I am a board certified gynecologist and menopause practitioner with over 25 years of experience and it's very sad to see this information being given in this way. Certainly the term "bioidentical " has been used as a marketing term, but refers to any type of hormone that is biologically identical to human estrogen, including estradiol, progesterone and testosterone. Certainly some FDA products are bio identical (isomolecular) and are very good -but the idea that compounding pharmacies are not regulated is just blatantly false. She does not understand how compounding pharmacies work -they absolutely do batch testing and third-party testing if they are in the 503B status which is where pellets are made. She is talking about a topic as an authority and has no idea about how compounding pharmacies actually operate . There are also numerous studies and incredibly high levels of science that go into producing compounded hormones- her suggestion that they are made by a guy on the street is not only false but incredibly misleading. Women don't have any testosterone products that are FDA approved and so we are left with no option but to use compounding pharmacies. High level compounding pharmacies have very strict levels of regulation (503b status) and the idea that we don't know what's in those products is absolutely false. The idea that we know exactly what we're getting from an FDA approved product like an estradiol patch is also absolutely false. The absorption is incredibly variable and without ordering blood levels of either, we don't really know what we absorbing. Women wearing an estradiol patch can have a blood level of zero or a level that's way too high because absorption is very variable. Basing the dose on a patient's symptoms misses a lot, and suggests that the only reason we give it is for symptom relief, which is not accurate. We give it for prevention of multiple diseases as well as symptom relief. The American College of OB Gyn supports using testosterone for women for treatment of low sex drive and recognizes that we don't have any FDA approved products so we have no current option but to use compounding pharmacies, which are very good. Pellet therapy is also FDA approved for men in the form of testopel. It has not been approved yet for women, but the only difference is that women get 1/10 of the dose. The idea that you have no idea what you're getting in a pellet or with compounded medication is completely false. These drugs are very highly regulated and have very stringent levels of internal regulation as well as third-party testing. I prescribe FDA approved ismolecular or bio identical estradiol as well as compounded formulations when we don't have an FDA approved option and her suggestion that compounders are all bad is wrong and misinformed. The benefits of estrogen (specifically estradiol) for reducing the risk of cardiovascular disease, neurologic decline, osteoporosis prevention, sexually wellness, sleep, moods....as well as protection for the breast has been well established, and suggesting that estrogen increases the risk of heart disease is completely misinformed, and using very old data from the women's health initiative study, which is now 22 years old and needs to be abandoned as our source of anything to do with wisdom about hormone replacement . The idea of getting the lowest dose for the shortest time and not starting it when you're 10 years Postmenopause has also been found not to be true although the benefit is greatest in the first 10 years it is not contraindicated to start hormones after 10 years- that idea has been studied and shown to be false- while the benefit may be less there are still benefits when we start hormones for many women over 60 and it depends on their particular health situation. I'm glad she admitted that she doesn't know anything about pellets that is the only take away that I would take from this interview. She clearly knows absolutely nothing about pellets and should not be speaking about them. Of course, pellets need to be dosed by somebody who knows what they're doing like me, and we would never get into dangerously high ranges because they should be followed very closely by an expert as should anything.. There are so many things wrong with this interview. I hope women are smart enough to recognize that this woman is speaking as an authority about things (compounding and pellets ) that she knows absolutely nothing about.

What a wonderful response thank you. I wish that the commenters could realize that the other menopause specialists on the Internet are saying a lot of the same things that Dr. Gunter is saying. For example, Dr. Mary Haver might be more up-to-date in menopause hormone therapy and when it can be administered, and for how long, but unfortunately, she shares the same opinion that compounding preparations should not be used because they’re not regulated . it goes to show that we judge people by how they deliver their information and not actually what they’re saying. Yes, some of dr Gunters information is outdated for sure, but as it relates to compounding preparations She shares the same thoughts and messaging as all the other so-called menopause specialists on the Internet that people seem to love so much.

I appreciate your speaker and the information she provided. I do have to say as a woman who’s in the early stages of menopause, I’ve been getting HRT in the form of pellets for the last year and a half. This treatment is in addition to meeting with my doctor and her doing regular lab work to check my levels. True you may not know the actual amounts of these hormones going in but that’s why it’s critical to do regular lab work. HRT has been a major game changer for me in my menopause journey.

As a WHNP, I can appreciate the studies that go into the FDA approved HRT. However, doesn’t make the compounded ones any less effective. There are some studies done on non FDA approved methods, just not as many. Why? No money in it. I hate the idea that all the agencies encourage use of FDA approved drugs because everyone profits, except the patient. In my practice, approved and compounded are both used. What works for one doesn’t work for another. They are all tools and none of them are “bad” IMO. Why not try pellets, when nothing else seems to work!

