Thank you so much Doctor Shim! I just had 2 series of epidurals to help with 3 ruptured discs, with one resting unpleasantly on l4l5 nerve. I am now in PT but, for me, the procedure worked well, and fast. Wishing everyone who is suffering restored health and healing. And wishing everyone well & all the best!

I’ve seen so many videos and you by far explain procedures so throughly within 5mins and I thank you 🙏🏻 I have a herniated disc L4-L5 And S1 this happened unexpected did nothing ...come Jan 1st 2023 back pain and with in days I had severe sciatica so bad couldn’t put one foot in front of other ...I had the epidural injection helped the back but the sciatica has no end it’s coursed numbness tingling right leg and now result to getting Microdestomy surgery in 2 weeks... I am praying 🙏🏻 my sciatica relieves...Again thank you for your videos I also saw the animation video of the surgery I’m having ..

I had these a year ago. 100% relief for months. Wanted to go back again, but insurance now refuses, saying the procedure is 'experimental'. So am back to non stop pain again. Thanks, Anthem.

It just last about 6 month . That’s why my Doctor want to do 2 on me . Did you hear me Conner and window lol myself having bowel movements new day new game going to sleep all night thinking about it and I have a sister Debbie seizure patient in Bluemercury today he’s going to have some of those hanging on your back with the results and you said it is he was dating A Bird and remove really bad paralyze me today I am not going to hell we go to the gym tomorrow PTO it’s just me this is the living room space in my days have it thank you very much explained everything will you show me because you say no to CW geez

For years I have been in pain in my back and abdomen. My Dr always looked at me like I was crazy. Finally went to a new Dr who sent me to pain management. First round and that constant pain is gone after 1 round of injections. I can’t help, but thank her for believing me.

I had the epidural steroid injection done in 2012. I had three injections total, once a week for three weeks. It helped me out a great deal. it took five or six years before I needed surgery

Great video, had the procedure done in 2004 and it brought great relieve for many many years. Now i had it again yesterday at Unitas Hospital a Dr R Swanepoel again after all these years I pray for the same result. I hurt myself in the gym in 2020..

@@lobstermania4025 thank you for asking, there is a relief again, but not as much as the initial injections. I would say 50% less but still there is an improvement. Also it is 18 years further... All the best with your decision and I hope your pain subsides.. i try to use warm/ cold and rub also anti inflamitory creams sometimes when the pain increases.. and they say regular stretching helps.. good luck mate.

@@Andre_Becker Great. I am on the doubtful side of getting injections. I just worry something could go wrong or make it worse. I have a pinched nerve that hurts my left hip. It's been a year and it's almost a way of life now.

I am suffering from a herniated disc that is causing significant sciatica pain. Did the epidural injection work to decrease your pain?

Dean Wood Hi, I'm sorry for the delayed reply. Yes, I believe that the epidural injections helped me. I think that what it did was give me relief so that I could start moving without too much pain. Next, I started getting some wonderful rehab physiotherapy instruction. Not heavy..actually quite gentle and not at all tiring. It's now 4 months and I am feeling much better. Happy to offer more info if required. Good luck.

@@JLitt-us4vs i started to feel less pain and a bit more flexible about 4 or 5 days after the injection. I'd find it difficult to say how long the effect lasted because the injections made it easier to be more active. And that too may have lessened the stiffness and pain.

I had this done a few days ago. It's pretty painless. My doctor explained it is a helpful diagnostic tool to understand what's causing my left-sided pain/numbness. It has already provided some relief, but isn't a cure.

It lasted a few weeks for me. Just had a microdissectomy back surgery 3 weeks ago. The pinched nerve was much than the MRI showed. Hope the injections help for you.

@@brittanyturner5004 did you end up getting the surgery? I just got my first shot on my L5/S1 and can’t feel the sciatica I was feeling now. I know cortisone / steroid shots are temporary relief. What scares me from surgery is the chance of something going wrong is fairly high based on percentages.

