I have been going through this. Last year I was referred to a physical therapist for my tmj pain. I started out at about 28mm and progressed to about 38mm. Physical therapy only really helped me in the short term. I have a feeling that the jaw stretches they had me doing probably did more harm than good in the long run. A year later and I’m still dealing with limited jaw opening. Actually, just in the last two days I have been going through a terrible flare up with limited jaw opening, jaw pain, and headache. I finally, today, got the pain to subside with massage and a combination of Advil and extra strength Tylenol. I have been dealing with this for years. My dentist has been trying to figure out a treatment plan, but they only know what they know. I’m glad I stumbled on your channel when searching for tips for tmj pain relief.

Thanks for the great videos. I am currently undergoing treatment for locked closed TMJ with a TMJ specialist in Reno, NV. I went during the acute phase, probably about 2 weeks after it first locked. It has been almost 6 months since I started treatment and we have seen progress, but I currently can only open to about 32 mm. I still have quite a bit of pain. I am worried that I am in a chronic locked closed position. I do have a nighttime appliance that I wear, no longer wearing the daytime as it was causing a lot of tooth pain. I have had to have my teeth ground down because my bite is so off. My treatment seems very similar to what you described, and I now understand why when I told my dentist about the popping and clicking, I got no help. Thank you again for these videos.

Great tip about the estradiol and relaxin! I absolutely notice a difference in pain/injury when relaxin levels are higher second half of cycle, after ovulation. A PT also mentioned this in the past but didn’t give the details you did, ty for your videos and for educating dentists. I truly hope they change the curriculum in dental school to at least mention how identify TMD!! 🙏🏻🙏🏻🙏🏻

I have had a jaw locked close (1 finger width) for the last 2 weeks. I went to see my dentist. He did not look at it but immediately referred me to a TMJ specialist. I called 3 different clinics and they all suggest botox. Is this a common solution? I feel like there should be a more natural way to solve the problem. I have tried heat and Advil already. My jaw has been clicking for years and locking once in a while but never for long periods. Thank you for your very interesting and clear videos!

I have the same exact issue with Denise and it's been happening for a few days now. A couple of dentist did suggest botox but I can understand that it won't fix the issue because, as Dr. Priya mentioned, the TMJ was caused by misalignment. So I just wanted to ask what does Botox do and when is it appropriate to use? I was able to pop it back into place usually but it has not been working the past few days. I live in the Bay Area edit: asking this here because it seems relevant to original post and could help others who are curious as well

Thanks for these videos! This describes exactly what's happening to me. My history and symptoms right now point to a chronic case with a forming pseudodisc. I have my jaw opening at the ~35mm range (an oral medicine doctor measured it today) after it was only about 1.5 finger widths months before. I've been trying to get effective treatment for TMJ for about 10 months but still can't find a dentist/doctor who understands what's going on and can help me. All I have is a flat plane splint made 8 months ago (before locking) that I wear at night, and some basic muscle exercises. Is there hope for me yet or is there anything I can do on my own? My jaw is still deviated to the side and I can tell that the entire mandible is slightly lopsided when at rest. I have a new open bite when before all the symptoms started it was quite even. Would my bite stay like this even when I can be pain free and have good range of motion with the pseudodisc?

I have had jaw issues my whole life, but when I got pregnant, my jaw started locking shut! I had no idea why. I would wake up with it locked shut, and it would unlock maybe a half hour afterwards, yet continue to pop and hurt all day. Even after giving birth, I breast fed & still experienced all the same issues. I’m assuming the relaxin had a huge play in this!

Hi Priya, Thank you so much for your videos. They are very informative. I am currenly seeing a TMJ specialist who has told she won't make an orthotic until my disc is unlocked. She referred me to an oral surgeon, who has recommended anthrocentesis. I am also going to PT, which has increased my opening from 1 finger to 2 fingers, over the span of a month. Do you have any thoughts on anthrocentesis? It seems the surgeon flushes the joint with a saline solution, and then a steroid injection, in the hopes of clearing out scar tissue and other debris. I have read varied reports online, and am worried it may make my condition worse. Thanks again for your time!

Acute locked closed from a dental proceedure, perimenopausal but the jaw tracks to the other side? Sore throat/pallet same side, tongue root (same side) pain, inner ear numbness and some hearing numbing. Some relief with neutral position jaw and gentle pressure on opposite side of jaw. I was a Sports and Occupational Massage Therapist, and and currently a Corrective Exercise Certified Personal Trainer with Cancer Exercise Specialty and am going through this myself, at the moment. Seems like the gods, as usual, are driving me in a practical experience direction to better help others. I would really like to dig deeper (and involuntarily) use myself as a guinea pig, to correct my own problem so I can better help my clients. Would be grateful for more information. You seem to be the most knowledgeable first step I've found.

