Thank you for your integrity. Four years ago I was scheduled for a facelift and brow lift. The night before surgery, which was the next morning, my plastic surgeon called me and he said in good conscience he cannot do the lower facelift. He said I didn’t need it at this point even though I truly believed I did. But I trusted him because I could tell it was heartfelt. He said he sent pictures of me to some of his colleagues and they agreed with him. So I just went in and had a brow lift. He told me he would freeze the price for the other work for when I do need it which he said would probably be when I’m in my mid 60s. I’ll never forget this guy and I will take him up on his offer when it’s time. The last thing I want to do is waste money and that’s exactly what he said… You’re a good man!

thank you for your concern for your patients and us on here. I’m ok with aging and looking older. I was always worried about future. Well I’m here and I have a turkey neck, tiny jowls, bit of wrinkling around outside of eyes, but I’m accepting who I am. I’m not interested in changing to look good for others. I’m looking good for me and living my best life without a young face. It’s ok with me. I’m not 20, 40, so for those of you worried about the facials changes, do whatever you choose. It’s not who I am. I don’t wear make up. We’re beautiful bc our hearts are in the right place

I'm genuinely curious why you follow a plastic surgery channel? I'm not OK with how aging has changed my face, but if I was, I don't think I'd be near content like this at all, so it perplexes me to see such comments. I don't mean to offend, I am very curious though.

I'm glad you're happy with aging and looking older, but I agree with other comments on here that it's a bit odd that you are listening to a video about plastic surgery made by a plastic surgeon and taking the time to comment given that you are happy with looking older. It seems unnecessarily antagonistic towards Dr Karam and his viewers. You have instructed us "do whatever you choose"....... well, actually we are not here looking for your permission or approval.

Channels like this are full of holier-than-thou viewers who like to preach about accepting aging, but are secretely intrigued about the possibility of eradicating it. Motives for secretly wishing while preaching to the contrary are a combination of lack of money (most frequent), fear of medical procedures, and a desire to hold a virtous image. At this point, I find such comments hilarious. If you could offer these folks a free, no risk pill that could erase their jowls and turkey neck, you can be 100% they would take it.

Had a horrible experience two years ago with a doctor that did threads. As soon as she walked in, she gave me a long list of everything that was wrong with my face and what she could do to fix it. Probably 10k in lasers and injectables and threads. I walked out in pain with a bill (overcharged but a quarter of what she wanted to sell me) that could have been saved for later. Left an honest Google review and she wanted me to come back in to "make it right". I refused. I never even had a followup because I waited in the room for almost 2 hours, and eventually walked out. I am upset that I was suckered into thinking some of these procedures would fix my insecurities, and found someone that just fueled them. I won't even consider an office or doctor that offers some of these procedures because it's all just a waste of money, time, and sometime our self worth. Thank you for your channel. I've also been researching that bioidentical hormone treatment can also sometimes effect our appearance, and of course diet!

Had a similar experience with a very popular derm - 2000 euros spent for nothing, she couldn't even properly remove some moles... And for reference in my country you can get a facelift for 5-6000 euros, so it was a lot of money for no result. The appointments were also always delayed, and she was treating several patients at the same time, running from one to the others, so she never had any time to discuss or answer questions. Terrible experience...

Does anybody really "NEED" plastic surgery? Of course not. But that doesn't mean we shouldn't do it. We pursue all kinds of things that aren't necessities. After the struggle for survival is won, isn't it normal to then wonder, "what else do I want from this life?" As long as you aren't taking money from the neccessary things that you and your loved ones really do need, why not spend it on something that makes you feel good about your appearance? People spend more on fancy cars, bigger houses, and fantastic vacations. I made the decision that those things aren't as important to me as feeling good about my appearance. It's a personal decision. I have friends who go to France every summer, friends who drive a vintage sports car, and even friends who spent a huge sum on their professional gourmet kitchen. Everbody has priorities. People who try to belittle us or shame us for having plastic surgery need to take a look at their own glass house first. PS, I had a facelift in my 50s and never regretted it. I'm 72 now and I still look much better than I would have looked had I not had it done. 😊