Had mine two months ago first injection and I'm actually worse, lost all feeling and functions in my bladder and pain is slightly better but still keeps me from sleeping most of the time

I wish this doctor right here worked in Morehead Kentucky I would feel more comfortable he seems like he knows what he's talking about I have not got the injections yet but I have got appointment to go and get mine just kind of nervous cuz I have never went through this kind of pain with my sidekick pain in the last week it's usually pretty bad but it's never been where I was on the floor crying and I couldn't get up off the floor and nothing helped when I went to the hospital when I finally did get up it's every ounce of energy to get me up and I was in so much pain that I have never been in my life it was washer and my pancreatitis and that put me on the floor too but usually when I have a flare up with my sciatic nerve it's never caused me this much pain but you have a good video keep up the good word thank you very much

I'm having this procedure done tomorrow and I'm extremely nervous. I've got Ankylosing Spondylitis and have had chronic back pain for almost my entire mature life. I've had 4 children and 2 epidurals and I think that's what makes me the most nervous. Getting close to my spine scares me. I had issues with both epidurals I had during labor. I'm hoping this procedure goes as planned. Thank you for explaining the process!

@@FluteStream Oh my gosh, mine is in the same lumbar region L5-1. I have been getting a lot of heart palpitations lately, so I haven't noticed a particular rise in them after the injections. I also have insomnia so I take tizanidine for muscle spasms and sleep. I do notice however a lot more spasms in the area from it. It's kind of painful.

@@FluteStream I wanted to come back and see how the rest of your night went. Did you sleep at all? I had my procedure later in the day, yesterday, so it had only been about 6 hours when I replied. I was sweating A LOT last night. Like whoa a lot lol. I started to notice the racing heart later in the night. I took my muscle relaxer and slept from 12am to 6am without waking up. I've found that steroid medication does that to me. Makes my heart race and I sweat and get these bouts of energy. Even when I've been prescribed orally. Today, my lower back is sore. I feel like I've just had 5 labor epidurals lol but I feel really good and was up early. It's kind of incredible.

@@susieast450 I had trouble sleeping last night and could only sleep from 3 am to 7 am. I did not sweat much but I was troubled by the racing heart and insomnia. Called my provider in the night 11 pm and they told me that my body is reacting to the steroid in a certain manner and nothing much to worry about. Good to know that the injection worked for you so well. In my case, my pain also has been reduced a bit but I think it might take some time for the steroid to fully kick in.

I'm having lumbar injections Monday and i really hope it works because I have been going through pain everyday in my lower back I have fibramyagia and Eds and just tired of pain I can't walk far without pain radiating from the lumbar region of my back down the back of my legs abc's passion in the left side

Thank you Dr, Shim I watched your video and was very well explained. This made me comfortable. Anyway, I had my epidural injection today here in Florida. I am very satisfied. I was told every 2 weeks 3 times total then i will feel much better. I had sciatica. Will this last a life time? Thanks again for video.

I just got my epidural steroid injection its not painful very easy Medication hurts a bit when its going inside you but it eases off within 5-10min

Oh gosh!!! Neither of my doctors told me about the possible rise in blood sugar! Glad I didn't do it!! Tysm!!! Idk if they wait until you agree to try it and then they tell you about the risks.

I am pretty sure my Dr. messed up. I had a S1 L5 epidural injection. About 3/4 of the way through the procedure, the Dr. hit a nerve that sent lightning bolt like pain from my buttock to my ankle. I thought I knew what a level 10 pain was until today. The pain was excruciating and lasted about 20-30 seconds. That doesn't seem long, but when you are in that much pain, it is an eternity. I've had many procedures and accidents that has caused horrific pain, but nothing like that. My best description was an electric shock with tingling throbbing pain. After that my whole leg went numb. Ankle stayed numb for about 30 min. I am curious if anyone else had a similar experience with this sort of pain from this procedure. Again I believe my Dr. made a mistake. My post's goal isn't to talk people into not getting the procedure.

Did not happen on my procedure. I had my first shot today and started to feel pain relief almost immediately with pressure sensation in the location of the injection (L5/S1). The only thing that did happen to me was I got light headed and felt my body getting warm, close to as if I was getting drunk.

Should only be a little pressure and minimal pain if done correctly. I had L3 ,L4 on both sides (4 injections) each of 3 sessions . Cleveland clinic doctor . Each one lasts for about 6-8 months .

That’s what scares me. Glad you didn’t have lasting trauma from it as I watched a Dr Oz video where women were injured for life with excruciating pain for the rest of their lives.

