The thing that has helped me the most in therapy was when I let go of the idea that the Cluster B’s were columns with bullet-points listing symptoms and started thinking of “Cluster B” as a continuum with LOTS of overlap. Because my PD is a grab bag of all of them, my therapist just treats the traits that are causing me occupational and interpersonal problems and I’m okay with that. I wish Dr. Grande would make a video for others who have an unspecified PD diagnosis, because with all the overlap and co morbidity I am willing to bet you have other subscribers that are in my group. I was thinking the same thing. I think it is more helpful to find an effective treatment for things that cause you distress in your life rather than worrying about a specific diagnosis for one disorder (or many). I seem to have traits from all the Cluster Bs but I am trying to learn which ones I need to work on to have a more productive and positive life experience and which ones are actually beneficial to my mental health. Thankyou Dr Grande. I’ve been buying back to back books on NPD.. I thought it was narcissism. But after watching this I realized my husband (of 6 months)exhibits 100% of the traits of BPD. He refused to see a Psychologist.. but I finally got him to agree to marriage counseling.. it’s a start..

Interesting. I was convinced I was dealing with a narcissist as so many of the traits lined up but not quite. I genuinely felt the empathy thing and some other traits tended to come and go. Almost as if there was a split personality going on too. As well as having witnessed some bad panic attacks when challenged or questioned. I've seen this person repeatedly say 'I didn't do anything" while slapping herself in the face for 15 minutes.. on top of all the narcissistic wound stuff. So it's confusing to me to figure out what's going on with the person. Good to know there are overlaps. I didn't realize. I think I'm dealing with a big mixed bag. Either way being in a relationship with this person left me a shell of my former self, as is what apparently happens from dating a narc.

At a day program I go to for depression, during a group a therapist mentioned that narcissistic personality disorder is the worst and most harmful because of the lack of empathy for others that makes them prey on others..your video was helpful, i think the inability to empathize with others feelings needs or boundaries makes them potentially dangerous because they do not care if they hurt another person, they use them like puppets to gain what they wish. That therapist also explained that borderline is really more related to a mood disorder, and there is a movement to have it changed to emotional dysregulation disorder- whereas narcissistic disorder is a pathology like sociopathy, hallmark trait of lack of feeling or empathy for others and tending to prey on others to gain what they want

I found out about his BPD when my son told me he'd tested high on traits of NPD (narcissism) so I looked up NPD online and there, in my face, undeniably my BF's disorder in the Cluster B next to Narcissism.

don’t know why all the BPD here are seeing themselves as victims and can be run over by a narcissist. I am a male BPD , had 4 month relationship with a narcissist. I picked up her personality Overt narcissism straightaway. We were frank with each other that she does not have romantic feelings towards me and I on the other-side knew that She can never be a long or a life time partner and I told her that and she accepted it. When she tried to gaslight me , I used to stand my ground and not be defensive but in a good friendly way expose some of her narcissistic behaviour like always wanting to receive and not give ... not wanting to work on the relationship cuz there is no relationship in the first place .. Her double standards ... She had nothing to say .. she actually was silent and agreed with me ... I broke the relationship 4 months after ... and she was expecting that .... she accepted it as probably she knew she will get no more narcissistic supply with me ... one time she was trying to gas light me ... and I fully criticised her personality as being mean and nasty .... she told me that no one had ever called her nasty before ... so I told her .. that’s fine but what you did was not nice at all .. and may be nasty is a bit of a strong word but I am so disappointed in her .. I am on strong on very strong mood stabilisers and that had reduced my impulsivity a lot .. when I got upset I went into the passive aggressive mode .. which made her upset and did not like one bit but I stood my ground and explained ... well its better this way than to explode at her in a way that I am sure she would have more than freaked out and got the hell out of my life all together... we are still friends by the way ... I have quiet a bit of some narcissistic traits too ... especially lack of empathy which I developed in my early thirties ( I was very empathic before) probably as a defensive mechanism ...

When I worked as a psycotherapist at a private psychiatric hospital in the Pacific Northwest I found that, like all labels, the term 'borderline' was sometimes recklessly applied, and sometimes when I thought it was recklessly applied it was apt. That kind of defines the concept of 'borderline', which can be problematic to say th eleast because it is often reflected by lethally impulsive behavior.

The DSM symptoms do seem quite different. In practice however, there is far more overlap and the waters can be muddied. The person with NPD can feel victimized and misunderstood and the person with BPD can easily feel unique and unable to be understood by most people, among other things.

The common assertion that NPD/BPD _lack_ empathy might actually be incorrect or misleading. In some cases, I feel that the disordered individual DOES have the capacity to "place themselves" into another's shoes, it's just that they either don't care what the person is feeling or more importantly, they do care, but they GET OFF on knowing exactly what the other person is feeling (anxiety/confusion/fear). The only difference between my sadistic, high-functioning BPD ex-wife and a psychopathic serial killer is that my ex-wife never needed to dispose of a body. But, she clearly derived great pleasure from being abusive as well as from her ability to invert calm, pleasant situations into pure, manufactured, non-sensical chaos.

