You are right with "western diet", but I would be more specific: 1) fat is not the problem _per se_, it is sugar (fructose or fructose+glucose respecitvely) what is the real culprit here. However when adding fat to sugar, the effects are significantly amplified (Randle cycle is one of the mechanisms) 2) the actual mediator is the insulin. Insulin inhibits autophagy by several pathways (one is the activation of mTor, for example). That's why even if someone is eating a bad diet he can pretty mittigate the effects e.g. by excercising or by interventions like time-restricted-eating, which both lowers the insulin independ of the food quality and quantity ("calories").

4:49 -- "if this pump does not work properly...." Yes, that is the reason why proton-pump-inhibitors are detrimental in the long run as these make the autophagy (any sort of) less effective and promote agaeing and chronic diseases. There is even a controversy in the literature whether PPI's actually cause more cancer rather then preventing easophagal cancer...

It's quite clear that macroautophagy, CMA, and the downregulation of pathways such as mTOR are crucial for optimizing cellular function or in a broader sense, an individual's health. Why is modern medicine so reluctant to incorporate strategies(such as fasting, that has been used for centuries by practioners) into modern day treatment? As a medical student/practitioner do you see any change in the current treatment model? Would enjoy hearing your opinion. Thank you for the video, well done.

Our medicine (the big pharma) do not want to cure anybody. If so, how can they keep on selling drugs and making its money??? They like to remedy your problem or get some relief to it so you are always buying the medicines.

That's why excercising while fasting is a very good idea. Or any mild physical stress. (BTW, from my own experience I wouldn't recommend sauna after about 4~7 days of fasting. I know, once I try going into sauna after about 10~14 days of fasting (don't remember exactly) but that was REALLY harsh...)
