My assumption is that had you treated me as a cardiologist, I would still be able to walk. In addition, I believe that my blood pressure would still be around 130/80, rather than swinging wildly as it does now, from 220/140 to 85/40. It wouldn't surprise me if with this information you could take a good guess at how my pulse rate has wound up, even though in my 70's, with intense exercise I could comfortably do intervals of running my heart at 140 or more and go an easy hour at 125-130 "rpm".

After the first industrial revolution, people began to consume refined flours. Perhaps, one of the first junk food ever was the refined wheat flour. Before this era, the ratio of people actually dying from heart diseases was 1 to 22, though this does include birth defects like occlusive valves. After the second industrial revolution ( when a German after practicing autopsies realized that a huge percentage of those people who had died from heart related issues observing a major cholesterol accumulation on the heart artery walls) the cholesterol was targeted to help solve the problem of heart diseases. Today, in 2021, the ratio is almost 1 to 4 and we haven't got (during all our mankind history ) such a huge statin drug prescription to fight high cholesterol levels since the 80s. It doesn't look like a huge mistake of mother nature as in providing human beings with a weak heart, considering that this muscle has been given the responsability to fill from head to feet and viceversa all our body with blood beating approximately 42 million times in just 1 year. How do you explain that there are populations like those ones living in the Amazon where the diet is based on fruits, vegetables and fish have basically no heart disease issues?.... Diet, way of living, oxidative stress and living away from the truth is our killer number 1 I had a heart attack when i was 38. i was taking statins nonstop for 7 years. I stopped beause i could barely move freely. I am 60 yo now and stil having a good diet, exercising, taking some preventive medicene combine aspirins with natural antinflamatories, meditating, praying, bring a good husband, son, brother, neighbor, friend, dad, coworker and citizen. @CardioGauge5 жыл бұрын
Hello. It sounds like postural hypotension. Because the symptoms are brief it does not sound very pathological. Like you say, when you stand up gravity empties the right side of the heart, there is a sudden decrease in how much the right ventricle pumps into lungs and resultant decrease in how much blood is returned to the left side of the heart, and decrease in the volume of blood pumped with each heartbeat into the aorta. Bottom line is when you stand up, your heart suddenly pumps less blood. This situation is at its worst when a person is lying down asleep and they get up to go to the bathroom or whatever suddenly. The situation is alleviated if the person has a higher heart rate to start with and is being moderately active. The body has a mechanism to combat this drop in pressure. The carotid body in the neck senses the decreased distention of the carotid artery when the empty heart is pumping less blood. The carotid body then signals the heart to beat faster, and with increased force. Some people can feel this as a pounding sensation in the chest and head. As the volume of blood pumped increases due to increased heart rate and squeezing power (contractility), the carotid body senses this and decreases these two parameters. I have seen metoprolol both help or hurt the situation. Some patients become lightheaded with metoprolol, because it blunts the increased heart rate and worsens the drop in blood pressure with standing up. Other patients feel better with it because it decreases the palpitation sensation. Your symptoms sound more like a palpitation than a dizziness / lightheadeness, so metoprolol would be expected to help you. You are on a very low dose. 400 mg per day would be the absolute max dose, to give you some perspective. Another thing I find helpful is for the person to fidget with legs before standing, like flex and extend the ankles, pick the legs up and down a few times, to increase the heart rate a little before standing. This sometimes helps prevent symptoms. Your symptoms are unlikely to be all due to anxiety, because many people have palpitations with standing up. Your head could feel weird too. Anxiety could make the symptoms worse though, in that if the symptoms freak you out, your heart will beat harder and faster. I doubt it is a hypertensive response. Hope this helps. Best wishes!

