Conductive and Sensorineural Hearing Loss and deafnes for students preparing for the USMLE.
CONDUCTIVE HEARING LOSS DEAFNESS
Conductive hearing loss invovles the part of the ear ranging from stapes to outer ear. Rinne's Test is negative meaning the bone conduction is greater than air conduction. Weber's test lateralizes to the bad ear. Absolute bone conduction will be normal. Pure tone audiometry will show normal bone conduction and the air conduction will be lower than 10 decibels known as the AB Gap. Rarely goes below 60 decibels.
Congenital causes include, meatal atresia, ossicular discontinuity, stapes fixation. Acquired causes include obstruction such as ear wax and foriegn body. Tympanic membrane perforation, infection. Ossicle fixation or disruption of the ossicles. Eustuchian tube may also cause.
SENSORINEURAL HEARING LOSS DEAFNESS
Sensorineural hearing loss involves cochlea and cranial nerve 8. Rinne's test is positive meaning air conduction is greater than bone conduction. Weber's test lateralizes to the good ear. Absolute bone conduction in sensorineural is decreased. Pure tone audiometry in sensorineural hearing loss will show drop in the higher frequencies and there will not be a large ab gap, but goes below 60 decibels. Speech discrimination will be poor in sensorineural hearing loss.
Acquired causes include labyrinthitis, physical trauma, noise trauma, acoustic neuroma, presbycusis, Meniere's disease, drugs, systemic diseases (DM, Hypothyroidism), renal syndrome, autoimmune diseases, blood dyscrasias and multiple sclerosis.

About the Rinne's Test.. Is it possible that ac is better than bc? If Rinne's positive then it means that ac and bc are equal. Cause ac can't be better than bc.

Thanks for explaining it so well. My father also loss hearing after he met the accident then he used many medicines but nothing works then we came to know about an Ear care pack from Planet Ayurveda. He used it now he is fine.

I wanna ask something how they compute my average hearing I dont have any idea how they calculate my haering..because I worked as crew in cruiseline company which they set required standard hearing average both ears which is 30/40..my left ear is good and i got 32 average while my right ear I got 42.50..this is the reason I'm unfit to work onboard..can you explained to me how they compute my right ear..thank you.

I think for the conductive hearing loss A-B gap should be 20dB and not 10dB

Great explanation! I am just wondering, when you are referring to the different type of test where BC>AC, what do you mean by greater? Greater in what way? Through the amount of noise that is transmitted? Or the level of noise?

Greater means that person can hear that for a longer time or better.

That implying the sound produced by TF when struck. So AC greater than BC means that air conduction of the sound produced by the TF is better/louder/longer than bone conduction FOR THAT PARTICULAR TEST EAR.

No, that statement is actually wrong. It should be AC < BC ( For conductive HL) and AC> BC ( Sensoryneural HL)

Thank you. I had a radical mastoidectomy was rebuilt with bioglass. 4 yrs later certain sounds hurt my ears a kettle boiling I'd get a sudden static noise or in the car with window open. I'd always moan for ppl to speak then moan they're shouting 🤣 I do have chiari malformation too. Been told at 40 I'm young to have recruitment hearing etc so do I have conductive hearing loss because of the radical mastoidectomy or is is sensor neural becuase of the recruitment?

I have random episodes of sensorineural hearing loss on my right ear... I lose about 40 decibels of hearing. My hearing would usually come back within 2-3 days. It's been going on for a year now. It's happened about 6 times. It's usually triggered by being around loud environments for long periods of times, Smoking weed, or drinking. It's also accompanied by Tinnitus, a low hum. Why is this happening? Will it ever be permanent? Can I go death? What can i do to avoid this?... How can I go about healing my inner ear? Someone help :( I'm only 23 years old...

Puro South Do you have vertigo as well ? Since its associated with smoking ,it may be Meniere's disease. Just a guess . Its common in 35-50yrs though and there must be vertigo lasting 20min or longer. Again it might be due to use of ototoxic drugs that damage hair cells. Or it can be noise induced hearing loss if you are chronically exposed to sound of large intensities.

I just had my very first episode of Vertigo 3 weeks ago.. I got dizzy and lost balance, It was ongoing for like a week until it started to settle down day by day. My ENT also said it might be Menieres.. Got treated with steroid shots again. I feel ok now. But now I'm wondering if Menieres disease is a life long disease or will it go away? Can manieres cause deafness? How do i treat it? Right now im praying for Frequency Therapeutics & other companies to come up with a break through cure... But a cure is still about 10 years away. I keeping my fingers crossed
