ABOUT DR. HASSAN MOGHADAM
Dr. Moghadam joined Argyle Associates in 2002 and is the Chief of Dentistry/Oral and Maxillofacial Surgery at the Montfort Hospital. He holds degrees in Dentistry, OMFS, and a Masters degree in Science in Bone Engineering. He published the first-ever stem cell regeneration in human jaw bone using BMP and is internationally recognized for his special interest in dental implants and bone reiteration. He is currently on faculty at McGill, lectures at the University of Ottawa, and is an active staff surgeon at The Ottawa Hospital, The Montfort Hospital, and University of Ottawa’s Faculty of Medicine.

This is such an excellent and informative interview with an oral surgeon. The surgeon explains it with such clarity, so easy to understand and Margaret asked all the right questions. A very dear friend of mine will be undergoing jaw surgery for spontaneous osteonecrosis. She may lose up to 4 teeth and part of her jaw. She was on Prolia for over 2 years. I would stay away from those drugs and find more natural alternatives to build bone.

As someone who suffers from ONJ caused by taking Boniva, I thank you. I was diagnosed in 2014 & that was when no doctor would admit that this condition existed. I was diagnosed after suffering with an abscess in my lower jaw. I am still suffering with the condition and now I am told by a wonderful new dentist that my condition is probably permanent. This has been a long & painful journey and I will NEVER let a doctor prescribe any bisphosphonate for me again. I do have osteoporosis but I do my best to eat healthy and exercise regularly and I am aware of the risks but I have made my decision. I learned the hard way that if I get a doctor that pushes a drug on me that I don’t feel comfortable with then I find a new doctor.

In the US, drugs should be the last option for a doctor in a non-emergency but it’s always the first. 😢

So sorry that you are dealing with this. If you are using health measures, then you may know about Vitamin K2 from supplement or natto a Japanese fermented soybean food, daily magnesium taken at a different time of the day then when you take calcium, D3 and SAFELY do squats, lunges and use of a weighted belt (like a scuba belt) or vest for daily wear. With the weighted belt or vest, I plan to begin with very low weight - 2 pounds for 3-4 weeks and increase by one pound every month.

I was on Prolia for 3.5yrs, my last dose was meant to be done last Sept. My Doc wasn't happy about my decision, she told me I need to have the scan in March and if the numbers are low then she wants me to start again. I'm healthy, not diabetic etc. I requested an x-ray of the jaw last Sept, fortunately there is no sign of problems with the jaw. I don't have any back teeth, which is something I learned from this vid which could be an issue if I restarted the prolia. I will stick to my guns and not any this drug again, sometimes I think the risks of fractures can be overstated, as long as I continue eating healthy, exercise as much as my body can tolerate then I would prefer that than the dreadful repercussions of this drug. My mum has 2 injections and third one will be due very soon. I plan to discontinue it cause i read a lot about the side effects of this prolia . My question is can we stop Prolia suddenly ? Thanks a lot

@kb8205 Hi Karen, no, I haven't used any other drugs. I use Vital Collagen powder, also I'm now (6 months) carnivore. I haven't had another scan, so as far as the density is concerned, well, I'm not even worried. There are more and more research papers showing that prolia is dangerous. Ì thank God that I stood firm and stopped having the injections. I certainly don't feel weak or wobbly from not having the jabs. I hope this helps. Where in Australia??. 😁

I have just found out I have osteopenia and osteoporosis and My dr wants me to take it. I told her I have heard not very good things about it, will do more research about it and will let her know. Now that I have done my research I will not be taking it. I will try diet and exercise

Thank you for providing an opinion we don't always get with oral surgeon, dentist, and endocrinologist being three distinct monoliths that don't sync up in the same room with the patient.