Thank you very much Dr. Shim for the usefull info. I've got a question l'd appriciate if you answer. I just underwent this procedure yesterday Feb 28, 2024 and l am expected to resume work on Monday Mar 04, 2024 after being in a sick leave for 5 weeks. Now my question is: Is it OK if l go back to the work on Monday Mar 04, 2024? To this question yesterday l felt that my physician did not pay much attention he just said if you wont feel pain you can go without elaborating so much. Now please l just need to know is it madically OK if l resume my office work 4 days after this procedure?

For what reasons do patients stop responding to ESIs? I cannot find anything in my scientific literature searches (of course my access is limited, as I’m not a doctor). Last summer I had a debilitating flare up and my MRI showed several bulging discs, spinal stenosis, and disc as well as joint degeneration. So I received an L4/L5 ESI as well as a Facet Injection. Within 48hrs I was nearly pain free, with no sciatica symptoms and only mild to moderate dull aching in my lumbar area (likely arthritic pain). It gave me nearly 5 months of relief. Over the course of 4 years prior to that, I had a few Facet Injections, a couple ESIs in L4/L5 and L5/S1, as well as a bilateral Sacroiliac Injection, a knee injection, and an injection in each heel for plantar fasciitis - all of them gave me significant relief. In fact, the plantar fasciitis has never returned. However, I’m currently having the worst, most debilitating flare up of sciatica symptoms that I’ve ever experienced, which has been ongoing for 8 weeks and is getting progressively worse. In that time, I’ve received two L4/L5 ESIs, and a bilateral Sacroiliac Injection. None of them provided any relief whatsoever. None - not even 10%. I am scheduled for a L5/S1 injection five days from now. I was told that I cannot receive any more after that as there is a limit on the number of ESIs per year. I was referred to a neurosurgeon 2 weeks ago, but still have not received a call. So why would I suddenly stop responding to ESIs? And is it indicative of a more significant problem?

Dr. Shim it would also be helpful to discuss why the FDA hasn’t approved epidural spine injections and why they think it’s dangerous. After all, it sounds like it’s a fairly common procedure and should have plenty of research by now. Thanks.

Thanks for the comment. The FDA regulates drugs and decide if there are risks and side effects that makes the use of the drug dangerous. But there is not enough funding to explore every use. The FDA has not explore its use in this manner. It is a common procedure. At the same time, there are risks and benefits. Steroids can decrease inflammation, and prevent over reaction by the body. At the same time, there are risks such as infection, bone weakening, elevated blood sugars, and mood alteration with the use of steroids. Before having a steroid, orally, by injection, or in the epidural space, the risks should be assessed by the treating physician.

Does the effect of the injection eventually wear off? And if so, the pain return? In other words, is this potentially a permanent fix, or not?

The injection helps your body cope with the inflammation. Over time, most people do get better. The injection sometimes is repeated if the pain returns before the body has had the chance to remove the chemicals from the disc herniation that is causing the pain. The permanent fix is your body removing the disc herniation chemicals and removing parts of the herniation. The injection itself is not the cure.

Would this help with numbness I still have in one side of my calf area and a couple of toes on my left leg? I had tweaked by back about 11 weeks ago and had back pain for a couple of weeks with sciatica for about two days. Pain has gone but the numbness has never gone away

I have degenerative disc disease/herniated disc and Aetna just DENIED my injection. What can I do to get them to approve it. He prescribed oxycodone but I'm afraid to take them

I had pain at the L5 lumbar. I had the epidural and it did absolutely NOTHING. Why was I injected twice? The pain was horrible, the worst pain I have ever experienced. I almost jumped off the table. Was something done wrong? I will never do that again.

@Ellis had mine just last week Monday 12/13.. it sure hurt I screamed for real but it's 6days so far, am I'm most glad I did. I have so much relief in my lower back. A doctor told me its dependent on who did it as it's not meant to be painful.. but you can request for a doctor change if you're not comfortable with who did the last one. Above all, I hope you feel better 🙏

Hello I have a question regarding this injection. Will one injection be sufficient for multiple nerve roots? For example, if I had an L4- L5 injection will it work for both l4 and l5 nerve roots?