BPD here, your observation is correct, i like to use the term "distorted" empathy, it's sort of a childish trait. As a matter of fact that's another trait all Cluster B "cousins" share. For instance i can understand your pain when you tell your story but i would lie if i said "i'm sorry". To some extent (some) B's can be the most honest people you'll ever know cause we lack..filters. Unless we manipulate. All i can tell you is something my therapist likes to remark: there's no such thing as good or bad people, only people who suffer. If you're healthy, try to enjoy life as much as possible cause it's hell on earth this side of the barricade, even during the healing process. Everyone has the right to be happy. Regards

Actually, borderlines tend to have MORE empathy than the average person. Notice that lacking empathy is a symptom criterion for npd, NOT bpd.

What about a recurring situation where you can definitely place yourself in another's shoes but are so battered and worn by years of internal negativity, hopelessness and anxiety that you deliberately begin to distance yourself from that as to not add to that cesspool? As if feeling that would be the straw that breaks the camel's back, so to speak. I've always considered myself an empath, but over the last 3-4 years of a difficult relationship, tough life situation and therapy I've grown to see myself as a sadistic puppeteer who can only operate personal projections and cannot experience the true empathy of connecting with another's real emotions. Then again, that's the assessment of my ex which i've taken to accepting.

Thank you for posting this video !!! I suspect that a family member has BPD/NPD/and Dark Tetrad traits. It is a co-mobidity, because the behaviors (from each disorder) rapidly shift, and the only way I can describe it coming from someone who is functioning, despite horrific abuses/victimization is like "emotional whiplash", and/or "navigating through a landmind of eggshells". Because you will never know what "sets" them off, and they have a failure to take any responsibility for their actions.

I am really confused now. She has both symptom sets. How they manifest depends on the day and the specific topic. On one had, you speak as though it is not uncommon that someone shows signs of both. Yet, some of the differences seem to be consistent. Why does it matter? Well, I just watched several videos by people who claimed to be diagnosed with BPD. Their story implies that I just needed to be more patient, more tolerant, more understanding and speaks little about manipulation. On the other hand, I don’t know for sure that she lied, was unfaithful or was being truly manipulative. I think no matter what I’m. It going back, but i will have regrets if I could have somehow fixed this instead of walking away. No contact isn’t the best or only answer to everything.

I am guessing here, but from a clinical point of view, it is probably very easy to misdiagnose many narcissistic personalities as having some other problem. As far as I know, the structural feature of "pathological grandiose self" (Otto Kernberg) is often assiduously hidden by narcissists. Basically, a clinician can often detect anxiety and depressive mood, but it takes a careful investigation to find out that the anxiety is about being dependent on those "miserable creatures" and the depression is about being defeated by those "miserable creatures". It would be very interesting to know if in your clinical practice you have encountered difficulties in diagnosing narcissistic personalities, especially if the client is not a stereotypical extroverted arrogant type. I agree that there are aspects of personality disorder assessment that lend themselves to misdiagnosing. Differentiating between Borderline Personality Disorder and Bipolar is particularly challenging. Distinguishing between personality disorders is also difficult, as they are similar in many ways and often more than one can be present simultaneously. Specifically to your question, Narcissistic Personality Disorder is typically not especially difficult to recognize among all of the personality disorders, however, it is often only clear after seeing someone over several sessions.

If a diagnosis is what you're looking for, good luck, because I've tried getting a diagnosis for AVPD as well as asking if I had BPD, psychologist said he thought I had AVPD, but not BPD because I didn't "act" or "looked" like I had it. He said he wasn't going to diagnose because PDs are looked down on and it'd be hard to get a job. He even told me most Drs refuse to see or treat BPD patients. I don't know what it's like everywhere else, but in California, USA there is a lot of stigma, so just be prepared for that and not getting the help you need.

Borderline Personality Disorder with type B traits lies on the border between wickedness (Narcissistic Personality Disorder) and dullness (all the other Personality Disorders). BPD without type B traits is dull. The wicked stuff is that NPD and BPD doublebind society, because for a NPD you can't be a Narcissist (narcissistic traits) while for a BPD (with type B traits) you must be BPD.

I don't know much about this topic yet, as it is still a bit new for me. But one important difference between BPD and NPD that I seem to be observing is in the aspect of emotional empathy. The BPDs I've seen so far (very few, of course) seemed to value empathy a lot more than NPDs. I think their overriding sense of guilt arose from the experience of not being able to be as empathetic as normal people. People with NPD, on the other hand, seemed to accept lack of emotional empathy, without guilt-tripping over it. You mention that these disorders tend to be co-morbid. In your experience, have you seen many BPDs who lack emotional empathy?

Does the boarderline Identity disturbance, stem from the age - pre-toddler issues that created a lack of object permanence, meaning this occurred at a age before they established a personal individual sense of themselves. While a Narcissist doesn't display the ID disturbance because either the abuse or the causal factors of the disorder - occurred slightly later in childhood after the child had established a sense of self/personal Identity?

