Watch the clip at 4:57. Gabby doesn't fire once, or twice. She fires _three times_ and doesn't _once_ notice that it's her own gun that's killing her teammates. And she's not holding M2 like you would with ADS, she's hitting it three times consecutively without realizing it's her own weapon.

Fun fact: the Spas-12 is a single barrel shotgun. When you fire the right click it is firing a shell out of the magazine tube as well as the barrel. This is not a thing. If your shotgun discharges from the magazine tube, you have no shotgun. And possibly no support hand.
