Age is not the definitive factor it’s made out to be when it comes to our health. We can use our age as a baseline for tracking our health and longevity, but it isn’t stagnant. For example, certain types of testing can help us compare our biological age to our calendar age in order to tinker with our wellness routine and achieve the milestones we’re after. With the right steps, we can slow down and even sometimes reverse the aging process.
When it comes to our biological age, or the measure of how well our body is actually functioning for whatever life stage we are in, there are many things that impact it. Diet, lifestyle patterns like exercise and sleep, and stress are all involved in forming our biological age, along with many other factors like blood sugar, inflammation, and genetics. This week on The Doctor’s Farmacy, I’m joined by Dr. David Sinclair to explore the topic of longevity and anti-aging and how he reduced his own internal age by more than 20 years. Dr. Sinclair is a professor in the Department of Genetics and co-director of the Paul F. Glenn Center for the Biology of Aging at Harvard Medical School, where he and his colleagues study longevity, aging, and how to slow its effects.

I’ve been interviewing longevity experts in 2019 on my KZbin channel. I interviewed Liz Parrish in July 2019. Liz has dramatically reversed every biomarker of aging she’s been tested for because she is the first person to have a telomerase gene therapy and was the sixth person to have a myostatin inhibitor gene therapy on September 15, 2015 in Bogota, Columbia. Liz had to do these treatments in Columbia because it's illegal in almost every country sadly. Gene therapy is the best way to extend human healthspan and lifespan. Gene therapy is expensive though. It currently costs about $150,000 for a telomerase gene therapy which is why I haven't done it yet.

So interesting. I came down with chronic fatigue syndrome after being exceptionally well serving in our Air Force. At 38 about time of Incline village outbreak. Fatigue was crushing. So started college trying to cure myself because no one was helping me. First human biology class relearned about mitochondria again. Bingo there was my cure I thought asked my professor how do I increase my ATP he had no answer in 1985. So began my dive into fixing myself. My next question was how can I reverse why happened to me and how do I undo this. Skip to now suppliments have done more for me than any doctor ever did., they say you can take all that and pee it out. Anyhow My energy is fairly well and I am much older but at 70 husband and myself take no drugs eat well And take about 20 suppliments I research by university studies mostly. I did get a paper published in medical journal. Journal of Neuromuscular Pain in 2000. Am a lay person after 2 degrees. Don't want live forever just live well while I am here. Love that mitochondria.! And go ATP! I take most suppliments you both mention they help, husband too. Not young again but for age pretty good. Keep researching

Adam from the bible died at the age of 930 years old and his descendants died at the age of 800 to 700 to 600, etc and Joseph died at the age of 110 years old. So Dr. Sinclair is helping us live longer the way God created us to live. Sugar and the poluted world is making us live shorter.

@James George I do not take the Bible literally but when I read the end of his statement, I thought OK..if that is the path he needs to take to get him to embrace Sinclair's research, I'll live with it. I disagree with the years lived part of his sentence but it's OK, let it be, if he listens to the research, that's a good thing.

Lost me on the plants but still agree with 90% but you can't agree with everything in life this creates balance. I started with Keto now switched to carnivore started very unhealthy at 62 years old 268 lbs now 147 lbs and reversed much of my ailments. It's all experimental. on your own body I'm happy so far.

Would love to hear David's homemade yogurt recipe, as well as the one he buys (51m10s)? I wonder if it's the same recipe that Ken Ford and William Davis use, that contains Lactobacillus reuteri (ATCC PTA 6475 and DSM 17938 strains). You can get those bugs in BioGaia's Gastrus. If you haven't taken ALCAR or R-ALA before, I would first try stand alone supplements, not in combination. ALCAR can cause headaches; it did for me at first. I lowered the dose by only taking part of the capsule and slowly ramped up to 500 mg on an empty stomach. I can't take R-ALA at all since it causes me to have hypoglycemia attacks; I may try again in future, though.

David Sinclair is nothing short of genius and gift to humanity in my opinion (and from my research). Thank you Dr Mark your show is inspiration to many. I wonder what both your thoughts are on what is also an emerging science, the role our thoughts, feelings / emotions and beliefs play in regulating gene expression / health? These 2 theories combined (how to enhance the mechanical body for optimum performance and health and longevity) coupled with how to ‘consciously’ (I use that word very purposefully) pursue health choices, seems to me to be progressional evolutionary path?. Exciting time to be alive. Very grateful for your work.

Very interesting, but a bit concerning to take Metformin. Ellsworth Wareham worked as a CV surgeon til he was 96 and died peacefully at 104. I think I will follow his lifestyle of eating a plant based diet.

Take berberine instead, I'd say it's better. Wonder why he didn't say anything about it, I might ask in email personally :) there's a lot of studies that show berberine as strong as Metformin without side effects or even more positive effects.

I keep reading about fasting, and it's mentioned here as well. I don't see that as sustainable for most people. What is more alarming to me is how much low-quality food and drinks people consume in the US, and lack of cooking skills.

