This video brought tears to my eyes I have been suffering through these symptoms in my right ankle during pregnancy soo much so that I fell when I was 7months pregnant and the pain just got worst now 5mths postpartum and still no resolution. I have never felt anything like this and it is difficult to explain to my husband who rubs my feet and try’s to target this seemingly unreachable tender spot... thank you for this video

Oh. My. Gosh. You have described this condition and my pain so well. My primary care doctor didn't know why my feet and ankles hurt (and kind of gave me the feeling that I was exaggerating and being a baby). The podiatrist diagnosed PTT and sinus tarsi, but couldn't describe it very well and basically told me my only option was surgery. Thank you SO MUCH.

I'm glad you found it useful! See if you can consult a sports physician - they are usually pretty good at sorting this out with an injection - or maybe a different physio who can give you a rehab programme to help with your ankle and foot control. xm

@@TrafficTravelDelight I was in a cast for about 3 months and off my foot for almost a year. I had a real good physical therapist too, for a few months. It's better now but my muscles obviously are very weak, and I have to be careful how I turn my leg. Sometimes my foot still randomly turns and I end up going sideways. But I can walk and I'm grateful for that.

@@ek3197 thank you for your response I just got this diagnosis from my mri. I been dealing with this for about 2 months now and have to be on and off work because of the pain and have to go back on crutches to walk around after about 3 days of being on it when it starts feeling better.

@@TrafficTravelDelight I empathize with you so much. I found that the only way really is to be off your foot until it heals. I was actually off longer because I pushed through it too much. Good luck and I hope you start healing soon.

Finally I got the answer I was looking for, I’m a runner and I had this pain after my runs, it’s nor swollen, it’s not bruised, it’s just annoying, I knew it wasn’t fracture or anything, your explanation makes total sense, thank you!!! 🙏🏻🙏🏻🙏🏻

I went to the Good feet store forgoing the Dr. recommend orthotics guy , after one day I have relief and almost zero pain from this affliction with severe osteoarthritis in the ankle!!! Good luck to all!!!!

@@hareemtanvir2371 I got the injection today. Dr gave me an ankle brace to wear too. I've had a cortisone shot in my shoulder before so I know it’s sore for a couple of days before you feel pain relief. I have three days to recover before going back to work. 🤞

Hello, I’ve battled with ankle injuries for the past 8 years. I am a career athlete. I’ve rolled my ankles many times over the years. Every dr has told me the same thing, which was to stretch or take this pain killer, which did nothing. About a two months ago I sourced out a foot dr and he diagnosed my ankle pain as STS. After watching your video the years of pain started to make sense. Thank you for make this video it was insanely helpful. My question to you is about five years ago An MRI showed I had torn ligaments slight arthritis and bone fragments floating around in both ankle, do you think surgery is the better option. I did my first injection three weeks ago. It helped lower the constant pain, but the everyday 24/7 pain is still there. Any advice you could give would be greatly appreciated. Thank you again.

This was such a well explained video. Thank you. I broke my cuboid bone back in July. Ive been going to PT for 6 weeks. I’ve been in so much pain. Went back to my orthopedic doc a week ago and was given a cortisone shot in the sinus tarsi joint. I was told I have Sinus tarsi syndrome (from the break/foot trauma) and plantar fasciitis from being in the air cast for so long. I’m really hoping the cortisone shot and continued PT will get me back to normalcy soon. I can barely walk comfortably for more than a few minutes at a time. Should I be a avoiding dorsiflexion exercises until my pain is gone?? PT has me doing that on the footboard every time I go.

I really appreciate this video. I’m 29, took 4 years off from soccer ⚽️ and recently returned to gym and soccer only a couple months ago and I’ve been having a lot of pain in my ankles. First training session I sprained my ankle, when I returned after 2 weeks off I was ok but after training my ankles felt so weak. 3 days ago I trained and was mostly fine, 2 days ago I was at the gym training my calves, calf raises all the way up to the toes (multiple variations) also did a bunch of heel raises. Yesterday I trained at soccer but my ankles had sharp pain on every step. Wasn’t even able to get out of breath as I couldn’t break into a proper run as the pain was so bad. Today been trying to figure out what’s wrong, I know I’m getting older but plenty of players older than me. I’m also 6,6” and a slightly heavier build at 110kg but with decently muscular build.

