Tuono and Rsv4 valve failure : The reason

I just realized my 2013 failure was on the same exhaust valve, inside #4. Hmmmm... why that valve?

Just pulled my heads on my 2013 Tuono after 16,000 miles and its exactly the same way. I found my exhaust tolerances way too low on 2 of the 4 cylinders. #1 and #4 were down about .13mm Vs the .20-.25 tolerance. Do you know if it was contact with the valve and piston that caused the major failure?
