Ma'am we do get higher percentages of tranexamic acid (10%) formulated in combination with arbutin(1.5%) and kojic acid(2%.) (Haven't used though.)

@@ReethuVincy22444422 Regulations are different in different countries, in some countries Tretinoin is available free of prescription as well. I would consult with a professional before using high concentrations, but that is just me usually erring on the side of caution.

Update I'm on week 5 of my tranexamic acid pill treatment, and the melasma has almost disappeared. I have had no side effects and an truly happy with my face for the first time in years!!!

It's work on me too. My 10 years melasma almost completely gone. But after I stop taking it, it's re-appear again.. 😔😔

I just received an order of Dearskin Tranex. It is a 5% tranexamic acid serum that includes l-ascorbic acid and licorice, etc. I bought it on Amazon. I found it through Penn Smith's You Tube videos. It has had no negative effect on my skin, but it is too soon to tell if it will have a positive effect on my age spots. I really love your no nonsense, well informed videos.

@@Contagiouslearning I found the formula to be much less effective on my skin than the Naturium tranexanic acid serum, so I switched several months ago. The Naturium does lighten my age spots.

Potential side effect are discussed in the video. And yes, pigmentation can always come back if there is a new trigger.

I had malesma for 8 months now its fading on its own. Can you please answer how many days will it take to fade naturally. And should I consider this treatment.and what could be the reason for my malesma. 1.I took too much vit c in lockdown is this the reason. 2. Deficiency of vit b12. What are the reasons ? I can´t answer these questions, as it depends on the individual how long it takes and because the reasons can´t be determined like that. Melasma is most often due to hormonal changes rather than vitamin overcosumption or deficiency, but if you want to know for sure, you need to see a doctor.

See a professional. Hyperpigmentation is hard to treat, and if the peel left you with PIH, you might be prone to it, so experimenting on your own could lead to more problems.

Hello dear i have the exact same problem as you after chemical peel i left with melasma on my cheeks under the eyes..then a doctor gave a laser peel and now i have even more burn😪he said is rare what happened to my face and thet he cant fix it.i literally dont know what to do and who doctor to trust anymore.im crying every day because i left with pigmentation scars...i hope you found a solution for pih

@@Neha-rh3wj Do you still suffer from acne? If yes, I suggest you see a doctor for a treatment plan, retinoids might be helpful. Indented scars can barely be changed with topicals, and the redness will fade with time. Tranexamic Acid might help a little with that, but other ingredients might be better - I have a dedicated video about PIE treatment on my channel if you are interested.

@@DoctorAnne yes i did go to the doctor and had a proper treatment , but my pimples relapsed. My doctor at that time also said the same that marks and scars will go with time but its 2021 now ... m still here with the scars and pimples

@@Neha-rh3wj You probably should go back for another treatment then, sometimes several courses are needed. But to be fair, the indented scars are probably not going to go away unless you do more invasive procedures.

Hi dr Anne , I have noticed when I apply adapalne 0.1% obove my eyebrow or forehead , my skin sometimes crusts which peel away and leave hyperpigmentation spots which even after 4 months are still there so I don’t know if adapalene 0.1% is bringing up hyperpigmentation or melasma from beneath the skin or is adapalene giving me new melasma or what ? I been using adapalne 0.1% on my skin on dark post inflammatory hyperpigmentation areas which are slightly pitted with dark post inflammatory hyperpigmentation on then for past 9 months without any fading of the scars .

Both is possible, it can bring out hyperpigmentation but also lead to hyperpigmentation due to inflammation through irritation. If it is combined with crusting, that might be a sign of inflammation. Maybe your skin above your eye brows is very sensitive?

Doctor Anne thanks for getting back dr !!!!! I don’t get any irritation from adapalene 0.1% though , no stinging or burning or redness . Only slight dry feeling on the cheeks . But I have stopped using adapalene on my forehead. Will the same thing happen with retinol 1% or is that milder than adapalene . ? I was prescribed adapalene to lighten post inflammatory hyperpigmentation or melasma not to come out in new hyperpigmentation spots.

@@dynamitedynamite2737 Adapalene is the cousin of Tretinoin and comparable in strength, while Retinol is the weaker brother of Tretinoin, so Retinol 1% is weaker than Adapalene.

Hello! I just came across your video. I have used a 5% trancexamic acid, but the cream was very expensive, I did however see results. I then came across the Inky List Tranexamic acid, but it is only a 2%. Do you think it is worth spending more on the 5% or trying the 2%?

I would suggest you try the 2% and compare the results you get, that way you can see for yourself if it is worth it for you.

I can't answer that question, it depends on many different factors and should best be discussed with your prescribing doctor. But two months and already seeing slight improvement isn't too bad. These things take time.

