Diabetic neuropathy is nerve damage caused by diabetes. It is the most common diabetic complication, affecting at least 50% of all patients.
Chronic high blood sugar levels cause progressive injury to neurons. Sensory neurons are usually the first to be affected, followed by autonomic neurons that control internal organs. Rarely, motor neurons that activate voluntary movements may also be impaired later in the disease, albeit to a lesser extent.
There are several types of diabetic neuropathy. A patient may develop more than one type at a time.
The most common is peripheral neuropathy. It’s also called “distal symmetric polyneuropathy” because it typically involves multiple peripheral nerves, on both sides of the body, and affects the longest sensory axons that convey sensations from the feet, legs, and hands. Symptoms develop on the body in that order and include numbness, tingling, or burning sensation, which usually worsen at night. Sensitivity to pain can either be exaggerated, or, on the contrary, lost. Loss of sensation leads to increased risk of painless injuries, which often go unnoticed and therefore untreated. This, together with slow healing caused by diabetes, can turn minor cuts or blisters into serious infections.
Autonomic neuropathy affects nerves that control activities of internal organs.
In cardiac autonomic neuropathy, nerves that regulate heart rate and blood pressure are damaged. As a result, the body reacts more slowly to change of position, and patients may feel lightheadedness when standing up. Other signs include rapid heart rates, or sudden, unexplained changes in heart rate.
In the digestive system, nerve damage may cause nausea, vomiting, loss of appetite, difficulty swallowing, slow stomach emptying, and bowel problems.
Neuropathy in the urinary and reproductive systems may lead to urinary retention, hesitancy, urinary incontinence, and sexual dysfunctions.
Nerve damage in the eyes can cause slower adjustment to changes in light and darkness.
Nerve damage in sweat glands may result in either absence of sweat or heavy sweating, especially at night; different parts of the body may also produce sweat differently.
Another effect of autonomic neuropathy is hypoglycemia unawareness, meaning the body is unaware when blood sugar levels are low. Warning signs such as hunger or dizziness cannot be felt, and patients may pass out before taking steps to increase their blood sugar.
Mononeuropathy, or focal neuropathy, is dysfunction of a single nerve, typically due to entrapment. A common example is carpal tunnel syndrome, in which the median nerve is compressed as it passes through a narrow tunnel in the wrist, causing pain, numbness, and tingling in the hand. Damage to a cranial nerve can cause problems with vision or loss of control of facial muscles.
Proximal neuropathy is a rare type of nerve damage affecting the thighs, hip, buttock, and occasionally, the abdomen or chest, typically on one side of the body. Symptoms include severe pain in the affected areas, and thigh muscle weakness and wasting.
Diabetic neuropathy can be prevented by effectively controlling blood sugar levels. Managing diabetes also helps slow the progression of damage.
Treatments aim to relieve pain and symptoms, to restore functions and prevent further complications. Treatments vary depending on the affected organs.

I have had type 1 diabetes since i was 2 and i went through a phase in my teen years where i wanted to just be "normal" and refused to accept my situation and i often let my sugars get so high the meter wouldn't be sble to read it which means is was well above 600 where as a normal persons stays between 80 to 120. My A1C was almost 14. I grew out if it around 23 years old and managrd to get it down to 11. my fathers health insurance that i am on hankfully got me a insulin pump and once i started taking care of myself full time i got it down to 9. The damsge sadly was already done however and i ended up having neuropathy symptoms like burning sensations in my feet and stinging needle-poking feelings in random spots on my legs. Im 24 now and my A1C js down to 7.7 last i heard but i still have the nerve damage. My doctors have said they have never seen s person as young as me with neuropathy this bad. I am currently in the process of getting a neural stimulator surgically implanted in my spine that will dull the pain receptors in the effected areas. I had a test version done last week and it worked great. everyone said i was noticeably walking better and i was more energetic and upbest than they've seen me in years. Moral of the story guys take your health seriously and if you have a condition as serious as diabetes, dont mess around with it and understand that the medical technology we have in our modern world works miracles and is only gonna get better as time passes. Dont think you cant live a normal life because you absolutely can. i also just wanted to say sll this to let everyone know that neuropathy is extremely painful and is at times dabilitating. When i was still working at my old restaurant job ad a cook (i work from home now thank god) i was often groaning in agony and would sometimes go into the walk in freezer and bawl my eyes out because the pain was that bad and i knew i still had bills to pay so i felt trapped and also felt bad because i could tell alot of my co workers didnt understand and thought i was faking it to be lazy. I dont blame them. They likely didn't know what nerve damsge was like for people who have it and they sre people struggling to get by same as me and i was essentially a liability. Dont under estimate the pain someone with it is going through because it js a horrible thing to live with.

I have neuropathy too, I don’t think your coworkers would understand what is because it’s an invisible disease so people will always accuse you being lazy, unhealthy habits etc. and non of that is true with type 1. When I got diagnosed I was plying basketball and was in the best shape of my life and it felt like a ghost came out of nowhere and slapped me in the face, by the way my pumps beeping is going to make my head explode again

As someone that has just passed the 50 year mark with type 1 Diabetes, I did not find any useful information either. If you or anyone else here feels pain or burning sensations, I would suggest 250mg of Benfotiamine daily. It is a fat soluable vitamin B1. It took about 8 weeks before I noticed, but the burning sensations in my feet have completely disappeared.

@@steveorozco4843 I would recomend getting your fasting blood sugars lowered. This is the most difficult part of Diabetes to control. I have found that if i snack less in the evenings and take more long acting insulin (I weigh 140lbs and take 20 units of Toujeo) then my blood sugars are close to normal in the mornings. The sensations you are talking about sound to me to be symptoms of low blood sugar (reactions), rather than Neuropathy.

Diabetes can't be eliminated, as the pancreatic beta cells are permanently damaged, it can be only "treated" with insulin. Pre-diabetes can be treated and eliminated. A low carb diet, regular exercise, and medication are the main (and long-term) lifestyle changes that have to be made.

Im sorry man. Im 24 and in the exact same boat. I recommend doing what i did and am still doing. First, get a pain management doctor or consult your primary doctor about getting a neural stimulator put in you. I am in the orocess of it and it has completely got rid of my pain. It essentially dulls those pain receptors. It might be a little pricy but hopefully you can set up a payment plan or have decent insurance. I'll keep you in my thoughts.