I've had this since I was about 13... it started when I went on a long hiking trip in the mountains and hurt my ankle, then I pulled it trying to force my body into the lotus position in high school, then I was running just after my 19th birthday and overworked it so bad I couldn't stop limping for 1 month, and ever since then I've not been able to fully get over the pain. I'm 33 now, and it's nice to finally learn what I have.

My instability comes from club foot and my Achilles are extremely tight, so I’ve been told to do that calf stretch 18 minutes a day, and now I have exactly the pain you describe. How can I stretch my calves effectively without causing this pain? Thank you!!

I suffered from drop foot/pinched nerve as a rugby injury, and the ankle in the area you pointed out (of the same foot) afterwards had ached frequently ever since. Could it be related?

Thanks for this video. My MD thought it was Gout. I replied you know I had Gout and I know this is not Gout. My MD replied “it’s Gout until proven other wise.” I adjusted my diet got off of Fast Food and after 1 year I lost a solid 30lbs. However this did not benefit my Ankle. After a full year of on again off again pain and whining to my MD he sent me to a Podiatrist after looking at my ankle talking to me about my pain and x-rays he determined I have Sinus Tarsi. I was put on weeks dose of prednisone which raised my blood sugar and helped a bit. The pain eventually went away but is now showing it’s ugly face again.

I have come back to this video. I have very flat feet and previous ankle sprains from years ago. My foot dr gave me orthotics I love them but still have this pain :(

Thank you for giving such a thorough explanation. Can this cause numbness in the toes and a tight feeling on the top of the foot? I sprained my ankle horribly many years ago and have always had a tight, scar tissue type feeling in the sinus tarsi area ever since. Months back I was going around in very flat shoes which I now regret, my toes started to get numb and the outside of my ankle is often swollen and stiff now. Today before I saw this video I decided to walk a lot hoping to fix the numbness but it got worse and my ankle feels extra stiff now on the outer side.

It may be that you have more than one thing going on. It does sound like you may have sinus tarsi syndrome but perhaps also metatarsalgia or morton's neuroma - best to get it checked out and diagnosed.

I’ve had this pain for about 6 months. My MRI shows a lot of edema in the sinus tarsi area. Does icing along with a compression sock help at all. This really acts up after walking outside on cement for a couple of hours

Icing may help in the short term but you really must have your shoes checked and perhaps you can even benefit from orthotics - a podiatrist will be able to assess it for you and advise on what is needed.

Excellent video. I was just diagnosed with this after years of my primary care physician not able to give me an answer. The podiatrist gave me a cortisone injection (not with ultrasound), told me to wear more stable footwear and showed me how to do the achilles tendon stretch you advised not to do. The injection is providing some relief. The footwear (Hokas) has been really helpful but the stretch has caused severe leg cramps at night and daytime muscle pain in the entire leg. What exercises are best?

Hey i have the same thing, search up: “Dr.Goh Sprained Ankle” i know we don’t have a sprain but the stretches he shows are really good and I feel great after doing them for an hour or so. Its forward, backward with the foot. Side to side, circles clockwise, circles counterclockwise. Hope this helps. I messed up my right ankle 2 months ago.

Great video. I think, after watching this. I have STS. Also 99% sure I have posterior tibial tendonopothy. Every time I feel I’m making progress with one, the other flares up. As a postman I walk around 15 miles per day so as much rehab as I do, the ligaments and tendons never get a rest. Had issues for 7+ years. Going to take a month break (light duties as work) and hope it helps.

Look into getting good supportive shoes and well-fitting orthotics - it can help to support your foot and ankle and help it calm down despite having to walk on it.

Its been about two years since a doctor performed an arthroscopy on my right ankle to remove a tiny bone spur which he thought was the only thing that could be causing the impingement. 2 years later I probably have the most muscular lower leg of anybody ever with no improvement in pain. I think you may have saved me. My only question is whether it is likely that he cleaned put the subtalar joint as well since the surgery was on the supratalar joint. (The bone spur was on the tibia).

I don't have any recommendation - the shoes need to be chosen according to foot type. But slippers rarely give enough support. Crocs soft Birkenstocks (plastic ones) may be better option because they have a bit of arch support.

If you did not sprain your ankle (no trauma) then it is very unlikely to be a ligament injury.

