What's the difference between a medical student, medical resident, fellow, staff physician or attending physician?
Medical teams can be so confusing, there are so many people taking on different roles. This video highlights the different levels of medical training and how many years it takes.
I'm a first year internal medical resident - I can prescribe medications, I receive a salary, but I can only work under the supervision of an independent physician, as part of an accredited university residency program.
In this video, I focus on the medical training in Canada (where I am!), which is similar to many countries... but if you know of differences in your country, let me know in the comments below!

As someone with 2 chronic illnesses, I spend way more time than Id like at the hospital and in doctors offices. Siobhan i just have to tell you- i PRAY for a doctor like you. Your demeanor and knowledge are amazing and you are so articulate!

I know EXACTLY how you feel. Typically I tend to see mostly residents for regular appointments and most of them just are not what one hopes for in a Dr. I had 2 Drs (residents both) very much like Siobhan and I can tell you I actually cried, for days when they moved on. Just met the new one on Friday and I'm keeping my fingers crossed that she turns out as personable and nice as Siobhan!

I know she is so nice! I see over 8 doctors regularly and none are as sweet as her! She definitely doesn’t seem like the type to tell you to “get over it” when you’re in lots of pain 💚

My rheumatologist I seen a few time, literally is Dr Google. He didn’t explain anything about my diagnosis or the medication he prescribed. His literally told me to google my diagnosis and educate my self. As well as Google the meds he prescribed me. I chose not to see him again. Glad my mom went with me, because even she mind boggled with this rheumatologist. Definitely a poor example of a Dr.

I randomly found your channel about a week ago, don't really know how, but your videos are so interesting! I am 17, not going to do medicine, and live on the other side of the world to you, so I really don't know why I found you, but I love your channel! Hi Siobhan, I just wanted to commend you on your excellent videos. I've only just discovered your channel, but your enthusiasm is infectious! Gaining an insight into the medical system in Canada is great. I have applied for medical school here in the UK and I am really considering relocating to Canada after I graduate. It's amazing that you stay so alert, even after 26-hour shifts.

I would say being a doctor is the perfect job. For example, my dad is today 48 years old, and has studied all his career on the side of his “regular” hospital work. You can evolve so much as a person and your knowledge can be so wide! And keep up the good work! Love your channel! Btw sorry for my bad English, Swede over here.

I haven’t been a subscriber for long but you seem to always put a smile on my face. If you get hate which everyone does they are just real bitter people because you always have a smile in your face and never have I known spoke bad about anyone so I don’t see how people could hate on you. I have worked in peripheral medical fields and I am especially interested in your case presentations like with Alex. I also hope that you never lose your sense of caring, and hone your observational skills. I watched 2 elderly Cardiologists doing rounds and teaching residents how much they learn by simply observing.

Hi , I just wanted to say a huge thanks for taking the time and energy to make these videos . I am from New Zealand and have a 1 year old daughter with heart condition (CHD) . From watching your videos this last week I have had a real eye opener to your hours of work and study especially , so when at the hospital this week with my daughter I let the house officer have some food before attending to us - all because of your videos hehe . I’m 14 in 8th grade in Seattle so I have quite a ways a way before I can even think about med school, but I also have a very good teaching hospital near me, UW Medicine. I’m certain I want to go into some form of medicine, right now the 2 surgical specialties I’m researching are general and transplant surgery. This video, and your channel as a whole as well as other medical channels, really help me understand the great field of medicine more. Keep up the great work!

3 years later.. I’ve been subscribed for about an year. I sooooo wish I was apart of your community then. I plan on becoming a D.O. soon, and then becoming anesthesiologist for an while then getting my own pedi office. Not sure if it’ll work out but I hope it will. Congratulations on all of your hard work, and dedication!! I can only imagine how tough at times(and rewarding) it is to be in your field of study and practice. An interesting side note. In parts of Australia, male surgeons are called Mr:) Thank you for sharing!

Hi, your videos are really informative! I currently work in the uk NHS as a speciality trainee in General practice ( I guess you guys would refer to us as your family doctor?). This is my fifth year as a doctor and - you’re right - the time goes by so quickly, it’s scary! I would’ve loved to have seen videos like yours when I first qualified as you’re so relatable! It’s so interesting to see the differences (although not major) in medical training from the US vs Canada. Our system is slightly different in that we take our first licensing exam after 2nd year in medical school and then the second exam is taken after 3rd year. There is a third exam that has to be taken sometime during residency but is not stipulated for a particular time. Most tend to take I️t at the beginning of residency to get I️t over with. Also, I know internal medicine is a 3-4 year residency program here in the US. Also, we have both MD and DO schools. I’m currently a first year at a DO school! Thanks for the video!

