I'm not a dancer. I'm working in the forest as a firewood cutter. Man, my big toe is giving me fits. I will try this.

Typically 1/8th inch or 3 mm, some patients need 1-2 mm thicker. Rich

I dont understand about not putting a dancers pad “on” the metatarsal heads. We want to still allow the ball of the foot to make unadulterated contact? Having the orthotic right behind the ball of the foot?

Sorry Don, really not sure of your question. But, I think you are discussing the various ways to effect the biomechanics of the big toe joint. At times you support the big toe, at times you attempt to off weight it, at times you immobilize it, etc. I recently counted 34 things that I do to affect the big toe joint with dancer's pads only one of them. Do you guys recommend doing this and taping up the foot in a way the prevents the big toe from being bent backwards in the case of turf toe?

Why not put the pad directly on the sesamoid

Patients have to experiment with padding, and thus cushioning the sesamoid directly, or off weighting it with a dancer's pad, or both. A lot of trial and error! Rich

but this orthotics dosen’t have an arch support would it be comftable on the long run

Gilbert, of course, in the long term, if you need this protection, it is normally attached to an orthotic device. Rich

Richard Blake thank you 🙏- i have sesamoditis since 4 month i have 0-2 pain levels and swim 🏊♀️ alot- just a question please i m always non bearing since i when i remove orthotics pain is 4-6 ... is it ok to stay non bearing 24 hours.. i m afraid this could weaken the bone on the long run

@@gilbertabdo8813 Healing is best marked in functional gains unless we know the entire diagnosis and the treatment is very regimented. September should show more function than Aug and October more than Sept. If not, we have to decide on treatment changes or more diagnostic tests. Rich

Won’t that out more pressure on the toe joint? I have orthotics which have a hole cut underneath the toe joint and then a layer of memory foam underneath allowing it to ‘float’ over. I tried something like this myself and it hurt more

It can for sure if it just creates a hole to fall into. It must keep your weight more in the center of your foot. The idea is sound but sometimes it has to be combined with inside arch support when the balancing act is delicate Rich

@@drblakeshealingsole my issue is hallux rigidus and a broken sesamoid (both feet due to walking at work). What do you propose as a solution? I was given a custom orthotics with memory foam meant to ‘offset’ the pain. But theee is nothing placed under my big toes so I’m unsure as to how it’s meant to work.

@@rcoz6391 I got custom ones which I’m in the process of trialling. They’re ok, but still a little pressure on joint still. I’m finding that stretches help more than the actual orthotics. Unfortunately, shoe choices are limited and I’m left with only Hoka. So during raining season I can’t go out because they aren’t waterproof 😂