Dr Sinclair is too pharmacologically oriented for my taste. Why not use the precautionary principle and start with the natural mechanisms (diet, exercise) that increase telomere length and reverse aging by first reversing the chronic metabolic diseases that are healthy life span destructive. Net influence on health of drugs like metformin should be clinically evaluated with subjects that have already achieved baseline resilience via natural mechanisms. Otherwise we're just putting toxins on top of toxins...confounders on confounders...what the heck does that tell us? Dr. Sinclair mentions early that teenagers don't get cancer. I'm sure Dr. Sinclair must be aware rates of cancers in the young (developed world) are in an exponential progression, and the number of cancer hospitals for children is also. Let's ask what Dr. Alan Goldhamer (TrueNorth Health) is doing to look so young for his age...pretty sure metformin is not going to be on that list.

Look into the work of researcher Doris Loh, who will be co-authoring a new book on anti aging and longevity, world wide in April of 2020. I'm currently part of one of her amazing anti aging experiments which she will be writing about in her new book. I'm just shy of 60 years young. Merformin is a double edge sword and should not be taken by anyone with a poor REDOX potential.

Although I am obviously not David himself, if that’s whom you were asking, I would like to point out that the drugs you’ve listed are not fully interchangeable and/or equipotent. A big part of the niacin MOA (i.e. mechanism of action) relies on activation of the so-called niacin receptors. On the other hand, statins function as inhibitors of an enzyme that leads to cholesterol production, namely HMG-CoA reductase. Metformin and berberine are somewhat alike, given their shared ability to activate AMPK; however, metformin has been used and studied for over half a century, whereas berberine is relatively understudied (of course, there’s no real incentive for pharmaceutical companies, especially since metformin has such an amazing safety profile and is so cheap). For instance, berberine has been reported to inhibit cytochromes P450, so it could affect metabolism of drugs and lead to their elevated blood levels. My point is, if you want to have the benefits of metformin, I’d personally try to get metformin. Either way, » use the right tool for the job «, and know what you’re doing. And consult your doctor, particularly if you plan to combine various drugs, or e.g. you’re taking a statin and trying to replace it with niacin unsupervised. Take care.

@@szymon52 I dont have cholesterol or blood sugar problems. I do need a safe easy way to increase my iron though! I dont think meat or supplements are the answer. I havent found a great solution yet! If you have the answer I would be grateful!

Sparleberry zinc competes with the absorption of copper but be careful with copper as too much can do harm. Its very easy to get enough through diet. But like I said be careful, especially if you have slow metabolism or any gut issues

Mmm, the problem with what Prof. Sinclair is saying with regard to a physical fitness & dietary programme extending natural lifespan is, that I'm not aware of any1 who maintains what he recommends throughout their lives - ex-professional athletes for instance who maintain a healthy physically fit lifestyle into their later years, living any longer than people who don't, they all physiologically conk out generally in their 3 score years & 10 (or 20). I see none of this class of people who are living until they're 130, with the couch potatoes all checking out at 50. Physical fitness & good diet is fine & it may keep you in good shape thru your life, but I see no evidence that it actually expands that life timewise, which is what he's pursuing with his work.

They're not living longer because they have not changed their diet. As a physician, I know of 55 y.o, thin, vegetarian, marathon runners who are having heart attacks. I believe the key is to continue to exercise, get good sleep, and most importantly, maintain a diet that keeps your insulin levels low. I believe Dr. Sinclair's supplement recommendations enhance things even further.

Dr. Sinclair could benefit from having you manage his health. Four hours on Sunday at the gym? To make up for lost gym time? No side effects of metformin? I’m currently looking at Dr. Que Collins’ opinion on supplements- don’t take them. Admittedly I’m just looking at that... I still do supplements. Alpha lipoid acid... good stuff.

Why are you both being so darn vague? We listen to these podcasts for actionable information. What protocol did Dr. Hyman use to recover from his CFS? What are the “and some other stuff” he spoke about in his wife’s protocol? Other than NMN, NR and resverotrol, what other molecules is Sinclair talking about?

Well generally that is true. The percentage of people over 60 having cancer- I am sure is much higher than the percentage of teenagers. In fact- because teens don't get cancer that much - and because they have their whole life ahead of them- we remember and pity teenagers with cancer and remember those stories better than stories of older people getting cancer. We blow those older people stories off and just chalk it up to it being these older people's time to go.

@@lordjim3109 Dear Jim - yes a teenager can get cancer- and that is devastating for the teenager and his or her family- but this video is talking about generalities for the entire population- so something with a low percentage- i.e. low odds of occurring- in the general population- is what is important to illustrate the videos point.

He's talking in general. Even newborns can get cancer. We don't understand why. Obviously, it's not age related for these individuals, something went wrong: a virus, a mutation, a defect, who knows. But in general, cancer is a byproduct of age when your body fail to repair itself. That's the point he is making.