Anything that reduces melanin in the skin does slightly increase sun sensitivity, so yes, but not in the way retinol does.

I have a video series on Hyperpigmentation where I list the different options, but in total a combination of different ingredients is probably best, along with a visit to a doctor specialised in that, as some hyperpigmentation needs more invasive treatments than topical skincare.

Hi. Judging from the medication you are already taking, it seems you are under professional guidance already, meaning you have a doctor that is prescribing the Tranexamic Acid tablets and such. I suggest discussing that with them. Sometimes you'll need more invasive procedures like a certain type of laser, and that is best discussed with a doctor that you see in person.

Hard to say from afar. Acne can be triggered by various things, like weather changes, stress or hormones. You might either stick with it and see if it subsides or stop it and see if the acne goes away and then starts again if you use it again.

I have been using topical tranexamic acid for PIH in my face occured because of unfortunate accident recently will ofcourse update you after 4 weeks usage would like to hear alternative treatment option. That depends on the concentration, but in the concentrations studied no, otherwise it would not have been permitted for use without prescription.

No, melanin is produced as a protective response. But triggers could be irritation, friction or inflammation, not only sun.

I was recommended tranexamic acid and salicylic acid to treat my skin and to be used in alternate days but I havent used face acids before. Also I have combination skin with closed comedomes on my forehead and jaw. My skincare already involved serums and eye creams that have hyaluronic and niacinamide in it. How do I use these additional acids with it

Who recommended these products? If it was your doctor, I suggest asking them about when to incorporate these new additions. But in general Niacinamide and Hyaluronic Acid can be used with either of them, so there shouldn´t be a problem.

I have been using the dearskin tranexamic acid for probably about four months now and haven’t seen much of an improvement yet! I’m 77 and have had hyperpigmentation for along time sooooooo it might take longer to work don’t know! Anyway thanks for the warning about the oral which I won’t be trying. Great video, thanks 💜

This disease requires a diagnose and treatment suggestions from a dermatologist, so this is a question to direct to your treating doctor.There are usually other treatment options that come first.

would this work on dark red/purple scars on legs? i have sever dark spots/scars on my legs from bug bites, picking, etc. they are completely flat and disappear for a few seconds if i push down on them. i know laser treatment is an option but it's so expensive i'm wondering if there are other products/ingredients i should be looking for that would help. and i'm not sure if they are technically scars or post inflammatory erythema. my scars usually fade and turn white over time but these are taking longer than usual.

It sounds more like Postinflammatory Erythema from how you describe it, for which Tranexamic Acid is not first line therapy. I have a video talking about PIE and treatment option that might give you some suggestions.

Hyperpigmentation is a chronic disease, that is why it returns. If you want to get rid of it longterm, you will need invasive procedures.

I have used acno to treat acne but after six months now I have melasma over the upper lip the doctor recomended tranacix cream with 3% acid concentration but havent started yet as Im not sure if it is safe to use especially summer and the hot weather!

That depends on the concentration of your Hydroquinone - if it is prescription, you should discuss it with your prescribing doctor, otherwise you should be fine combining them. 3. There is no time limit. 4. Nothing applied topically will ever be the same as something you ingest, so if you can use it instead depends on the results you are expecting.

I’ve heard that when people with melasma stop taking tranexamic acid, it comes back. My PIH is due to injury, not melasma. So if I start taking oral tranexamic acid then stop taking it, would it come back?

Tranexamic Acid blocks the melanin production, but does not destroy the melanocytes (aka the cells that make the melanin), so any kind of hyperpigmentation will come back when triggered. This is why sun protection is so important when dealing with any kind of pigmentation disorders. The only permanent solution would be to destroy the melanocytes with laser or something like that.

Yes, you can use it, but I am not the best to give advise on microneedling - I personally do not recommend doing it yourself for most people.

I’ve just observed that upon using rosehipoil breaks me out. If you have acne or oily skin better to avoid rosehipoil. And ofcourse always listen to your skin.

I am about to get the Inkey list tranexamic acid to see if it helps to reduce my hyperpigmentation from acne on my jaw area even faster😂. But I have been using adapalene for 4 month now and already start seeing changes and differences on my skin, like smoother, brighter, and actually no more active break outs. So I wonder if I should try tranexamic in addition to that, since my skin is so annoyingly sensitive and doesn’t react to new products well specially another actives😱😂but thank you Doc for your time and knowledge! Do you use the Adapalene daily? If not, you could try using the Tranexamic Acid Treatment on the night off. Otherwise start slowly by adding it onmce a week with enough time in between both. But if you have to choose between Adapalene and Tranexamic Acid always go for Adapalene, it has much better data.

