FLAWLESS BRIGHTLY SERUM - NOW AVAILABLE ➯ bit.ly/3gEHfyt
If I had to choose just one Active ingredient which could deliver multiple benefits in many different clinic situations, Azelaic Acid would be a strong contender for top position. This versatile multi-tasker is perfect for patients in with ‘overlap’ concerns - a dab of rosacea on the background of acne, or melasma and rosacea both co-habiting on the cheeks.
I like:
1) Sesderma Azelac Spray
2) The Ordinary Azelaic Acid Suspension 10%
3) Paula's Choice Multi-Correction Treatment
4) Jan Marini Roslieve (my favourite, as the formula applies so nicely - many Azelaic Acid products are unfortunately quite gritty. )

Please, please, please, keep doing videos on rosacea. We are so alone! I just asked my dermatologist about azelaic acid to treat both my rosacea and SD, and she told me it's an "old-fashioned drug" which can cause irritation, and she discarded the idea. But, she didn't provide any other treatment, other than a Ucerin cream for atopic skin. I was thinking about changing doctors. Thank you.

I would love to see a video on what active ingredients you can use together or what should be used alone. There seem to be a few that you recommend Vit C, Azealjc acid, Niacinamide, Retinol but how do these combine in a morning/night routine over a week?

Not Dr. S. obviously but I have some experience in this. Niacinamide is not an active ingredient, so it will usually be found in many many other types of skincare. It can be used with anything, anytime, anywhere. Active ingredients, however, work best by themselves so that the skin can get the full benefit of them without being negated in some way by the inclusion of other actives that may work against it. My philosophy is Protect in the AM and Repair in the PM. Thus, I use my vitamin C in the morning and the exfoliating acids at night. I use the exfoliating acids after I remove my make up and wash my face, and then wait at least 20 minutes to apply the retin-A. I usually take off my makeup after dinner (when I cleanse) but brush my teeth right before bed (when I use my retin-A.) 20 minutes is enough time for an active ingredient to be fully absorbed. Sorry for being so wordy. HTH.

I don't understand exactly what it DOES. AHAs dissolve the glue binding skin cells together; BHAs dissolve the oil in pores; Retin-A encourages new skin growth; benzoyl peroxide kills the p. acne bacteria. What does azelaic acid DO?

On me, it has improved my skin texture (pore size, smoothness) in a faster and more noticeable way than BHA has ever had. I still use both, but the azelaic I use daily. Unfortunately, I haven’t seen a huge improvement in redness but mine is more related to broken capillaries and those do not respond to topical treatments. It has taken longer (2 months) of almost daily use to start seeing an improvement in my very mild hyperpigmentation. Overall, I’m in love!

It does many things!! One of its benefits is it decreases Toll like receptor 2 activity on the skin; these are the high-alert receptors which get switched on by environmental triggers, and activate skin redness and inflammation (hence AA reduces both of these). TLR2s also sit on sebaceous glands where their activation can trigger comedogenesis, so azelaic acid reduces this too. Finally it suppresses tyrosinase, reducing over-production of melanin.

Boots can supply 20% azelaic acid (brand name 'Skinoren') on a private prescription via their online acne clinic, as well as Differin. I believe the cost is just over £20 for one tube and you have to fill out a questionnaire and send some photos of your acne first. Superdrug run a similar scheme with a wider range of products - including Epiduo and even antibiotics, but no azelaic acid. Always best to visit your GP first, however. An NHS prescription will cost far less and might even be free depending on your age, where you live (only England still has charges, which is scandalous in my opinion!) and your personal circumstances.

Hi Dr Sam. Apparently Finacea and Skinoren - both manufactured by Bayer plc - are unavailable to pharmacies due to production issues. Have you heard anything about this? In the meantime I'm going for Azelaic acid at 10% by The Ordinary. The prescription I got from my GP was for the 20% version used twice daily, which I think is too much too soon. I expect there to be irritation but going so hard so soon seems a bit much. Would rather start off cautiously and build up a tolerance over many skin cycles. Thanks for all your videos, they're very helpful to people like me.