Where do I go to get proper care for a TMJ disorder? I have problems with my jaw locking closed. My dentist didn't know what to do so sent me to an orthodontic surgeon who recommended steroid shots if the pain got too bad or surgey. They both made me feel stupid and the only reason I had TMJ is because I am a woman and the hormones affect the TMJ joint. I don't feel like I got sufficient help and I don't want the pain to get worse. 😔

Thank you so much for all your informative videos! Do you have any experience when someone has this as a result of orthognathic surgery? I had double jaw surgery and my top arch widened in April. I can only open around 18mm and my jaw deviates to the left on opening. I've been going to physical therapy for a month where the therapist is putting in stacks of popsicle sticks to stretch the jaw. I'm also doing this same exercise at home but it does not seem to work very well. Do you ever treat surgery patients? Thanks so much for your time!

My Jaw was locked on 8/8/2021. Every morning I have to massage then my mouth can open 40 - 45mm. I have no problem eating and talking but my Jaw is still locked. No pain though. But I am a mouth breather. Can you please suggest any treatment?

Hello I love your videos. I have learned so much. I wish I lived in the USA close to your clinic! I don't know if you can answer this but I have been booked for surgery after an MRI. Degeneration of the left joint, space narrowing, subcondrial sclerosis, cystic change, flattening of the head and marginal osteophytes, disc poorly delineated, portion bunched anteriorly closed and maybe partial recapture open with diffuse degenerative tearing and mild lateral displacement! Wow what report. But no one suggested I rest it which I inferred from your videos! Surgery may be a year away. My question is is surgery my only option? Will intense rest allow it to heal or is it a fait accompli?

Is there a specific type of physical therapist for the intricate way of doing manipulation for chronic locked jaw? The TMJ dentist I see doesn’t do jaw manipulation, and has said my joint is too arthritically degenerated. I’m a 56 year old woman, don’t really have pain, sleep in my custom orthotics, but still only 2 fingers open and jaw will daily lock more closed until it “cracks” to give me back the two fingers of opening. Could I benefit from manipulation if I can find a qualified pt? Thanks!! Love your advice!!

I think everyone can benefit from jaw manipulation :). Having said that, it’s incredibly difficult to find a practitioner who does it well. Some TMJ physical therapists do it and I don’t know any dentists besides myself that do it. Sorry!

What do you suggest for a 64 y/o male with metastatic lymph node CA (oral) with jaw locked closed? It is chronic at this point (1.5 years). Have had extreme trouble finding anyone who treats this. Have tried stretching (with an apparatus specifically meant for lockjaw), botox, acupuncture, and hot/cold compresses. Treating the CA with holistic methods; have NOT had any chemotherapy or radiation. Any small gain with the very painful stretching is lost within 24 hours. Thank you for your time!

Hi I’m not sure if my other comment went through but I’m not seeing it on the comment section. I believe I have the same problem. I’ll give you a brief summary. So basically for about 3-4 years I’ve had clicking when I opened my mouth but I was able to open it wide and the clicking didn’t really hurt or bother me that much I just got used to it. However I remember every once in a while my jaw would get in a weird position where my back teeth couldn’t touch and it felt like I couldn’t open my mouth as much anymore, but after a few hours it would go away. This happened maybe 3 times a month. Fast forward to 5 months ago. I’ve had that weird jaw position thing happened and when I felt better, my clicking disappeared and slowly I had some bad symptoms such as pain and lots of discomfort and muscle spasms. The symptoms seem to get worse as time goes on and I was wondering if you could please tell me how I can get my jaw disc back in place. Is there any exercises you recommend ? Should I rest it as much as possible? Please help!

Unfortunately the disk being displaced without reduction is the most difficult intracapsular tmj condition to treat. There are no home exercises that can truly help with this. I would recommend looking for an experienced TMJ physical therapist and/or TMJ dentist in your area.

@@amitjeph269 Well, I went to a oral surgeon/tmj specialist, and she gave me some botox, along with some jaw exercises to do and I must say, the pain did go away, and I can open my mouth past 30 milimeters, so I guess im making progress, but I do still feel discomfort like its not in place, but I guess I just got used to it. She also gave me a new mouthpiece to wear at night, custom made one so that might've helped a lot too. Overall, I would say it's getting slightly better overtime.