Actually I DO NEED plastic surgery, on my upper eyelids. (Blepharoplasty.) My skin on my upper lids and skin on my temples has dropped so much that it's impairing my vision. When it gets REALLY bad, where skin has drooped to the center of the pupil, Medicare will pay for it. I'm not waiting til mine droops that much. I'll pay out of pocket for the surgery. Also if you qualify for Medicare to pay, the surgeon doesn't make his standard cost. Medicare pays the surgeon much less, & there's nothing he/she can do about it

Awesome story about your mom’s journey 🤍 One of my dads favorite sayings is that cardio is the poor man’s surgery and I totally agree. I’ll eventually get a facelift but until then I’m all about mineral sunscreen, cardio and excellent diet :) and sleep. Restorative sleep like it’s my religion, slightly elevated. Plus daily gentle lymph massage. I’ll be 50 in a few years but so far those things are helping quite a bit to avoid the dreaded droop lol sunscreen for over 25 years, since I was 16. The staff at my dermatologist think it’s pretty cool, one of them. The other place they were mean on purpose lol so I left there. No sense being around people who are regretting their choices and want to tear others down.

They require a lot if creativity and problem solving . It keeps my work endlessly interesting to me. Like you, one of the most important things to me is that i live up to the trust my clients put in me. I do my very best for each of them. I have actually turned down clients who insisted on ignoring sound professional advice in pursuit of something i knew would not yield the result they wanted. Knowing they would be unhappy with the result would make both of us unhappy. My philosophy in my practice is very much like yours, even though we work on different platforms. Thanks again for taking the time for this honest informative and heartfelt video. I think its important for people who are interested in face rejuvination to be informed that not just any procedure will correct and give them the results like a face and necklift /with fat transfer can do. Throwing money, your time and possibly having bad outcomes into procedures that wont work is important for people to know who have expectations of looking 20+ years younger is duly noted. Also it was very nice hearing about what brought you to your chosen career and learning more about your family. My take away from this discussion....is only a board certified cosmetic surgeon can do the heavy lifting procedures to look natural and years younger. Sitting in an aestheticians chair and getting injections of botox and fillers won't, plus the real possibility of risking unatural outcomes.

Where there is no regulation there is no informed consent and corruption takes over. Big pharma, corporate capture and insurance companies are ruining medicine . Docs have lost their ability to practice the art of medicine and take proper care of their patients because of this..sad. My father was a country Doc and loved his patients and they loved them . Dr. Karim, as someone who is not in your field, maybe what you're saying is correct. The only thing I have a problem with is when you say, "aging sucks". I understand to a point what you mean, although I think it sends a negative message to aging women who feel blessed to be here. I am 67 and my mother only got to live to be 67. Let me say, "cancer sucks!" I feel blessed every day that God gives to me. I would have given anything to have my mother here wrinkles and all. Losing my mother when I was 32 was a big deal. She missed out on all the wonderful moments I have got to experience with my children which are now in their 40's. I think you are genuinely a very good man. I just wish this world was more focused about their health and spirituality. Blessings to you and your beautiful wife.

Aging means two distinct things: decay and living longer. The deterioration of our bodies and faces does suck. It is the price we pay for living longer. Let's not be in denial that decay sucks. As for living til 67, my mom lived til 52, and I'm grateful she got to have a long marriage, three children she saw to adulthood, two grandchildren, traveling, meeting people, studying literature-a full life. Anything over 50 is a blessing, but people forget that we start deteriorating in our 50s. There's this sense of entitlement now. I want to live longer than my mom or yours, but losing my health, my vitality, AND my face is difficult to accept. Why should my face not matter? Our faces are our identity. They don't deserve to be discarded in our desire for a good aging life.

Right now I am doing microneedling and it's my fav treatment by far. I've noticed a huge improvement in my skin. Thank you for sharing your knowledge with us. I appreciate your authenticity and honesty!