Doctor Anne yes doc, I do now, every night. Not at the beginning, since my skin can be very reactive, so I started once to twice a week, then every other night and on top of my moisturizer, and after the moisturizer is fully dried. I was so afraid that I will have really bad reactions from it since I read a lot of horrible stories from some people using adapalene. So I was super careful! But it is definitely paying off!

@@harrystyles8059 That is the same. The pigment building cells are there, in the skin. You can minimize the pigment, but they will start to produce again if there is a stimulus. There is no difference between oral and topical treatment there.

Yes, tranexamid acid is safe to use in the kitchen. For best melasma options I suggest talking to a dermatologist, it is tricky to treat.

Hi dr skinceuticals have a product called discolouration Defense with 1.5 % tranexamic acid and has kojic acid in too and niacinamide. There is a another product called melan trans3 duo with 6% tranexamic acid . These are topicals . Is the oral tranexamic acid better and will you get a rebound once you stop the oral or the topical tranexamic acid ,

We have more data on oral Tranexamic Acid than on topical, which doesn´t mean that one is better, it just means we have better data on oral application. But as I said I wouldn´t recommend taking a potent drug for cosmetic reasons and risk it messing up my blood clotting.

Doctor Anne hi how long should you use tyrosinase melanin inhibitors for to maintain or prevent hyperpigmentation, ingredients like arbutin , liquorice root extracts , mulbery , kojic dipalmitate. Does arbutin convert into hydroquinone in the skin ?

@@dynamitedynamite2737 All these things are for long term use, when you stop using them, the hyperpigmentation will come back. And yes, Arbutin does convert in the skin.

Doctor Anne omg so arbutin converts into hydroquinone in the skin ? So how long can you use a product with arbutin in it , is arbutin safe to use everyday twice a day indefinitely without rest breaks then like you need rest breaks with hydroquinone? Or will you need rest breaks with an arbutin product?

I just started taking the pill 4 days ago. I have pigmentation on my forehead, temples and above my lip, I've tried hydroquinone for years, steroid injections above lip (they do work, but it takes 4 to 5 months between treatments, it took me over a year to do one side). I did laser for 4 months, and last month was supposed to be the 5th and I was furloughed, so I waited until this month and all the pigment came back. Now on the pill, hope it works.

Doctor,I have sunspot and I use 5% Tranexamic acid+azelaic acid 10%.i use 1%retinol every 2 days...is it enough.?

That depends on your skin - if they lighten, then it is enough. You do wear sunscreen I assume? Because if you don´t, everything else is pointless.

I use Curilogy TA 5% with 4% niacinamide and .05 % Tretinoin. . They started me on lower % Tretinoin and we have increased with time. I still have spots but they are slowly fading. I frequently get compliments on my skin from strangers and acquaintances can’t believe I am 66

How about glutathione? People swear that it lightens their skin. I'm white and care less about light skin but I do have stubborn hyperpigmentation and women swear glutathione helped them. Please let me know your opinion. Thanks doc🥰

It is not among the first line topical treatments, to my understanding people get it as iv or oral supplement. If you decide to go that route, make sure to do so under professional supervision.

The dermatologist has prescribed both TA pills and hydroquinone for not-so-dark melasma on the sides of my eyes which I believe I developed last summer. I had never had it before and I moved to a very sunny region two years ago. I did take antibiotics last Spring-Summer but I did not take contraceptives and was not pregnant. Could my melasma be due to the sun alone and thus can be gone after topical treatment? After some research, I became very concerned about side effects and decided not to use the prescribed meds, but rather start with safer topical ones - just like you suggest in the video. Thank you so much for sharing your valuable professional opinion.

Melasma is very hard to treat, and if you had a consultation with a professional, I think it would be wise to trust their opinion and discuss any concerns with them directly. very often prescription is needed to treat even mild forms of melasma.

I have used The Inkey List tranexamic acid along with other products (retinol, Vit C, and niacinamide) that assist with hyperpigmentation. It did take a while to see any change in the pigmentation, but I do love the results. I am now trying Curology for hyperpigmentation that has 5% tranexamic acid in it. I’ve been using it for under a week, so I cannot give an opinion, but I hope it continues to do a good job. My hyperpigmentation is light, and I have to point it out for anyone to see it. I tan really easily so I try to avoid the sun and use a good sunscreen. Thank you for giving such an in depth review of tranexamic acid!!!

Doctor Anne it does, but it’s so rewarding!! Would you ever consider doing a video on women who are going through or about yo go through menopause? Besides the emotions and sweats, nobody tells you about what goes on with your skin besides it starts to dry out. I’ve always had oily skin and it’s changing back and forth. I feel like I go through all the skin types in a month. I would be interested in seeing what can stabilize my face both hormonal and anti-aging, so I can concentrate on all the other changes. Nobody really discusses this 😊