I have a jaw locked closed, have Ehlers-Danlos syndrome, and am already on Flexeril (max dose) and have been for years. I also take Celebrex, a prescription NSAID, and since this started have taken some aspirin without results. I am disabled and on disability and am so scared the only treatment plan that they'll be able to offer me is going to be so expensive I won't be able to afford it and my jaw will be this way forever. The pain is excruciating. 😢

I suffer from disk displacement without reduction 3 months, and after physical therapy and conservative treatment I was scheduled for an arthroscopica surgery, I wanted to ask you 2 questions: 1 - Doesn't the physical therapy stretch the ligament too much, and as a result there will be no chance to return the disc to its place, because the ligament will be loose? 2 - Do you recommend the arthroscopica surgery? I have suffered from closed locked jaw for 5 years...It is impossible for me to speak normally because my jaw locks closed when I try to speak.Currently seeing a TMJ specialist..I experienced a notable improvement between months 4 and 5 but by month six my jaw locked again( I lost all my gains)I feel really frustrated .In your video you mentioned that people who have had their articular disc out of place longer might take longer to unlock..Is that correct? Yes this is not a good situation. I’d recommend seeing a tmj dentist. You likely won’t be able to get braces if you can’t open your mouth. I wouldn’t recommend braces until you recover your full range of motion.

My dentist wants to give me a night guard and told me to do exercises 😂 my bite is misaligned because I had back teeth missing so they tilted into the gap so my bite isn’t right ! Then I had dental work and my jaw locked 2 weeks after and been locked closed since. Dentist is referring me for massater botox as he can’t do the Impressions for my night guard, so it’s a waiting game, and he tells me my bite is fine but it’s really not and I think it caused me to lock ! I’ve found someone who does jaw manipulation there a osteopath so I hope they can help unlock me 😢

I have been going through this last one week. My left side of jaw got locked closed. I’m able to open to mouth only 2 fingers with. Earlier also i wen😢through this but those was very short period of time but this time it has been from past one week. I went to couple of dentist and they told me it’s because of wisdom tooth. And they told i have to extract wisdom tooth to fix this issue. Am not able to decide what to do..?

Mam I have tmj anterior disc displacement without reduction as mri reported on both joints. I have clicking and poping sounds for several months but two days ago when I woke up in the morning I found that I couldn't open my mouth more wider than two fingers. My left jaw had locked. Although it had happened with me several times before also, but every time it usually opened itself. When I went to a dentist he advised me arthrocentesis which was very painful. But my jaw suddenly got unlocked after the procedure And next day when I woke up it again got locked. Then the doctor advised arthrocentesis one more time and after that it unlocked again. And now I can open my mouth up to 4 fingers but there is a clicking and poping sound in the jaw. And it also clicks when I try to touch my molar teeth together. And I also feel some pain on touching the molar teeth together. But I don't feel any pain on opening the mouth. Can this exercise help me? And my doctor was telling that if arthrocentesis would not help you then we will have to perform a surgery called arthroscopy. But I can't go for surgery because I recently had an ear surgery two months ago. So please mam tell me what are thealternative treatments for whom should I go for? Please 🙏 mam reply.

I’m under the care of a TMJ dentist. I have chronic locked jaw. I am a 56 year old female. I have had my day and night time custom mouth guards for about 10 weeks. The philosophy at my doctor is that when a joint is really unstable and arthritic like mine, they don’t do any manipulation. (I will be having sinoplasty in a few weeks due to deviated septum and bone spur). In your experience with patients my age and chronic lock jaw, do you do manual manipulation, and have any success with getting more opening amount? Thanks!

Yes we have had success with patients at your age with a locked jaw. However, it's highly unpredictable as to how long it takes to reduce the disc.

Hello doc I am from India and my father recently had brain stroke after which he is not able to open his jaw and eat anything he has been on food pipe for last more than 2 weeks, he is admitted to neurolgy dept for last 2 weeks but they are not able to figure out the reason and have done multiple test starting from MRI to Jaw X-ray. My father is able to open his mouth wide open when yowning do you think this is sign of jaw locked? And I am not sure in India we have any such good doc in India who can identify and treat this symptom, please suggest

I am 17 .. before two weeks i had jaw clicking issue, it was not painful .. but 4 days ago my left jaw is locked .. now it just opes 2 fingers.. i tried to extend it but its painful .. do i need a physical therapy? .. can i go to a general dentist ? Please reply. ... Its a request.. i am in trouble

I currently have closed lock jaw 2 weeks now no pain and i can eat i have bad headache's seen my ortho and they recommended botox injections too relax my jaw muscles said after they recommend a night guard and will have too pop it back if the injections dont help. Is that a good route too go

I really cannot recommend a good route over the internet and without examining you. We treat a closed lock with orthotics, myofascial release, and osteopathic manual jaw manipulation.

Hi Priya, will the jaw come unlocked with orthotics alone? Mine locked 6 weeks ago and mu dentist TMJ specialist is getting me on an orthotic but doesn't implement jaw manipulation or myofacial massage. He said as the muscles relax and joints heal the disc will go back into place.

