Yes. Cleaning and irrigation are the keys, according to endodontists I have discussed the process with. The chairman of the endo. department at my dental school once said, " If you clean and irrigate the canals well, you could fill them with horse manure and the procedure would be successful." I am certainly not advocating poor canal filling, but he made a point. Take your practice to the Top Tier. The objective of the RD procedure I suggest is not to keep 100% of the saliva out of the RD, but to keep almost all of it out. You assistant has a suction for that small amount of saliva. Don't make the hole so long so that the RD fits tighter against the enclosed teeth. This RD method 1) keeps the tongue, cheeks and lips out of the operating field, 2) keeps any pieces of amalgam, old crowns being cut off, other debris cut by the burrs out of the patient's mouth and keeps 99% of the handpiece and irrigation water out of the patient's mouth. Fabulous! It is such a simple, effective and quick technique, you will find your self using it for most of your procedures. Take your practice to the Top Tier. Hi Dr Cutbirth I have noticed in all of your endo videos you prep all of the canals to the same size. Is this correct? Almost always 30/40 I believe. Do you not place much importance in choosing a specific canal size prep?

I generally prep canals to size 30-45, unless the canal is sclerotic or very large, like a cuspid or maxillary central incisor in a younger person. I want to clean and irrigate the canal thoroughly, but do not want to over file. Endodontists have told me, and it makes sense, that overfiling can weaken a tooth and make it more prone to fracture, especially if it is an anterior tooth. Our to the small size of anterior teeth, you would prefer not to place a crown on an anterior tooth that has had endodontics because you would be removing tooth structure both on the inside and outside of the tooth, really making the tooth more prone to fracture over time. Remember, the thorough filing and irrigation is the important part of the endodontic procedure, but you do not want to overfile because endodontics makes a tooth more brittle over time, making it more prone to fracture. I had root canals and crowns onmy upper left cuspid and lateral incisor from a baseball accident when I was 17. The cuspid root fractured several years ago and was extracted and an implant placed. Xp shaper file from FKG. They also have Xp finisher file. This file is straight when outside the canal as soon as you insert the file inside the canal, the temperature change makes it wavy. The concept behind this is that root canals are not cylindrical, they are irregular, so the files tries to scrap as many portions of the canal as possible.

Real World Endo. makes the rotary files I use. The "curvy" file can perform all of most of the canal filing with that single file. I normally include at least a #30 or 35 additional rotary file to open the coronal part of the canal a bit more. Take your practice to the Top Tier.