Unfortunately, all the major social media menopause specialists have the same messaging and viewpoint on compounding hormones. I think the issue is Dr. Gunter’s approach is a lot more harsh and direct whereas for example, Mary Haver and Heather Hirsh speak a lot softer. But they’re messaging is all the same. It’s funny that when you read the comments from when Mel had Mary Haver on her podcast, no one bashed Mary Haver for her comments on compounding products yet everyone’s bashing Dr. Gunter for her comments on compounding products lol. Dr. Mary Haver and Heather Hirsh have the exact same viewpoint as Dr. Gunter on compounding products. They say not to use them, they say they’re not regulated and they basically said the same thing that Dr. Gunter said in this video. I think people just don’t like the Way, Dr. Gunter speaks, but essentially she’s saying the same thing that the other specialists are saying. Mel, I love you and your podcast! However, I missed important information about women who can't do hormone therapy due to cancer treatment. I know you can't address all the exceptions but in a world dealing with high levels of cancer in young women and many of them (including me!) entering menopause earlier, would be great to have more information explaining other ways to deal with the menopause symptoms without hormone therapy. ❤ I had breast cancer 2 years ago and entered menopause on the first month of the chemo. It was not only hard to deal with the whole cancer treatment, but "out of the blue"menopause, and with no help, it was even tougher! It took me a lot of research and reading to understand what I could do without hormone therapy and to find doctors who could help, which is very hard.

Mary Haver, Heather Hirsch, their delivery is softer than Dr. Gunter, but all of them say we should absolutely not be using compounded products- so it’s interesting that the comments here Are making it seem as though Dr is saying something different about compounded hormones than the other menopause experts lol. It scares me as I’m going through peri menopause. As I was sweating every night and hadn’t had a period for 3 months I’m 54, I was put on a pill. Then I got a short period and my gp eventually tells me to stop the tablet. I’m off the tablet. Holy shit it’s so frightening. Hi Barbara, if you can have a watch/listen to Mels conversation with Mary Claire Haver on menopause, I found her more educational. I'm an Aussie and would recommend you find a GP who specialises in Womens Health/Menopause as not all GP's know their stuff... I was offered antidepressants 🙄so found a specialist GP through word of mouth... she also diagnosed my Hypothyroidism where my GP had the signs 10 years prior and did nothing... and told me the other day that because I have Hashimotos I will ALWAYS have antibodies and no need to get them tested... which made my blood boil because this is just wrong!!! I'm on BHRT (Bio-identical) which this Dr on podcast does not like using this word hahaha in the form of a troche (pellet) and it has all three hormones in it... This Dr has in fact got me thinking about the whole compounding side of things... and I'm not sure why Dr's (Australian) Dr's would prescribe them (Compounding) if they were not safe and effective. I also have 6 monthly blood tests to check hormones which again this Dr does not recommend... and flatly refuses to do....I would like to hear Dr Mary Claire Haver's point of view on what this Dr has told the listeners.. because I'm not convinced on what she has had to say.

So my OBGYN put me on compound medication and I have been on them for about 7 to 8 years now and my symptoms improved dramatatically! She said that I could stay on the compound medication for longer than regular HRT. And honestly, most of my symptoms are either gone or very minimal. I can't even finish it and that's never happened with any on your podcasts. Bring Dr. Mary Claire Haver back. I'll leave it at this, the fact that she said you don't need labs to check your hormone levels floored me. I am estrogen dominant, yet I have menopause symptoms as I am in menopause. Without labs, I would have been prescribed estrogen, I needed progesterone, which was super low and is what I needed to help regulate and bring my estrogen levels down. Without the labs, I would have been throwing a dart in the dark and compromising my health.

There is also another thing here, most GP’s are not trained much about hormones or menopause, and so they often dont know enough about the typical symptoms and what helps. Too many women are for example mistakenly given antidepressants. And, how can you know in which form you should have your HRT when you dont do blood works? It’s not like everybody has the same levels and that everybody needs estrogen. Some may actually have estrogen dominance and can benefit from progesterone alone… 🤔

Instead of getting caught up and feeling offended by whatever therapy we are on that wasn't supported here; I think that the main take away to this is that it is important for women to understand that "bio-idental" "compounded" whatever.....it's the same raw hormone used in FDA products. It's not "safer" or "new"; or 'different' with respect to one that is regulated by FDA; vs another that might not be 'as' regulated per se. I have encountered places that are physically telling people these things;so that they buy their products/subscribe to their monthly "case management fees"; it's a lie telling people these things. I think that is the most upsetting. Taking advantage of others vulnerable states and their wallets. That's NOT fair to women. I will say though, that usually the places doing this type of thing are NOT OBGYN or even endocrinologists. I don't think a 8-12wks course in "hormones" from a functional medicine clinic makes anyone an expert or specialist--especially if they have not had previous training & clinic experience in a similar field of study.