My guess would be no. When I’ve needed them they've done each one individually, as in 3 separate injections for L3/L4, L4/L/5, and L5/S1.

Dr Shim, what exactly do you mean by "chemical spill" by disc herniation? Is that if there is an annular tear and the contents of the nucleus of the disc are seeping out, or some other type of chemical reaction? Maybe such as protein irritants? Thanks!

With each discovery, scientists are finding more and more chemicals that are in the disc that can cause irritation and inflammation. Some of the more well named ones include phospholipase A and Tumor necrotic factor. Over time, the body can remove these "chemical spills". That is why a disc herniation can be so painful initially, but as the body removes these chemicals, it becomes less painful.

REducing the inflammation is the goal. If there is continued resolution of complaints, gradual return to normal activity is possible. Please speak with your own doctor for individual advice.

I’ve had so many of these at the L5/S1. Due to narrowing of space in that area, I now have to fully sedated😔I’m actually having one done next week.

Have you tried Physical Therapy. If not, please consider. It's better than taking steroids repeatedly. Long term steroids can have serious complications. Always better to go for rehabilitative treatment like Physical Therapy; preferably try Neurodynamic approach for your nerve pain! Hoping you recovery. Take care ❤️

Me too, lately more than ever, I have two of 7 bad disk touching nerves. They used to work for a long time but not so much now. I'm finely giving in and getting surgery, but not very happy about that...I'v been holding out as long as I can. Good luck to you.

@@shrutipandya2959 I did nine weeks of physical therapy and it seems like it made me worse. It definitely did not help! I have two herniated discs and two bulges and s-1 nerve root displacement. I'm trying a shot next week.

Use x-ray guidance. Some doctors also inject a small dye to confirm placement in the epidural space before injecting the medications.

I have been get these injections for a few years now. Every time I go the pain seems like it’s nothing, just a shot, however, when the medication is released the pressure feeling as the nurses and doctor call it, seems painful to me and although it’s not something that I can’t handle it still hurts pretty bad. Does that sound right to you?

It is SO PAINFUL do not let anybody fool you.. asked to be fully sedation.. i am a grown man.. it hurt me. And gave me a month of headache and not really helped.. so no more for me!!

So I have had my back problem for about 6 years now, I'm only 29. They want me to do these shots but if it hasn't healed yet, these shots just mask the pain while my body fixes it correct? So in theory this is really a band aid for me right?

@@rositabonita7358 I’m supposed to be getting one in the next month or so. Doc told me they can give propofol or something to help relax me for the injection. I’ve done 3 SI Joint injections and they’re not bad but for some reason I get very anxious and tense up. Plus they haven’t been lasting as long so I need to try this now. I hope it works because I can’t take it anymore. Has been well over 2 years with this pain!

@@lm3049 I feel your pain I been suffering with pain a year already and nothing seems to help me hopefully it helps you. they gave me numbing medicine are shots before they did them but didn’t work I felt everything but it might be different for you. I hope everything goes well!🙂

I got MRI done and have L4-L5 disk protrusion... however after going the specialist he recommended a facet injection instead of an epidural injection does that sound about right ? I dont have radiating pain down the legs just severe lower back pain on left side at times

Facet injections are better for neck or back pain. Epidurals are better for pinch nerve pain that causes leg or arm pain.

I had the same exact issue with L4 and L5 from an auto accident 25 yrs ago and a bad fall on wooden steps 2 yrs ago. My MRI results showed them herniated with mild scoliosis. At my 1st evaluation the NP recommended the Facet Injection which didn’t do a single thing. I chose sedation with this procedure. It didn’t make sense to me that they would recommend a nerve blocker for the facet joints when that is not what was on the MRI. Came back for a follow up appt since I called to tell them I had no relief and during this evaluation the NP determines that I try the Epidural Steriod Injection. Like you, I did not have any leg or arm pain but I did have sharp pain in my right foot. I didn’t tell the NP about it bc I thought it was related to my bunion surgery. This time I chose NO sedation bc after the disappointment I had the 1st time, the $1200 that I was billed for just sedation and my out of pocket was approx. $380 after insurance paid their share, I figured I’d give it a try. Let me tell you, I just had my Epi Injection today and OMG, the most painful part was the cortisone shots they gave me just to numb the area. I think the Dr gave me about 3 on each side to throughly numb the area (which I was great full). It burned, hurt like hell, palms got clammy, started feeling cold sweats, and my butt twitched a bit. I tried really hard not to curse. Lol! I think I had the staff laughing a bit too much. I figured if I can tolerate giving birth to 2 kids, I can do this. It was a quick pain but didn’t feel much after the numbing meds kicked in. I know this is a mouthful but I wanted to share my experience in detail so you know what to look forward to in case you haven’;t gotten it done yet or have doubts. I honestly don’t know why they recommended the nerve blocker over the steroid inj. I just rolled with it since they were the professionals. All I can do is wait the 3-4 days for the steroid to take affect and see if I get any relief. If you had it done before I did, please let me know your experience and if you got any relief.