What makes me question Sinclair’s overall intelligence is the fact that he espouses the importance of higher NAD levels systemically through out the body, yet he does very little outside of taking meds and NMN to accomplish this. He could easily go off the Metformin and high levels of statins by following a well formulated ketogenic diet. I’ve heard him mention that he realizes the importance of keeping carbohydrates low for overall health, but uses the excuse that it’s difficult for him to do because he has children. He seems to just be looking for the free easy ride home. To the point, I have seen very little if any compelling evidence that NMN or Resveratrol do anything at all to extend out the years in any human or primate. Metformin has shown some promise in research. Metformin coupled with a well formulated ketogenic diet is probably at the top of the list of things we can do to extend our years. Sinclair needs to realize that he is already metabolically compromised from his dietary habits (hence the statins). Taking all those compounds is probably still netting him into the negative. You literally can not have your cake and eat it too!

I have to just note how expensive these regimens are! I take many of the mentioned compounds. Probably costs me around £100/month. That's really not affordable for majority of people! :-(

He has hyper cholesterol. There are people who may need statins, just not all of us like big pharmaceutical wants.

Mary Harper , he mentioned a cholesterol number of 200. That doesn’t appear to be hypercholesterol. Even Dr. Hyman made some comments after he mentioned statins. The latest research does not support statins reducing all cause mortality. Lastly, the chains it breaks in the body, along with the permanent muscle damage outweigh any perceived benefit. Inflammation blood markers and regular CAC scans would be better for him.

Why? It’s certainly the proper route for pharmaceuticals meant to be used potentially by tens, or hundreds of millions of people, in the long run... Check the story of thalidomide.

AMP-activated protein kinase I am not saying FDA process is not important although it could be made more efficient and less profit driven. It’s the time it takes to get through FDA approvals and the cost which drives up treatment cost. No one wants another thilodimide tragedy but take for example Navacims. A potential cure for most autoimmune diseases that could literally eradicate the suffering of millions. It was discovered 15 YEARS AGO and just now going through human clinical trials. Just this year, it has been individually licensed to two companies for nearly $1 Billion each license for the development of just a few of the most profitable (diabetes, IBD, and arthritis). Meanwhile many other devastating autoimmune diseases like MS, Lupus, and about a hundred more are waiting for another company to step up and pay the next billion dollar licensing fee to develop their drug on this ground breaking platform. I reiterate that this is not being researched as just another treatment but an outright cure. It will probably be another 5 years before even the first drug hits the market. It shouldn’t take 20 years for a drug to come to market. That’s 20 years of unnecessary suffering and death. FDA process is necessary for platforms like Navacims but why take something that is basically a form of vitamin B3 (niacin) that is only patentable because of how it’s produced through an extensive FDA process?

@@dakotaknight1572 It won't take 20 years you can purchase NMN now and try it yourself. Also if you actually read his book this is a small fraction of what he's working on to combat aging. They're also looking into gene therapy, specifically the gene editing that reverts cells into stem cells, he's been able to dial it back in mice that they don't become tumors. The exciting part is that not only do the cells become healthy again but damaged/severed organs like optical nerves start regenerating.

Fasting puts the body into repair mode as part of a survival mechanism built into all life on earth by the means of several mechanisms to include activating Sirtuin genes. Google calorie restriction and fasting for longevity. NAD precursors assist the same Sirtuin genes to work at capacity and Sirtuin activators (think plant chemicals) increase their function.

Also, just learned the depths of Dr. Sinclair financial interests in age defying products. Conflict of interest anyone? Wow. Any wonder why he's on all the pop-shows BEFORE human trials? We all WANT to believe. And who knows ... Some of his N=1 studies may prove valid. Meanwhile, buyer beware.

@@davidivers9295 He prefers NMN to NR. He says NR can break down into nicotinamide over time. He also says he doesn't have an interest in NMN, but I wonder about that. (Pretty sure both he and Hyman are getting some sort of incentive to wear their Oura Rings everywhere). Meanwhile there's Nuchido's Time+ which uses nicotinamide in combination with other stuff (parsley/apigenin, rutin, etc); they claim it can boost intracellular NAD+ much better.

whats wrong with this picture: dr Sinclair exercises 1x/wk 4hrs on Sunday; takes metformin and statins; does not get enough protein for a person his age, sits a a desk all week, advocates caloric restriction in the elderly...heard of sarcopenia?? and last but certainly not least Sinclair teaches at Harvard...only jokin on last item. I do agree with his views on intermittent fasting and feeling hungry for a good part day which I practice and of course the science of aging is fascinating and should be pursued.

Questioned much of what they discussed - prefer a more holistic, diet based approach than taking chemicals. When he disclosed that he takes a statin as he is "concerned about his cholesterol"; I turned off the video. Cholesterol is both healing and necessary for our bodies - studies have shown that those with the highest cholesterol levels actually live the longest...

What worries me about the obsoletion of age is that it means death will be even more feared than it currently is. For example, no matter how well I take care of myself, I will die of old age; death will come to me. However, if aging is no longer a factor, that means that death is completely avoidable (ignoring the fact that the Sun will explode and that nothing can stop entropy). Won't there be people that become so scared of dying that they hide from the world and its many dangers?