Oh my gosh, this was SO informative! I had no idea of the connection between dental procedures and osteoporosis and the medications. So often we don't regard dentistry as a part of health care but, of course it makes sense as both are impacted by bone health. It highlights the need for all health care providers to treat the "whole" person and not just parts. I was recently been diagnosed with osteopenia bordering osteoporsis and my doctor was wanting me to start Fosamax immediately. It was only in general conversation that I'd mentioned I'd just had oral surgery and bone grafting for an implant. She immediately backtracked, saying I'd have to wait at least a year, until the dental procedure was finished and I'd healed. But that wouldn't have happened had I not volunteered the info first. Thank you for such a great posting!!

This reassured me concerning osteoporosis. I'm on the osteoporosis medicine Risedronate and the warnings indicate that it can cause death of the jawbone. This can be a problem if I ever need serious dental surgery on the bottom teeth. It's suggested to stop taking the medicine for two years before surgery to get it completely out of the body. This doesn't sound feasible to me. I'm glad to learn how strong the jawbone actually is!!

Hi Margaret. Wonderful interview. My doctor wants me on Prolia and said Dentists are just paranoid because ONJ is only if you have cancer. Thanks for putting me straight with Dr. Hassan. You're a treasure.

Am terrified now..have been on prolea for 10 years….no lower teeth on one side..am healthy..none of health conditions mentioned…but now will ask if l should stop prolia..great video..🇨🇦

Thanks so much for this very informative interview. Neither my GP or my rheumatologist recommended I go talk to my dentist before Prolia was prescribed. They should have.

As someone who has been on Prolia for four years I recently had a back, lower molar extraction which seemed to heal quite quickly, I seem to have developed an ulcer, yet to be confirmed as I have just made an appointment with my dentist so this conversation is very much appreciated. I am not happy about being on this medication and am listening to medical advice such as yours to find another way to treat my osteoporosis. I am 76 and otherwise in good health.

Very informative. It prepared me to a situation I am in right now. Taking alendronate for 2 years and prednisone(very low dosage). But I need to have a tooth extracted back lower jaw, which is more prone to necrosis as the surgeon stated. I know now what to ask the oral surgeon. Thanks again.

Thank you for doing this interview. After my very first Dexa, my doctor advised me to take Prolia without having me come in to discuss any side effects or options. I learned all about it on my own. I like my doctor but I feel like I can't trust her anymore with my healthcare. How many patients just accept this without being informed? I'm in the process of revamping my nutrition and exercise habits and will take my chances there.

My dr did the same…no discussion. I told them I wanted to try diet, exercise, & supplements first & that I wanted a referral to PT. Still waiting to see the doctor so I can request the PT; or is it so he can try to convince me to take the meds? I refuse with over $10k in dental work.

I had wonderful oral surgeons at David Grant Medical Center at Travis AFB in Northern California back in the 90s. They aligned my facial bones and did other procedures, and they had to use 6 titanium and 27 screws. I have a cool looking panorex (facial x-ray). In my early 50s I was diagnosed with osteopenia and my doctor wanted to put a medicine for osteoporosis, but thankfully he never did. This video caught my attention because my facial bones are really thin and my jawbone could break at anytime. I’m 60 years old now and my facial bone loss started in my 40s. I’ve had one bone graph in my lower jaw because dentists refer to work on me. I had an implant that literally fell out due to bone loss. The latest oral surgeon said my upper jaw joints are 98% deteriorated, my lower joints are 90% gone. They don’t have a diagnosis for me. I don’t want to bore anyone but I’m relieved it’s not osteonecrosis. Curiosity is why I watched this video.

I am so glad this information has been shown. I was on Prolia for 5 years. My endocrinologist said is was safe for the rest of my life. I have no other medical conditions. I developed ONJ and lost two teeth in my back upper jaw.

Hello Margaret, I find your interviews with experts to be so clear and so helpful. I wonder if you could consider exploring the effects of Prolia when the patient has two conditions, particularly osteoporosis and chronic kidney disease. CKD is a significant risk factor for osteoporosis and according to my nephrologist team, CKD is increasing in the population, so I think it could be a very relevant question for your audience. In my case, my rheumatologist and my nephrologist are in complete disagreement about Prolia, so it’s very hard to make a decision. The particular side effect that concerns the nephrologist is hypocalcemia. Thanks very much for all your efforts.

