When I say extremely , it's like one in thousands. because you need a strict regime to get to that point . in medieval times , not many people could eat to satiate their hunger , even less get enough protein to build consequent muscle mass. vikings for example , although depicted as tall and strong , were actually pretty small , and fit, because of malnutrition.