Thank you for the explanation that you give! Now I'm able to understand this matter. I've been suffering with this sinus tarsi since 2018 after an accident that happened to me. So i believe mine is structural instability. Please advise to which doctor i should see to get a steroid injection? Is it orthopedic or something else? Your answer would be much appreciate it. 🙏🏼

I stretched my quad and something catched and felt a snap in the ankle I also felt a Achilles pain, next day the ankle swelled up quite a bit.

Have problem for three months ...treatment for post tibial tendonitis. Just got MRI which has indicated sinus tarsus but pain is on inside of the ankle and not on the outside as per diagram?

Yours is most likely due to a Medial facet irritation of the STJ!, still a part of the Sinus Tarsi Complex.

Could this be present with people that have fibromyalgia and rheumatoid arthritis? I have had rolled ankles in my youth which is common for people with ADHD but as an adult I have as recent within the last month started experiencing pain in this area on a daily basis most commonly when I walk

Check if your shoes are actually supporting your arches properly - the most common reason for this to get irritated is if your foot rolls in too much when walking - sometimes the simplest solution is the one that is needed. But any type of arthritis can affect that area, however, it sounds as if it is mostly painful when doing activities that strain that area so I would not think that arthritis is the main reason for this. Fibromyalgia most commonly affects muscles and soft tissue so I would not immediately blame that either.

Thank you for this. I have severe flat foot and have had multiple ankle sprains from soccer and basketball. The most recent was 5 months ago (ankle roll) that never fully healed- i felt pain every time i moved ankle sideways, more so on the outside. Two months of physio and shockwave therapy didn't help. 5 sessions of Acupuncture helped significantly and till this day i have not felt the pain, though can feel the ankle is unstable. Finally got the results of MRI today that revealed it could potentially be sinus tarsi syndrome. Could you please recommend some self therapy exercises that i can try at home to stabilize and strengthen the ankle? like i said the pain is gone but i am worried an abrupt movement would bring me back to square one.

Hi. Did the acupuncture had a lasting effect till date? I am dealing with this pain since 3 years and it’s awful

@@hareemtanvir2371 acupuncture with light electric shock was temporary relief but I ended up receiving a cortisone shot. Luckily it worked and I have been well since (I heard it doesn't work for everyone or only offers temporary relief). I also now use custom orthotics and ankle braces when exercising.

My daughter is busy with athletics. Little bit of uneven grass and cramping in foot while running in race but also overall inward of one ankle caused severe pain on outside. Busy with fisio and all exercises you mentioned and taping. She is struggling with that sharp pain. For now doing exercises. Too calm down do we use heat or cold, and she is only 12. Do we possibly take injection. Really want to avoid invasive. After few days better and starting to run on tartan now. Feel like we maybe did that stretch against wall and not helping. Will definitely remember to focus on glutes. If so focused on injury we forget other muscles I realised last week.. Which could help her because she trained consistently for last 2 years and has goals which she is struggling with now. Now swelling or blue now. Ligament in outer lower ankle got hurt and tendon under foot

Maryka, does running in barefoot tend to make sinus tarsi syndrome worse? Also, is ankle impingement the same as sinus tarsi syndrome? Thanks.

It will only worsen it if your arch collapses when you run without support - so, it really depends on what your foot does when you run barefoot. Ankle impingement is different - it is when something pinches in the main ankle joint which is formed between the top of the talus and the bottom of the Tibia and Fibula. Sinus tarsi is at the underside of the talus

I’ve had one surgery on my sinus tarsi it did help for a while. Is it possible to get another op The pain is chronic .

I would rather opt for injection + good shoes + good orthotics. Surgery is not something that should be repeated too often.

Thank you so much. This exactly describes what I’m dealing with. I finally just had X-rays which confirmed no break or fracture, she said inflamed tendons and sinus tarsi, but she didn’t give me any exercise or guidance how to fix it. Basically get new shoes and wait. This was much more in depth and now I feel like I know how to support the healing.. also the tip about not doing the runners stretch was helpful!!

After watching this video, I know 100% that I have this. My problems is I was have advanced arthritis in my knee, and had my tibia is starting to bow, recently, from the arthritis. Do you think that my ankle problem has anything to do with my tibia bowing? And is it still treatable? I can hardly walk at times, its very painful in my ankle as well as my knee. I take no prescription pain pills, just Tylenol and advil.