@@sdvsdv8589 Your experience is similar to mine. I have a disc protrusion in my L4/L5. Had an epidural steroid on 12/13/21 and it was fine till the cortisone shot. I let out a scream with no shame cause that sure hurt. So far it's 6 days and I can tell you the relief is amazing. The pain had gone from like a 10 to a 1. So far it's just discomfort I have. I pray I don't have to go for another 🙏

You are very correct. There are two schools of thought regarding the use of anti-inflammatory medications like steroids. Doctors who believe increasing the inflammation thus increasing blood flow can heal soft tissue issues. These doctors use approaches including prolo therapy , which is sugar water injections, or bee venom injections to promote inflammation. It can be initially very painful, but some results have been excellent. In the case of the disc herniation, there are released chemicals in the disc that cause increased inflammation, which will increase the pain from the nerves. Sometimes the inflammation nerve irritation can become unbearable. To lessen that inflammation pain, until the body digests those chemicals ( pacman effect), a steriod injection may be a better option than surgery. There is no right or wrong, as every person is an individual. Just be aware each action has its benefits as well as risks. Thanks for the comment.

It HURTS ALLOT. 9/2023 I HAD EPIDURAL INJECTION IN BACK.. i had to be held down on the table.. it took 30 mins for me to walk.. and it caused great Headache for 30 days. I got little relief. S1 l5 back damage.. I will not do it again.. it is better to see a neurosurgeon to go in your back.. clean it out once a for all! The shots do not work. Temporarily fix.. not worth it!! I promise you!!! 😢

Sorry to hear about your spinal cord injury. Steroid injections may or may not help in your condition. If you have some numbness, but your muscles work, I would concentrate on strengthening your muscles.

The main side effect of the medicine is the cortisone used. It can lead to diabetes, water retention, and softer bones. There are risks associated with putting the needle unintentionally in a vulnerable area. Every person has individual concerns. Your risks need to be assessed by Your Doctor.

Dr Shim....after a year of on and off sciatica flareups, is it too late for an epidural injection? May 2019 - Diagnosis by MRI L5 S1 disc herniation with annular tear. If it does work over time and healing, can you avoid surgery, being careful of course and working on core and back strength. Thanks for your time.

I think you can still go for it and have some relief for at least a few months. That way you can resume physical activity and help the healing process. I had this procedure done today due to a herniation on L5-S1 and till now, I am feeling alright.

The scientific answer is epidurals may not work for spinal stenosis or disc herniations that have already started the process of resolution. On the other hand, recurrent episodes of pain my actually be a new or recurrent disc issue. Epidural injects May still be an option. You could also resolve your pains with some more time and patience. Please see a physician you trust for evaluation.

Unfortunately, any treatment can have side effects. Specific to epidural injections, there are the risks of injection in the wrong place, infection at the injection site, and injection of contaminated medicine. The steroid medication injected can have multiple side effects, the most common being increasing your blood sugar, making your bones weaker, and causing reactions to the steroids. It is always a risk vs benefit situation. Please discuss with your doctor.

If it works, a few weeks to several months. In General, Epidural steroids helps to reduce the inflammation caused by the disc herniation faster.

Wouldn't do a L5 not without nerveblock being worked in peripherally. Assures success on an iffy procedure. Nerve block fits the bill of a miracle drug. Puts regional anesthesia on the map.

Usually not. Sometimes, a Doctor may give you a light sedative, but most people have the procedure awake.
