I can’t get my doctor to do anything but a general lipid test. HDL over 100, triglycerides in the 50’s, LDL of 160, with a zero CAC at age 75. She wants me to take statins. Says they can’t do advanced lipid testing. Frustrating. Thank you for your clear presentations. I wish my doctor would listen! Or read the Pub Med publications I’ve told her about.

Nadir - this is great stuff. It seems to me that we need to completely redefine interpretations of lipid panels based on individual dietary and physical exercise characteristics and not in isolation from other markers of metabolic health. As usual the existing interpretations were developed during a period of marked increase in population carbohydrate consumption over the last 40 years. BTW I fast, eat LC (mainly animal) and keep physically fit my last LDL level was 6.4 (UK) and TC 8.2 (UK) but my TG/HDL ratio was 0.54 - I'm a fat burner and happy to be so.

The problem is every doctor I have ever spoken too does not even recognise diet as a factor at all. Unless you eat low carb, then they say eat more carbs.

Thank you so much for boosting my confidence. I am on low carb diet with intermittent fasting for almost a year. My TG is .60 mmol/L, HDL .96 mmol/l, insulin 77 pmol/l, Glucose 4.9 mmol/l, A1C 5.4 however my LDL is 3.5 mmol/l . My family physician by default gave me hight doses of statin (which i would not take anyway). God bless you, please keep up the good work. All the best.

I have heard other metabolic doctors say that the reason low LDL correlates with all cause mortality and high LDL correlates with reduced all cause mortality is because when people are suffering end of life disease like cancer and infection their LDL plummets until death. I would like to hear Dr. Ali address that.

What a wonderful study and presentation, I respect honest people in every sphere of life. There is always the danger the big capital will take it over and emptied inside and use it for commercial world to exploit the weak and vulnerable ones

Thanks for explaining this subject. Especially why fasting raises ldl. I might eat high carb low fat before my next doctor visit. On second thought, I won't eat carbs. My trigs might go up and my hdl go down, three days to get back into ketosis. Just not worth it.

So, the last time I did my checks my HDL was 54, TGL was 69, TC was 393, LDL was 326.... am I concerned? Not really. I was eating a mostly saturated fat and probably too much protein diet before, but now am experimenting with a 80:20 diet but consuming daily about 100g of EV Olive Oil along with some butter/lard to see if it makes a difference ( something is -is it the switch to olive oil and/or the 80:20, something has lowered my fasting BG from 95 to 75(lack of gluconeogenesis from lowering protein??), and put my ketones from 1.0 to 3.0) But given the folly of the SAD and the obsession with LDL while misreading/ignoring the other markers what really is "normal" LDL?? In the UK "Normal" LDL is classed as about 116mg/dL, that's a world away from my results and the docs would try and get me on a statin for sure... but what if "normal" LDL was 250 for this diet? 300? (A diet which is closer to what humans have probably been eating since who knows when rather than all those carbs, sugars and processed rubbish??)

Hello Nadir... I would like to add on to this .... I have been reversing any component of metabolic syndrome using traditional rice , pulses and healthy lifestyle and invariably during the reversal of diabetes, triglycerides go down ... sometimes HDL goes down as well with normalization of blood sugar without any drugs. So , Its more about energy delivery ( Insulin resistance ) which are reflected by the numbers. One may not worry about low HDL ( on high healthy carb diet ) & high LDL ( on LCHF ) as long as TG are within limits.... Its my inference, sir. The remnant cholesterol does matter most rather than looking at HDL or LDL...

Great info! Before the day of my blood test, I will eat high carbs only to get my cholesterol level low. My all blood numbers are always good except high cholesterol. My doctor wants me to take statins, which I don’t want, and that is making me not to visit my doctor.

Tell him you made a personal decision not to take them. He will leave you alone then. Dont even argue the science.

same here my doctor freaked out when my LDL went up but everything else was great. He did not care about anything else but the LDL , disregarded the fact I dropped 30 pounds on LCHF diet andI feel great. Makes me want to find another Doc.

Thanks for the video. High LDL contribute to atherosclerotic plaque buildup. Some people with high cholesterol also have high Lpa. I appreciate your sharing your knowledge and expertise. The medical profession is now advising LDL be lower than 70 and the total cholesterol less than 153. Thank God I have finally been blessed to receive these goals.

When I was on SAD, my Trigs were 231, HDL was 31, Insulin was unmeasured but probably high, and LDL was 136. When I went on LCHF with intermittent fasting, Trigs went to 84, HDL went to 52, Insulin was around 7, and LDL went up to 200, exactly as predicted here. Now if I can just get my doctor to ignore the LDL number...

At 7:45 in the video: In the fasted state: "The liver is not picking up cholesterol from the blood nor it is making cholesterol either. But it has some cholesterol and it starts exporting it to the blood stream" At 10:00 and after in the video he begins to say the opposite. In the fasted state: "The liver is making LDL. LDL production rate goes up. In the fasted state the liver is making a lot more LD. LDL production rate goes up". At 10:22 in the video: He is repeating it, how in the fasted/fasting state LDL production goes up and the liver is not picking up cholesterol from the blood either. Where in the world all the HDL go? they just float around in the blood? In other words the livere is not picking up HDL so the level in the blood goes up because it is just floating around instead of it being taken care of in the liver ( not good) and also producing more LDL and sending them to the blood? By picking or not picking it can only mean HDL and by producing can only mean LDL. Nonetheless there are contradictory statements up there. Then at 10:30 he says the opposite again: He is describing the fasting/fasted state, the liver does not make any cholesterol nor does it need any cholesterol. It is not picking up cholesterol from the LDL receptors. He can only mean HDL. And the contradiction just simply repeats in the following slides. In the fasting/fasted state the liver is both producing and not producing cholesterol. At 10:42 in the video it says right at the bottom of the slide it says the liver is producing LDL in the fasting/fasted state. At 11:00 in the video right on the top of the slide it says that the liver is not producing LDL in the fasting/fasted state.

I was told by the phlebotomist that we shouldn’t Fast for blood lipid test but I had and mine was high. I exercise run mile walk 8 miles do lifting two times a week. My HDL is high but so is my LDL. Greeks and nerds rule the world.

Will try Intermittent fasting...usually stop eating by 8pm and coffee at 8am... Want to extend fasting for 24 hours...hope I succeed lol

At approx 13 minutes Dr Ali says, "so in this situation the person is eating a lot of cholesterol in their food and when they are eating a lot of cholesterol the liver gets a lot of cholesterol into it from the outside", but he doesn't give a reference of this. Dr Ali can you please tell me where to find the studies supporting the idea that a high cholesterol diet increases blood/liver cholesterol levels?

Seems to me from the explanation that vegan diet could the best cause you don’t need to eat consume cholesterol cause the body will make its own cholesterol. I guess that in vegan diet with veggies and olive oil will be enough right? So you can be ketosis with just veggies and oil? Maybe a little protein from nuts? Thanks if anyone reply.

Absolutely wrong, vegan diet is not ideal for humans,..I was a vegan for 17years with harmful effects on my health, I only eat animal food now and the difference in my health and well being is amazing.
