time I felt the pain but your explanation seems to ring a bell for me. A Hernia.

Already knew what it was before watching. He's mentioned it before in previous videos. I had my sports hernia repaired back in Oct 2010 and it still bothers me to this day. Like he said, they're no fun.

I think I have one myself, I don't want to get a surgery, it feels really uncomfortable, have it for 3 weeks or so, trying to take an entire week from lifting. Sports hernia isn't even that wel known. Still in pain yourself?

First, I feel really fortunate I never had an issue doing this exercise for decades with a dumbbell that got above 100lbs in my prime. But I’ll switch to this: never too late to learn a better technique.