Yes, what is going on in your knee and tibia may be the reason for your ankle pain. It may be time to consult an orthopedic surgeon to see if they can help with your knees. Treating the ankle will likely not be that successful if the cause higher up is not addressed.

This video seems to describe what I am going through. What I would like to know if if this Sinus Tarsi Syndrome can show itself as soared half ring on the front of the joint. I kicked a hard ball and next day I couldnt walk, it got better then went backwards. Ultrasound just show liquid but not ligament problem. Wearing a boot but I am not sure if it will get better that way. Any extra clues? amazing video. Thank you!

It sounds more like you injured your general ankle joint - resting it by wearing a boot for a couple of weeks can often be useful for it. Your doctor will decide what the next step is depending on how your foot reacts after taken out of the boot.

General Ankle joint sounds like your are right, not sure what type of injury could be, ligaments and tendons seems alright based on Ultrasound, muscle fatigue and balance loose for sure, lot of liquid within the joint as well. But that is exactly main part of the problem, I feel the doctors (at least mines here in USA) are not actually making contact with the patients, that is why I kept researching. Expended already US$1000 on different doctors that see you for 10-15 minutes and say "by" like Fast Food service without clear diagnose when I just paid $500 on consult and basic study. I can tell your passion and caring on your videos but this totally unusual. As today I am not seeing major changes with the boot, but because of your video I added an arch insole to the boot and feels better. Ice feels doing some good. I wish MRI would be just little bit cheaper. I coach soccer for kids and I can tell how difficult is to face each injury because doctors are just doing easy business without providing solutions. Again, I was very happy to see you answering my comment. God bless you.

Mine has been hurting for 2 months now and it kills! I think its a sprain but could be this... i bent the foot Inwards when i hurt it and must have torn something?!? I will keep doing my exercises for 4 months

They can usually only start stretching it once pain has calmed down and the injury has settles - foam rolling can be used instead.

Thank you soooo much for this! I have been in horrendous pain for nine months and in physical therapy for three weeks and my PT was about to give up on me. Now I know that a much longer time and maybe baby steps, literally, is the way to start. My Advil regime has not worked. Would love to know how long I should try PT before going for a corticosteroid shot?

I'm now 1 yr 9 mos into this horrible condition! 2 MRI's have seen nothing. I've had 30 "events" now - bilaterally. I haven't walked normally this whole time as I can't "push off" with my feet so I hobble around flat-footed. 5 doctors can't seem to figure out why this is happening - never sprained my ankles. 7 mos of PT and now daily exercises haven't helped, nor do orthotics. Pain is HORRENDOUS - literally this thing is trying to break me! I guess arthroscopy is next. I've heard rumors about ligament replacement and bone fusion but this terrifies me! Can anyone provide any hope?

@@TrafficTravelDelight I’m so sorry to hear this! I certainly wish I could provide you with better news, but at this point I am still searching. Most people respond to physical therapy and then most others to steroid shots. I have not responded to either. I am now meeting with orthopedic surgeons. Didn’t like input from the first two surgeons but I have higher hopes on the last one as I have heard he is a miracle worker. Again, I am so sorry I can’t provide more encouraging news from my side, but there is nothing to say that your experience will be like mine. Best wishes to you! 💕

I have had this for months now, it started happening after i ran on concrete with bad shoes while increasing my mileage..i also am flat footed. Stupid mistake 🤦‍♂️ - maybe I should try orthotics, ive been doing exercises but after a bit of intense physical activity it still hurts. Would that be your suggestion? Great video by the way 😊

It does sound as if you can benefit from orthotics, even if it is just as a temporary measure until your feet has enough endurance. But if you have flat feet they may also have to be permanent solution - just be careful not to get ones with too high arches as that can overcorrect flat feet.

I have pain exactly there but It like burns and tenses up to the point where sometimes in the middle of a walk I have to stop and wait to relax my ankle enough to move it again. Is this still what I could have? I’m fat so a doctors visit is only gonna get me a “lose weight” diagnosis when I need exercises that will allow my ankles to work long enough so I *can* exercise >.>

Can this pain come about after rolling the ankle? I rolled my ankle coming up for 2 years ago in a blind pothole, and still get pain in a certain position, I seem to have range of motion with the ankle, I keep moving the ankle doing circles with it back and forth, am still able to run and walking good but notice it going up stairs, I'm wary about rolling it again so keep trying to strengthen it. Thanks for the video.