I really enjoy watching your videos! I'm a doctor myself. I live in Costa Rica and here you have to do 5 years of med school and 1 year of internship to get your license to practice medicine. After that you get to apply for a residency, I'm a Oncology Radiotherapy resident, I'm heading into my 3rd year in february, it's actually 4 years of residency. In my case I have to be On Call every 5 days in the internal medicine ward or the emergency room. I must say it's pretty tiring but I'm sure you can relate. Anyway just wnated to say that I like your videos and keep on going, good luck! :)

In Australia we have a bit of a different system - a first year resident is an ‘intern’ but they get paid - a second year resident is a ‘resident’ . A senior resident who is almost registered and thinking about specialising is called a registrar - we have bpt registrars (basic practical training - you’re not specialising yet and you’re a new registrar and haven’t done many rotations yet as a registrar) and apt (advanced practice training - the person is a senior registrar, has done lots of rotations and is about to be registered, and about to sit the physicians exam, looking at specialising) Then we have fellows, who are people who have passed the physicians exam but are continuing in a specialty field and have not done the final physician training in that field. Then we have junior or senior staff specialists/consultants - junior ones have just completed speciality training and seniors are rehearsed professionals.

I'm in law school, and I'm studying medical negligence / informed consent right now. Watching how your days unfold really brings some of the cases that I'm studying to life.

Hi , I am Selin from Turkey, i started my residency in pediatric surgery 20 months ago and it takes five years, too, i love my job but it is really tiring,i don't know how i tolerate that but the children make me always stronger, and thank you so much for the video, i am glad to learn about the titles, because i am sometimes confusing to compare with turkish titles :) now it is exactly clear , the process of medical school takes six years here, 3 years of 6 is for just theoratical training , 2 years for both of theoratical and practical training at the hospital and the last year is just for practise and no exam in the last year-that's really cool :)- and at the end of six year, you are becoming a doctor with no specialty and you can work in an emergency unit or in an ambulance or in a center of public health etc. but if you want to keep specializing , you have to take an important exam and than you choose your specialty, for example i graduated from medical school in 2014 and i had worked as a doctor in ambulance for 1,5 year and than i took the exam and choose pediatric surgery ... :) thank you again , happy to know you Dr. Deshauer and sorry for my mistakes in english, just i am still learning , bye

It's exactly like on Grey's Anatomy! But this is actually really helpful for me because I go to a university to have my lupus treated and sometimes before I see my doctor, nurses, medical students, and residents will be involved in the case asking questions or doing tests and it get's a little confusing to know who I'm talking to/what they're objective is if they don't explain what's going on. Since it's a research / teaching hospital there are always so many new faces around.

It's different in here where i live !❤️ I'm attending medical school here in jordan. Medical school is a 6 year journey (to get your M.D.)with one extra year called the internship (you have to do it to basically be able to practice or be a resident). Crazy to see how different med schools are! ✨

UK junior doc here, we use: house officer (first year postgraduate), senior house officer (second year postgraduate to core training), specialty registrar (specialty trainee), and consultant (senior doctor who has completed specialist training). Family doctors here are called GPs (general practitioners). Physicians and GPs use the title 'Doctor' while surgeons revert to Mr and Miss upon completion of royal college membership (a Royal College of Surgeons tradition due to the fact that surgeons used to be tradesmen and did not go to university).

In the Netherlands, medical school takes 6 years (3 years of studying and then 3 years of practical learning in different departments of the hospital), and then you have to specialize (which you are doing) which takes another 4-6 years! So we will be busy being educated for at least 10 years :)

Hello! I come from Viet Nam. I am the first year resident, too. There are a lots of differences in medical training program between your country and mine. We take 6 years to finish the studying at university and become a general doctor. After that, we also have to take an very big exam and about 50% all of us will continue studying and practicing at the hospital as residents. The remaining will go to another programs to take degrees before they can practice in medicine independently. Our resident program lasts 3 years for all specialities. For example, I choosed neurology and I have to study continuously in 3 years. Then, I can almost practice medicine by myself. I can study more to take higher degree if I want. I know that medical training program in my country has many limits so we are trying to resolve and getting better, catching up the world. Thank you for your reading!

Thanks for explaining all these terms. It was really a bit confusing for me before I had watched this video. From which stage on are you allowed to participate in surgery? As resident? And when you do night shifts - does always an attending physician have to be available? It is interesting for me to learn how things work in other countries.

as some of the other commenters have said, as someone with multiple chronic illnesses, i can only hope to come across doctors like you! i can only speak for my experience here in the US... but a lot of the doctors i have experienced (particularly 60+ year old male doctors) look at young women like myself as hypochondriacs or pill seekers... it doesn’t even really matter if there is medical evidence from tests backing up the symptoms we “claim” to have. if we experience something that is even a little bit unique (different than what they were taught when in school or different from what they have experienced while practicing medicine) we must be making it up. it can be REALLY hard to be taken seriously and get the help we need. it’s so sad and frustrating. i had a neurologist diagnose me with an incurable *chronic* neurological disorder based on my symptoms and medical testing results and then when he couldn’t cure me of it... he asked me if i had ever been tested for clinical depression because it may have been in my head all along... *eye roll* i wish i could say that was my first and only experience like that... but unfortunately it wasn’t.

In Russia we have the medical hierarchy: Chief of Hospital (Main Medical Doctor) Heads of Departments (for example: Head of Internal Medicine Department or General Internist) They has their own Department and every staff is doing what they say in their department (they are also teach their interns) Medical Doctors Residents (It’s a 1 extra year after being intern) Interns (3-year internship) General Nurse Nurses Housekeeping Manager Housekeeping Staff