Hello Dr Karam - can you do a video on what kind of professional treatments other than surgery do work to improve skin (texture, pigmentation, thinning etc)? For example, there are so many celebs out there who haven't yet gone down the surgical route, but have done other things such as lasers etc that have greatly improved the look of their skin. Do rich folks have access to other treatments/procedures that regular people don't know about/have access to? Would really love it if you could talk about this in a future video. I really appreciate your down to earth and honest nature.

I’m a laser tech & esthetician & I cannot push procedures if it doesn’t align with a clients true end goal. I will tell them to get a facelift. I’ll give them the truth with how lasers can fit in but they are not the magical tool that will erase. I can help support slowing down aging but also let them know what they do at home is very important with skincare & sunscreen. It’s a blessing to age

Thank you for demonstrating your commitment to your profession and care for each individual you encounter who wishes to be embark on this life changing journey and really come to understand what options are truly effective. I am from Canada and wonder how to consult with you - is it basically coming to your office in San Francisco? Would love to know what steps I require to possibly make in order to embark on this “look as long as you feel journey”Thanks! ❤

@DrAmirKaram I totally agree with you. 💯👍. I also think in some circumstances when a patient goes to certain "shady/quack/greedy doctors, that dont have their patients best interest at heart are not only taking money from them for something that they cant promise, to possibly putting their well being and life endanger, or have serious complications and very unpleasant outcome. On a lighter note, you were/are one hot looking doctor! You wouldn't have to perform heart surgery, you could get your patients heart going pitter patter as soon as you walked into the exam room! 😍🤩😁. Keep the videos coming, I just found you online a couple months ago. I went back and watched all of your videos. I am all caught up! I have enjoyed watching all of them and have found them interesting and very informative.😀

I haven't had medspas trying to oversell to me thankfully but I do often leave feeling bad about myself. There is so much focus there on what's not looking good and what needs changing that I can forget that I am still attractive at 46. My last visit all I heard the nurse say over and over again is how crepey my undereye area is. I hadn't actually noticed it and now it's all I see. So it can be great to learn what to work on with treatments but it's also at times made me more self-critical to things I didn't even know were 'an issue.'

This video content is why I trust you Dr. Karam. You care about us and give us videos that are educational and informative. You are honest and give us all the information we need to make informed decisions about what is best for us. This I appreciate so very much and would not want to miss skin school any time because I learn so much about things other than facelifts but learning about facelifts are very important to me and I feel I can make an informed decision now. Thank you so much Dr. Karam, you are worth your weight in gold but most of all you are a miracle worker and artist in what you do.

Obviously, you are an awesome surgeon from the posts that you have created. I wish that I could see more videos of post-surgery patients. It bothers me when people don't have frown lines or have high eyebrows from Botox, I wish, maybe unrealistically that surgery would be one and done, at least for a little while and hopefully longer. I had reconstructive surgery after an accident and my surgery literally saved my life, or more specifically, allowed me to live as a nice-looking person, and to continue to do whatever I wished in life. Aging doesn't suck. I also think that skin care products can make a real difference, as long as they are not too oily and moisturize well. Thank you for this video, I found it to be very enlightening.

I spent a lot of money on one ellacor session and one placement of structural volumizing filler*, hoping to avoid the facelift. Still went for the mini lift in the end. I think the earlier two procedures helped the facelift, but the cost was disproportionate to the impact. That being said, I stay away from energy devices, especially radiofrequency, ever since one device caused sag. *Not traditional placement, but at the bone level. One thing Dr Karam never touches on is bone loss, which you can't replace with fat. Other than that, i think he is right in what he says.

Thank you for all your advice! Can you tell me your thought’s on fibromyalgia and some neuropathy and how it can affect surgery? Extremely anxious about my nerve function, and possible circulation issues (and long term possible numbness) any advice on this? Do you see any of these medical conditions in your practice? I’m so nervous about having my face go under the knife, but don’t want to wait and regret never doing it if the surgery could be a big success. 🎉 I’m 51…. Thank you
