Dialysis can be of two types. In Peritoneal dialysis, the inside lining of your belly works as a natural filter. This is done more consistently than hemodialysis and it allows for a more flexible diet with kidney disease. In Hemodialysis, blood is taken out of your body into an artificial kidney machine which then uses tubes connected to the machine to bring blood back into your body.
No matter what you do with kidney disease, sometimes, you may end up on dialysis. Robert always focuses on ways to avoid dialysis but in some cases, dialysis may be the only option. In today’s video, he talks about the differences between Peritoneal dialysis and Hemodialysis. Robert did hemodialysis for about 2.5 years when his kidney disease was at its worst.
With all the experiences that Robert had with kidney disease, he tells us why peritoneal dialysis was less common than hemodialysis back in the day. Nowadays, with all the improvements in science and technology, much research shows that peritoneal dialysis is as effective as hemodialysis and it provides a lot of benefits.
With hemodialysis, you have to go to a center that provides hemodialysis. It’s very expensive to do it at home. With hemodialysis, most people go to a dialysis center for around three days a week for about four hours. In order to access your blood to do hemodialysis, you need a fistula surgery on a connection between an artery and a vein which allows for the dialysis needles.
Robert shows us his fistula surgery done over 20 years ago. It doesn’t look pretty but it’s very important. Fistula surgery can be painful but surgeries performed these days are more advanced and you may need more than one surgery. Robert explains why hemodialysis is not the ideal option anymore, especially with all the positive research about peritoneal dialysis. A majority of people on hemodialysis would experience other issues like headaches and fatigue.
Peritoneal dialysis is done by inserting a tube into your peritoneum and you have a machine at home that you run every day to perform dialysis on your own schedule. A majority of people who were on peritoneal dialysis tell us how it’s less painful and more manageable than hemodialysis. With peritoneal dialysis, a better, manageable lifestyle can be attained.
The decision is yours to make at the end of the day. With hemodialysis, you are looking at going to a center three to four times a day and sitting for long hours. Many people on hemodialysis also experience fatigue and headaches. With peritoneal dialysis, it can be done at home and your life is more manageable. A tremendous amount of self-care is required with peritoneal dialysis and you’re more involved in the process. Robert strongly suggests engaging with people who have been under hemodialysis and peritoneal dialysis and make the decision for yourself. Both types of dialysis are as effective as each other.

New Tablo machines for home hemodialysis are much easier to operate and produce better outcomes than NxStage machines. Latest data shows outcomes using home hemodialysis are better than peritoneal dialysis. Prior to COVID, several centers were offering nocturnal hemodialysis. Hopefully it will be more readily available soon. You forgot to mention that patients who are on peritoneal dialysis can suffer from very serious complication of peritonitis which then requires hemodialysis. Also peritoneal membrane stops working adequately after few years.

Can't say. Depends on a lot of factors such as can your mom care for herself, current state of health, etc.

My daughter did PD for 1 week and ended up with fluid overload and she ended up in the hospital for 1 month! So it was a no for her real quick! She said she likes the hemo better for HER because she likes the idea of having a trained staff with her just in case something goes wrong.

I switched from hemo to PD 5 months after I started dialysis after acute kidney failure …hemo dialysis had some horrid effects on me. Flash pulmonary edema. Total respiratory failure , and luckily the hospital was down the street. I quickly recovered after being stabilized , but I was out of there ..😳😳😳. I wouldn’t let them give me a fistula , as they really freak me out still. But , I loved loved. PD. SMOOTHER RUN. NO HANGOVERS, HEADACHES , no damage to circulatory system and your in control of your own health. After couple years on PD my daughter gave me her 25 year old kidney. I can’t say I’d still be here w strictly hemo. My body Just did not handle it well. The supplies really don’t take up that much room either.

Dear Sir, I am in USA. Doctor suggested peritoneal dialysis. As I understand, PD has some advantages like do it every day at home. Doesn’t need to pay frequent visits to hospital. No risk of driving before and after dialysis and loosing work hours or loosing job. Provided there’s no infection around catheter, is PD advantageous over HD. Does PD clean body better than HD. Is risk of heart failure and other organ failure more in PD or HD? Please shed some light! Thanks Sir!

40 minutes? That's not the case for everyone. I know one person who did manual pd for a while. It was a 16 hour day all said. Everyday! Fluid in for 4 hrs, then drain and fill again, 4 times! Luckily she was able to do it mechanically at night after a bit. But it was still all night, as it is for most people. I hope I can put it off for as long as possible!