I haven’t seen it happen with orthotics alone. But that doesn’t mean it’s not possible.

What if the locking is now and off? Sometimes I have it locked (less than 2 fingers) for days or for a few hours.

Thanks for these videos. I am a dentist with this problem 6 weeks ago. I am hypermobile. I seek help and TMJ specialist said disc will go back on its own if keep muscles relaxed. I was not happy with that. Will seek another opinion. Just wondering if botox will be helpful to help recapture the disc?

I don’t know what else to do. TMJ specialist here said will go back on its own. If I keep muscles relaxed during the day. Will get another opinion. The strange thing is that I don’t deviate on opening. Initially though it was muscular as from one finger opening went to 3 in a week. But still in pain and feel restriction on the left and muscle tension. Thanks for replying. Will watch your videos. I have always wanted to learn more about this for my patients. Never knew where to start.

I think I have a locked jaw. It feels like a line of muscle or something in my left side of the jaw which after opening feels like it's about to rip. It gets really stiff as well. I have had clicking before this past month but once I open really wide it doesn't hurt. Right now I can't open my mouth that much. I am stuck in this position and can't seem to get it back to normal. Do you have any tips?

Locked jaw is the most difficult condition to treat. You can try the tmj exercises and muscle releases I have in my KZbin playlist called “tmj exercises/muscle releases.”

Hello Doctor. Can I ask you do you consider the lack of overall pain, reduced mouth opening (2 fingers) and mouth shifting slightly to a side when opening the mouth, clenching teeth when focused and pain only when chewing or trying to open the mouth a bit wider TMJ disorder symptoms? I've read that it could also be a stress or muscle strain related problem rather than dental, is that true?

I cannot easily share that over the internet. All of it is very technique sensitive and if done incorrectly can cause harm. It took me a year of hands-on training with my mentor to learn that. So no I cannot share that.

These videos are very helpful! I started with popping and intermittent locking and pain with eating. Eventually just locked but always enough to eat. I was sent to a surgeon, never a tmj specialist. I was given a mouthgaurd from my regular dentist which made things worse, then got a mouth gaurs from the surgeon which was alot more comfortable but i dont know that it really helped. I saw multiple PTs which helped only slightly . I never went to a tmj specialist and i wish i did. I got pregnant and the pain got better but i still couldnt open my mouth and ive been that way for at least a year and a half. The pain is starting to come back but not a whole lot and it feels more muscular, not in the joint. Im hesitant to do anything because im Afraid it will get worse. But i am going to see a few specialists to get opinions on what to do because now its effecting my chewing /swallowing and i have frontal neck pain now. Do u have any advice? Do u ever recommend surgery? If you know of anyone good in Massachusetts let me Know! Thank you

Hi there..sorry to hear you're going through this. A locked jaw is the most difficult to treat and there is typically a 6 month window of opportunity to treat this successfully. Having said that, I have treated patients who have been locked for a few years and it takes skill, time and patience. I would not recommend surgery as there is no guarantee that it will get you out of pain. As for Massachussets, Dr. Konstantin Ronkin and Dr. Evetta Swartzman come recommended - they are both in Canton. I'm not familiar with their exact methods. Good luck!

I actually live in canton and have an appointment with them coming up. Ive also heard dr mehregan is very good in stoughton. Pain really isnt a huge issue. Its more that i have a significant bite change (at least it feels significant). And i am unable to chew certain foods. I also get a lot of muscle tightness in the front of my throat which also makes it hard to eat. And i gulp really loudly with swallowing. Thank you for your advice

Hi Doc, This is so informative! I’ve been experiencing a very settle pop for some time, there is no pain, jaw does not deviate, and my bite doesn’t feel off. I have however experienced locking once when I used those nightguards that can be found at a pharmacy🤦🏻♀️ this was about a year ago and lasted for about 3 days with no pain. Right now, I’m 6 months into having braces and just got them tightened (this is the third time they’ve been tightened) at my last visit. This caused my jaw to lock, I’m currently on day five of having it locked, with no pain. I’ll be visiting a TMJ specialist this week to help get it unlocked. How sever would you say my condition is? Do you believe it is chronic?

@@theTMJdoc last question - so the question was for my mother. She is older and she had a tooth pulling about 2 weeks ago and since then she has been struggling to open her mouth. It's not clicking but it's hard for her to open her mouth. We don't know what to do, we went back to the dentist they said they never seen anything like this and said it's probably lock jaw and gave her muscle relaxers for her to take 3 times a day. What should we do? I wish we were in your state 😔, we would just come to you. I'm looking for nearby doctors who help with this but not sure what the best thing to do for her to regain full range of motion and to be able to open her mouth fully. Can you please suggest something? 🙏🏾
