Dr Tobias. In this exercise instead of using the splint that you are using in this video I am using my shoulder splint to do this exercises. I can’t keep my involved hand facing up so what I did I put a weight around 2bls in my hand to keep it upward. How ever my hand wrist fold inward.

@rehabhqofficial5 жыл бұрын
Hi Michael. Thanks for watching. This is a great question! I get this a lot so you are not alone. I would say, keep working on regaining arm more movement. Try to make things easy enough but not to easy. there is a fine balance. Sometimes if the activity is to difficult you will compensate with stronger muscles and this can make a movement feel more difficult (or make the arm feel heavy :) ). If you are noticing a lot of swelling try an edema glove (swelling can also make it feel heavy) Here is a product link for a glove :) amzn.to/2YdrkwQ. Keep working hard. Worst thing you can do is stop working hard every day. :)

Do suggest wearing elbow immobilizer while sleeping?
@rehabhqofficial3 жыл бұрын
That is an excellent question! Generally speaking, if the arm is staying bent all night, it would be beneficial to find a mode to keep the arm straight. That being said, there is a risk of developing a pressure sore on the arm depending on the severity of the involuntary arm bending (spasticity in the bicep). Also generally speaking (this is not medical advice), If someone were to try and keep something applied to the arm to keep it in a specific position, I typically teach people to put it on for a couple of hours during the day (first), and do frequent skin checks (about every hr) for a few weeks. If any signs of redness after an hr of wearing it, that might indicate that the skin will not tolerate this all night and there is a high risk of developing a pressure ulcer. I hope that helps!

Hello Dr. Tara, is it safe to use elbow mobilizer for patients with an ashworth scale of around 3?
@rehabhqofficial Жыл бұрын
Thanks foe the question! It depends. First important to determine what is true spasticity and what is contracture. If you are the treating clinician, use your clinical judgement.

If you have concerns about “triggering” the spasm, I recommend using an air splint and monitor the patient while it is on

@rehabhqofficial5 жыл бұрын
don't go above shoulder height if it is causing pain. Many things can cause this and the pain means that you could be damaging some important structures in the shoulder. Try and get evaluated by a physical therapist to fix the problem that is causing the pain before forcing the arm about your shoulder.

I am unable to lift the arm only a small amount. If I stretch out my elbow and hold I can move with no pain if elbow is bent upper part of the arm is extremely painful. I can’t get any relief. What do you recommend.
@rehabhqofficial3 жыл бұрын
Thank you for watching Jennifer! Generally speaking, people who have moderate to severe spasticity in the arm/shoulder will have more involuntary movement in the muscles that bend the elbow when attempting to lift the arm. That is because the spastic muscles in this area cross the elbow and the shoulder. Generally speaking it is helpful to use an elbow immobilizer or some sort of air splint (I have lots of videos using this) to keep the elbow straight when working the shoulder. I hope that helps!
