1. Your katakana are wrong because it is ka-i-n, what you wrote is "ke-i-n". 2. He is right. "ka-i-n" in Japan stands for the character Japan writes out as "Cain". They specifically chose to do that. Just as they specifically chose to write "Cefca" for "Ke-fu-ka". If you do not believe it, import Dissidia Duodecim. The names are literally in there as Cain and Cefca. The "ka" line in Japanese is also used to mimic all kind of k-sounds, also when they come from a "c". And when making up their own names they even decide to ignore common pronounciation like the "s"- and "che" sounds, that normally come from "Ce", (similiar to how Tolkien did for characters like Cirdan or Celeborn) which is why they have no problem writing Cefca instead of Kefka.

Lightning and her crew just got added in Dissidia 012, though that doesn't answer your question of why they didn't add villains for each of them respectively. They just gave Laguna and her other friends an enemy in common with the regular Dissidia crew (Idk about Kain though) but she's just head to head with everyone else as the leader I suppose. It would've been nice to see more villains added to match the added 012 characters though.

FF XIII came out a very short time before Dissidia 012, so maybe they couldn't have fit Barthandelus or any other FF XIII characters in because of time constraints; Lightning was more than likely the only character that they had time to put in the game.

Like Lightning, she has an Aya Breya outfit, only available if you get The 3rd Birthday game, and Cecil has a "Knight of the Twin Moon" available for buying FFIVCC. KH Cloud, and Amano Tifa were only available for pre-ordering the game from Gamestop, and Amazon respectively. WoL has a free outfit from the PSN, the "Nameless Warrior"

Cloud never properly made it to SOLDIER...Lightning is more like Squall in my opinion... Lightning is initially cold as ice and was in a branch of the military in her realm, the Guardian Corps (like Squall was in his realm) and it took her awhile to really change her ways around people.