Thank you. I encourage you to contact an endocrinologist with this question. It is a tad too narrow a topic for me to tackle. I am not a physician, however, I reach out to physicians (and this case a dental surgeon) to explore broader topics that my affect the patients that visit my Physical Therapy clinic (either the virtual or physical clinic).

Very informative and thank you so much for this interview. For years I took Evista for Osteopenia but was not aware even Evista affected jaw bones. Last year had upper front dental implant and surgeon said I had significant bone loss in upper jaw area. Needed bone graft surgery twice at site of new dental implant due to loss of tooth bone.. It was not a pleasant experience at all.

@@alessandravillalobos150 from my research, Evista caused least bone loss of all known anti-osteoporosis medications at time I began to take Evista. There might be some better ones now.

My Endocrinologist downplayed ONJ when I brought it up. Dr Moghadam offered the most helpful in depth information for my decision whether or not to take Reclast therapy. Also, online research of patient reviews of Reclast revealed a rating of 5.2 out of 10. with some real horror stories with other adverse symptoms lasting weeks to years.I So I will NOT take the drug. I will revaluate avenues other than Rx. Thanks to you both.

I’m in current treatment of ONJ stage 2 . I have fragmented bone migrating out of the hole that was originally an ulcer . Not medication related . They think past oral surgery to remove mandible tori 4 years ago.

My oncologist asked me to see a dentist prior to start any cancer remedy treatment . The day I went to see my dentist he did a panoramic exam in which showed no problem. He simple wrote a letter to the doctor stating my gums are healthy. 2 or 3 months passed I was given a Iv to strength my bones. Not short later my tooth became loosened and I needed to replaced my implant, Now I refuse to take extra IV infusion for bones since I don’t have osteoporosis. Funny is i asked doctor for dexa exam and she refused.

I am a RN living with MBC since 2013. I had ONJ due to xgeva 2016 to 2017. It was spontaneous. I had bitewing xrays a few months before the symptoms started. These always cut into my lower jaw. What if padded bitewings could be used on people like me at the most risk for this horrid side affect? I had the hardest time finding someone that knew what was happening. I finally saw or found the doc that knew what happened 2 days after my dead bone fell out. It took nearly 18 months for this to go away. Noone told me about the importance of chlorhexidine mouthwash until I was a year into this. Couldn't the drug companies do more to help with this side affect?

Thank you for sharing. I am an RN too. The information I have received is next to nothing. Nobody warned me about side effects. My gut feeling told me not take this drug. I take other high risk drugs, but I'm staying away from alendronic acid

Thanks! If you do a followup, would you ask about romosozumab which is done only for a year but has ONJ as potential? It is apparently quite powerful for bone denisty, not sure how it compares for ONJ risk to the others (denusomab and bisphosphonates). So additional followup Q: What does he think about that last sentence?

I'm "only" 62, already have very advanced osteoporosis (had already had fractures by the time I got my DEXA, which we had to ask for--osteoporosis not on doctors' radars despite a broken wrist three years prior to diagnosis.) Not too excited about the prospect of drugs for decades. 😢

Dr said he hasn’t seen a lot of patients that are on “Forteo” and he has not seen problems with it why don’t more people with osteoporosis take “Forteo”? I’m not sure if it’s covered by insurance but does it work? Thanks in advance

My dentist recommended removing all my upper teeth and replacing them with a denture prior to me going on Zometa for breast cancer. Given that my oncology team reported the benefit of Zometa was two percent when it came to cancer prevention, I declined the drug rather than endure the risk posed by removing eight otherwise functional teeth. I am using vitamin D, TUMS, and exercise to deal with bone loss risk caused by the AI. Yes, a couple of the at risk teeth had deep pockets which suggested future risk so my dentist did have a reason for the recommendation but I am trying to address that by using a water pik and doubling my scheduled cleanings. There are no current issues with those teeth. I also wanted to avoid the impact on my body of removing so many teeth at once followed by a month of being unable to eat solid food, given that my body has already endured a mastectomy/lymph node removal, dental surgery to remove an infected impacted wisdom tooth prior to chemo, 8 cycles of dose dense chemo, radiation, and endocrine therapy. The nerve damage from the wisdom tooth extraction is still healing. I hope I made the right choice. Terrific, informative video. Thank you.