Would it be too much if using 2% BHA Paula's choice Resist Pore Treatment (OR 1% BHA Paula's choice Calm line) + Vitamin (SPA 10%) serum with vit. E and ferrulic acid + A product with Azelaic acid with B3 and B5? I am 29 and am looking for anti-ageing properties (also getting a Paula's choice SPF 30, now I'm using 50+ but it's rather sticky/tacky under the foundation). ALSO I am prone to very mild rosacea, have dry skin on cheeks and semi-oily T zone. There are just a few little red veins visible on my cheeks and at the corner of the nose, otherwise not really visible, but rather I am prone to blemishes. I have never used acid exfoliants before, nor the vitamin C. I would like to combine the 3 products I mentioned above, not neccessairly together (some in morning, some in evening routine). I like them for antioxidant and brihtening properties (vit. C), for evening the barrier (azelaic a.) and for mild exfoliation/cleaning of dead cells (BHA). Everything sounds fantastic, BUT I am just a little concerned that this would be too much of acidic stuff at once! What do you think?

Hi Dr.Bunting, Is it really normal that it itches after applying azelaic acid? I itch a lot when I first used it and I rinse it off right away because I am scared that I am allergic to it. I used clindamycin before I put it. Please help me, Dr.Bunting. Thanking you in advance, Doc.

Loving the content of your recent videos. Very useful and informative. Thank you so much. I use financea for rosacea and I find it helpful., I also have melasma so am happy to hear it is good for that too. Would you say that it is slightly drying? Also does it also act as a mild exfoliant?

How long would a client use this at 10% before you'd expect to see real results, please? I've been using once a day (night) for 2 months and would say it mostly calms skin down for a few hours. I'm hoping it'll suppress multiple pigmentation problems - certainly they haven't got worse, just not started to clear/disperse. Overall skin tone/quality is definitely better). Thanks!

Going to give it a try it from the ordinary. Can’t get it Canada so have to grab it over in the US. Ordinary won’t ship it here either. I guess it’s via script only here.

Great video (as always)! 😃 Thankfully, I only have some rosacea to deal with these days. 😊 Kind of off-topic, but at what age did you personally start wearing sunscreen everyday? You have inspired me to take better care of my skin -- thank you! Also, at what age would you recommend start using Vitamin A derivatives? I'm 22... Thank you!

Start SPF daily as soon as possible! Then Actives based on need eg if you have acne, use a retinoid. If your skin is just a little dull, try an AHA.

Hi, is azeliac acid for rosacea or not?? Most people here are talking about scars and acne, but I have rosacea and pustuals and redness, plus I have parts of my skin on my chin, forehead and t zone that's between my eyebrows, like large holes in my skin and it looks like orange peel, and it's like I get new bubbly skin in like circle shapes and lined shapes that do eventually go away or go down, but some dont, and I'm so depressed about it, all the dermatologist prescribed me was a cream you put on twice a day, which is in morning and at night, and I been using it since September 9 /2019 and also he gave me doxycycline which and harm the teeth, cause I used it for like a month and 6 days, and I looked in the mirror one day and my teeth looked yellowish to going a light brown color, and I was so disappointed I just stopped taking them, and I went back to see the dermatologist and I told him about what happened to my teeth, and he said that's not possible, it cant happen, and it would only happen maybe to younger kids like 12 of age, I was like no it happened, and he didnt believe me, so I asked for the minocycline which is in the same pill family as doxycycline and tetracycline, but I was diagnosed with a fatty liver just a month ago, so I did look it up on Google and it says it can harm your liver further, so I'm at a loss right now, and dont know where to turn anymore, the cream he gave me bee's mixed up at the pharmacy and it contains ketoderm/ nortate 1% with 1% HC powder so please help, can hydrocortisone wh is the HC powder do more harm then good, cause I feel like it, my skin was not that bad till using this cream.????