I don't know whether you're still taking questions, but I'd like to understand better the role of grandiosity as a protective or devaluation mechanism in borderline. A friend of mine has shown rather clear borderline-like behavior for half a year that was easy for me to deal with because I was not particularly special in her life. But now she switched into what is probably the devaluation phase. Her personality changed drastically, she stopped being self-reflected, and became rather “pompous” (not necessarily grandiose) to put me down. (I'm fine.) I don't think she thought that she was the best at everything, but her point was that she's better than me and much better than I think she is. (Even though I've always thought highly of her.) Is that grandiosity? How common is that in BPD? Is she generally in this mode now or only when interacting with me?

Would you agree that they are the two different sides of the same coin? Each one's symptoms are the differentiated manifestation of an underling insecure attachment which are manifested differently because of the differences in which that insecure attachment came about. i.e. someone that had a anxious or disorganized insecure attachment parent who at times might have been loving and at other times was cold and distant rears a child who in adulthood has frantic insecure attachment always yearning what they got a taste for developing BPD. Where as someone raised by a narcissist parent with avoidant insecure attachment who never gave the child any affection would develop that that similar insecure attachment and a more NPD? Or is the oversimplifying it?

Is it possible for a male 12 or 13 y/o be diagnosed with mood disorder and then as young man say age 23, gets his life together has been able hold the same job(union job- as a first responder)has had some problems. He is very attractive, athletic body builder but now seems to have NPD!!! So very rude to his Mom whom he always had very close relationship with and is constantly disrespectful. He withholds his three y/o baby girl from her grandma(who is terminally ill)he curses his Moma in front of the Baby and even attempted to fight his Father in front of the Baby after some playful teasing after a boxing match and Dad won the bet. The WHOLE family can NEVER have an enjoyable gathering because he must ALWAYS cause some type of arguement or disagreement in front of the Baby and makes like they caused him to act out and its their fault!! The grandparents do not defend themselves in fear of scaring the Baby!!! Can someone please offer some insight? So very worried for the entire family. This pattern has been going on for almost five years. How could someone rescue the whole City and can't let their own Mom enjoy her remaining time on earth enjoy her only Grand Daughter??

These disrinctions are really where the DSM fails. The ICD, the system that is used in most countries outside the USA, has got rid of all of the old personality disorders and replaced them with a dimensional model - it allows for a much more holistic diagnosis and doesn't try and shoehorn people into artificial little boxes like the DSM does.

People with BPD have empathy, perhaps too much empathy. Our emotions run wild sometimes.Wild mood swings , often lasting only minutes.As far as histrionics go, some people with BPD act out , have wild tempers.and can explode when triggered. Other "quiet" BPD people , such as myself, tend to keep the rage internally, most of the time.

Lack of empathy is symptoms of ASPD and NPD, and while people with BPD/HPD can act in a way that seems self centered, most (unless they’re comorbid) do not lack empathy and Borderlines specifically are often characterized by the great level of shame and guilt they feel for hurting others

Borderlines do have empathy so lacking empathy is not a symptom criterion for bpd like it is with the other cluster b's. In fact, studies on bpd found that borderlines tend to have TOO MUCH empathy.

@yerowww666 No, do your research because the studies on bpd actually say the exact opposite of what you're claiming. They found that borderlines have MORE empathy than the average person and are BETTER at reading emotional expressions than the average person.

How could someone possibly have both? Narcissists have no empathy; borderlines do. I suspect both being diagnosed in one individual is due to misunderstanding.

They are the same rhing. Actually npd is the result of bpd in a self defence. They feed each other in a cycle.

sounds somewhat plausible actually. but that's just from my personal anecdotal experience. i'm diagnosed with bpd, but am often feeling the onset of npd symptoms when dealing with conflict

Looking at the thread shows that I watched this video and commented a number of times before I finally got it. Got what this means For Me. Before I finally didn't just decide that it was over--but left. Before I didn't just leave for a while and consider going back... But 💣 Saw Reality.💣 💥❤️🔥💥 --FINALLY Understood!!! I researched and reconsidered Many times Before he finally broke everything, from the capillaries is under my skin to the trust of my only family--my elderly and Schild mother. To the trust of my own foolish heart it seems to have taken **many times** before I got it and went so far away that nothing could ever drag my poor my heart back. No Hoovering could ever dragged me back. No *not hoovering* me me could ever make me wonder "why doesn't he still love me anymore?" and go sniffing around. I just want to get away. To Escape To end it. Forever. I want to say goodbye to my poor, broken BPD hearted, narcissistically TAINTED, Once greatest love🗑🚮🗑☢️☣️☢️ 🗑🚮🗑 I wanna say goodbye and leave it forever. Even my heart Had to see Reality Eventually I couldn't see the forest for the trees. I needed the diagnosis. That's why doctor Grande and others like him -- that's how they helped me. !!! They gave me the Clarity of the forest in exchange for the wild confusion of the trees. 🌀🌀🌀