Try xylitol tooth paste and mouth rinse. It should help keep your gums healthy and prevent bone loss.

I take calcium with d3, k2 and magnesium. I refuse to take second dose of bone infusion since after I did I had a loosened implant. Had to remove it and do it all again of course no insurance paid. You should remove tums. Tums destroy your guts and the good bacteria your body needs to properly works.

I am treating cancer. The medication femara depletes bone mass. My oncologist said she wants to give me infusion to strength my bones. I had taken just one infusion. I lost my implant which became loosened. Now I wait the dentist to end the procedure bc my oncologist still wants to give me the infusion. I now know those details and will refuse to take it again. No one needs to pass thru a treatment in which causes more harm because doctors make money with the pharmas. I am deadly scare ! I don’t want to pass the bone marrow procedure again no matter what.. even when doc say the chances are minimal of a harm to occur 😮

If you can afford Prolia and Fosamax, you can afford to go to a dentist if you have an ulcer in your mouth. This side effect is rare and treatable. People with severe osteoporosis shouldn’t be left untreated because of this!!!!

What about having mini implants. Diagnosed with OP but not being treated for yet? Is it OK to do mini implants with a snap on denture. I have chosen to do mini implants rather than having a Tori removed from the roof of my mouth. Thanks

What about BRACES or orthodontics, is that type of dental treatment contraindicated while on bisphosphonate medication?

I have osteoporosis T score -3.1 in my.lower spine. Im fit and healthy otherwise; i attend the gym twice weekly and work on weights and resistance work for one and a half hour's each session. I'm active daily, gardening, walking my dog, housework etc. I eat a balanced diet and do my best to include calcium rich food's. I take supplements like Bonbalance, magnesium, vitamin D3, Vitamin K2Mk 7, multivitamin for my age group etc. I have been told by my doctor i must go on Prolia injections six monthly, im not convinced this is for me as i prefer a natural approach to my condition. I contacted a nasty virus this year that caused inflammation in the body and brought on Bells Palsy" where i was given to take 10 steroids per day for 10 day's! Would it be riskier after being on Steroids to start on Prolia,or is it safer to wait a while lodger?

I have tori. Two marbles on the bottom of my mouth. My Doctor insisted I get back on fosamax. I'm worried. Hip bone is worse than 10 years ago. My father broke a hip & never got up again. Small frame & and hypothyroidism, but 135 lb 61 yrs old. Don't look it. Sleep elusive. A mess. Help 🙏

I have a sincere question, with all the risks of these drugs, why prescribe them at all? To increase bone density? Why would we be satisfied with increasing bone density? Isn't it the case that bone density is only a marker for bone Strength (which is actually the thing we want) for the person who is not on one of these drugs? Bone density could be increased by infusing someone's bones with lead powder, but for some reason that isn't done. I think medicine is too focused on markers rather than causes. This is the case with the standards of care regarding statins. It is also the case with the standards of care for type 2 diabetes, which don't even focus on the correct markers. As far as I can tell, there is nothing about my skeptical question which runs counter to any of the advice given by the doctor, regarding the treatment of people who are taking these drugs.

Hi Margaret, I only just been given Prolia due to Osteoporosis, second dose is due Feb 2024. Is it safe or not too late for me to back out? Seen your you tube video's regarding the risks of talking Prolia. Thanks for posting such useful information.

If I understand well, there is no risk ordinary osteoporosis patients if three months passed after zolendronic acid infusion.

The risk of ONJ is elevated if you have been on an osteoporosis pharmaceutical for more than three years.