So i sit up in bed first and let the fluid shift, then u get up Everything you said makes sense.. and funny enough sometimes i do kick my legs or wiggle my legs to get the blood moving before i get up after sitting for extended periods of time because it lessens the symptoms. Glad to know its not harmful. I was worried that pounding pressure sensation would result in my great vessels damaged or a stroke.. its an upsetting sensation to feel the back of the head pound. My brother is an athlete who suffered a cardiac event that his cardiologist suggested was from working out while he had a bronchial infection. He believed that his system was already inflamed from the illness so a vigorous workout caused a piece of plaque to break off and get lodged in an artery. The result was a stent. Is working out while you're not feeling well also a bad idea? We have no family history and my brother is in phenomenal shape.
@CardioGauge4 жыл бұрын
Hi Corkfish1, there is no way of knowing whether or not the combination of the workout and the infection caused the plaque rupture. It’s a possibility, but it is more likely something that could just as well have happened without the bronchial infection. He very likely had some underlying coronary artery disease, for this to happen. So basically it is a pretty common story, person has heart event without prior history of any heart difficulty. Now they will look at the risk factors, family history, cholesterol profile, blood pressure, and try to address these. If anyone in else in the family has had heart issues, that increases the chance that other family members could have the problem. Coronary Calcium score often helpful in that setting.

Hi Dr. How long do you have to take baby Aspirin and brilinta for after you have a stent put in? One year or more? Thank you.
@CardioGauge5 жыл бұрын
the usual duration of brilinta (or similar medication like clopidogrel) is one year. It is variable though and different docs make different recommendations, some recommend to continue indefinitely. Aspirin is usually continued without stoppage for lifetime, unless there is significant bleeding. Doctors use a decision tool to estimate the risk of heart attack vs the risk of bleeding tools.acc.org/daptriskapp/#!/content/calculator/ They put in the various characteristics of the patient and come up with the risk and benefit of continuing the Brilinta. Bottom line is the most common duration of brilinta is one year, and aspirin is indefinite. hope that helps.

Hi James, thanks for your question. I don't know a lot about chelation therapy. It is not available where I practice (as far as I know). I do know that it surprised a lot of people in the TACT trial, when it worked quite well for people with coronary artery disease and diabetes. TACT2 trial is underway to validate the findings in TACT. You can check out this info from the NIH, which, to its credit, is respecting chelation as a possible treatment for vascular disease. If I had coronary disease, i would take statin, EPA fish oil, and eat a mostly vegan diet with whole foods and occasional fatty fish. I would personally skip the chelation therapy right now. Need more data. For someone who had worsening coronary artery disease or other vascular disease in spite of doing the stuff I just mentioned, I think chelation would be something reasonable to attempt. Best wishes!

High salt intake can increase blood pressure a lot in some people, and cause people to swell up. I have seen it many many times. Most people with high BP can cut out the transfats and get the blood glucose to 75 with the healthy diet, and the blood pressure will be normal. @CardioGauge4 жыл бұрын
Hi David, No it was not dumb, it was a textbook (it is in the guideline that the clopidogrel can be stopped after one year) move that many cardiologists make. I have done the same with some of my patients. I generally favor 3 years of clopidogrel if I am choosing a duration, but I have patients who strongly want to come off any medications possible and since the textbook says it's OK after one year, I will let them come off the clopidogrel at that point. I have had patients in your shoes who have had clots after coming off clopidogrel, and then, like you say, the clopidogrel and aspirin are continued indefinitely.

I gave this video a thumbs down for one reason. All the points he makes are valid, but he doesn't mention potassium in regards to blood pressure. Virtually everyone agrees that 4700 mg per day is what the body requires, but virtually no one advocates for it. 10 ozs. of leafy greens and cruciferous vegetables, or else take it in powder or pill form. It lowers blood pressure, decreases palpitations and arrythmias, decreases depression and anxiety. My blood pressure went from 130s over 80s (doctor said "don't let it get higher, but you don't need medication") to 110/70 or less. The skips in my heartbeat don't happen anymore...and I prefer Potassium over ecitalopram, which ruins my love life. What it comes down to: the vast majority of physicians go after symptoms with drugs, rather than trying to find the underlying disease causing the symptoms, like a simple potassium deficiency, as one of many, many examples. The body needs it, so why aren't we advocating for its increase?