Yes, that is actually a very common cause for it. Strength training is your best solution so keep at it.

My lateral malleolus is sticking out a bit, I guess it's rotated a bit forward.. What should I do to get it back to where it should be?

It may be best to have it assessed because there can be many causes for this. Also, keep in mind that it is normal to have one leg look slightly different to the other so it may not be related to your injury.

Can this be confused for gout or vice versa? They say beer is bad for gout, but I had quit drinking all together about a year before I had any symptoms and swelling. The pain is exactly where you described. And sometime I have pain in the Achilles area as well as either side of the shin.

6 months most client does not stick to or come for weekly physio for 6 months.

We don't expect clients to come to physio weekly for 6 months. We teach our patients their exercises and then check in with them every 2 or 4 weeks, depending on their preference. If people see progress, then they usually stick to their plans.

Hmm. It seems like this is what I have. Just moving around daily or going for a walk has my right ankle buckling and me almost falling several times in the evening. The pain has me yelling or cussing. 🤦🏽‍♀️ I have these ankle wraps, but they're not helping.

You likely need to look at getting good supportive shoes (like supported running shoes, not too flexible) + orthotics that are right for you (if the arch is too high it will push your foot out too much) + allow it to calm down by adjusting your activities. Once it is a bit calmed you may need to add in exercise. I think the first step would be to see a good podiatrist for advice on shoes + orthotics.

I’m not sure what’s happening with my ankle as my symptoms are more towards the back end of my left outer ankle .. It’s come on over several months and have got progressively worse to the point now where it’s continuously painful, imflamed. I have agrivated it even more by going over on it and today it totally went from under me and is now like a balloon! I also know 100% that I haven’t injured it back when it first started, it’s just come out of nowhere. There is a point on backside of my Fibula/ankle joint that when pressed it absolute agony, like the point where a bone is chipped or something (I have been for x-rays and they came back normal) and literally makes me jump!! (I know ALL ABOUT bone pain I assure you)… This is just getting worse by the week … I have Naproxin and Codeine Phosphate for a severe wrist injury that has had 7 bone crunching/drilling operations on but as you say I need to find out the source of the problem and mix up the therapy … Do you have any advise??

I usually get my patients to start with short holds e.g. 10sec x 3 reps and build up as they get better to 30sec x3. Once they can do that, I give them more challenging balancing exercises.

I have been working with a P.T. for 2 months with some decrease in sinus tarsi pain. But, i cannot do the eyes closed balancing exercises. My proprioception is clearly affected. Do you know any ways for me to work specifically on improving the proprioception in my affected lower extremity?

Start with simple balance first and work your way up - it can take a very long time. So the set I use most often is first master balancing with eyes open and staring at a fixed point - only once you can do that with NO wobble in ankle or body do you move on to doing with while turning head side to side and looking up and down. Again only then move on to closed eyes once that is 100% stable. Start with short holds for each and work up to 30sec. But this may not be the right advice for you and may need further adapting. I had the top of my foot ran over directly in the middle. Had a bit of bruising and pain after it happened but that was it. It's now been 2 years since it has happened and now my big toe is very tender and hurts a lot, especially at night. It seems like it's stuck flexing upwards a little bit and i can't bend it at all. I've been doing a lot of research and I'm thinking it is checkrein deformity possibly. It seems to me that it's the big tendon on top of my foot that hurts, the one that runs into the big toe, because it's stuck bending my big toe upwards. I can't put any weight on it either it hurts like a mofo. The pain is right at the top, where the toenail is but sometimes my calf feels achey and some days my ankle will hurt a little as well, but mainly my big toe. Any ideas what it could be?

Clicking can usually be improved by doing exercises to regain your full angle and foot flexibility and also strength and control. A physio will be able to help you with this, but it can take several months of rehab before things get back to normal. Pain-free clicking is also not necessarily something to worry about. Something that can help to stop the scoliosis progressing and optimize your alignment is doing strength training and exercise that help you focus on alignment (like pilates but regular strength training can also work really well) However, don't try and force your spine into normal position - that won't work because the shape has permanently changed. Instead the focus is on strengthening the muscles so they can support you properly in that position. As for your foot pain, you will likely benefit from seeing a physio in person for advice on that because they will have to look at your overall posture and movement to come up with solutions to help your foot.