Why is no one is bashing Dr, Mary Haver or Heather Hirst for having the exact same opinion on compounding preparations? They said the exact same thing - that we shouldn’t use compounding preparations because they’re not FDA approved and we don’t really know what’s in them or what we’re getting. I personally love my topical progesterone cream, but if you’re going to bash this doctor for having this particular opinion on compounding pharmacies, then you also shouldn’t be agreeing with Dr. Mary Haver or Heather Hirsch because they all have the same opinion on compounding preparations.

The most interesting thing about this podcast was reading these comments. Dr. Gunter does have a very aggressive and harsh way of speaking, which does not bode well with many people. And she does have a more traditional way of thinking when it comes to who should be given menopause hormone therapy and for how long which many people view as outdated. However, I don’t understand everyone’s comments on bashing her for her opinion on compounding hormones and pellets. Why is no one bashing Dr. Mary Haver or Heather Hirsch for having the exact same Messaging? Possibly because they speak a lot softer and their delivery is more gentle? All Of the menopause specialists on social media and Internet say the same thing about compounding preparations. They all say ,They’re not FDA approved, we don’t know what you’re getting blah blah blah. So why is everyone bashing Dr. Gunter for saying that but not Dr. Mary Haver? So strange. I’m in early perimenopause and I love my progesterone cream that I apply topically that I get from a compounding pharmacy. But I’m not going to bash Dr. Gunder for having this opinion because all the menopause specialists have this exact same opinion.

Not that ours is that great but you cannot get any serious medication without prescription so like hormone therapy with over the counter products is totally a no go. All these go only by doctors visits, exams and prescriptions. And because of this the meds you get are not from the side of the road they really do work. At least theres nothing confusing about this part. 😅😊😊

While vegan-vegetarian cuisine has improved greatly in look and taste in recent years, we should not forget that the protein yield of some of these meals is rather low. Best options for protein comes from animal based diet also the fats in red meat are excellent for your brain’s heath and metabolic health

Research PMU Industry. The recent push for all this education on menopause lately from "so many" channels feels unnatural. Whenever an issue is pushed from so many places it feels a bit like 'programing' that this time in life is supposed to be hard and of course there is a pill to solve the problem. Maybe we are living way out of balance with nature. There is no lie in nature. I love you Mel but this one I have to speak up on.

@@bathemeinchampagne right!! Our bodies don't make that estrogen that is synthesized so why would our bodies require it to "balance out"? We are living in overdrive and that is our problem. This podcast was all about pharma and scaring anyone away from attempting anything natural. At least she spoke of exercise being a big part of taking care of ourselves.

Can we help the body producing more hormones? What is the role of tostesteron? Are there any studies on sexuality, edging for a long period of time, that shows an increase of hormones in particular?

I am 65 and am now experiencing the hip pain and tightness when standing up. I see the commercials for provitalize which addresses this issue but wondering if it's another scam and is the hip issue menopause related. Anyone??

Hi @ sherrimeurer. I am in my 60ies and I am using ASEA .Have a look into that, it might reduce your symptoms as well. I am not stiff or anything exept need to be careful with the bones. It reduces my hotflashes and keeps me nimble.:)

Love you Mel, but I am disappointed in this episode, this Dr is so judgmental, rather old school and was given so much airtime to speak negatively about things outside of the pharmacies that pay her. I would have appreciated if it was a more nuanced approach, and with someone more of an expert on hormones, like Dr. Sara Gottfried, or the British Dr.Naomi Potter, or Dr.Mosconi who does research on our brain, hormones and menopause, or the Australian Dr Libby Weaver. They come from different angles, yet all really concerned for the women in perimenopause.

I found out if you take one B6 100 mg each day, and I promise you it will tamp down your hot flashes. I learned this from my gynecologist, and if I don’t take one every day, my hot flashes are more intense. Good luck. Renée

No. You need trt for that. Super disappointed she didn’t say anything about testosterone. We have more T than E in our body until meno. It’s more important for bone health than E is. 1:06:01

Well, Mel, I do not know if this is the kind of HRT this Dr is talking about, but if it based on pregnant horses urine, I can not emphasize enough what a horrendous thing that is. Do you imagine the quantity of urine needed for millions of women ? Can you imagine what it means for the mares ? I do not know if it is true, but I overheard stories about these mares being given little water in order for their urine to be very concentrated. There's been quite a scandal over that, I heard. In any way, mares being made pregnant on purpose, again and again, and in god knows what condition can not mean these poor creatures are having great lives. So it's like meat, getting these hormones into your body, even though transformed after being collected, means your body takes in the suffering, the stress, of the animals, same thing with meat animals. I certainly do not want that. There are great gynecologists in the US like Dr Rosensweet and his son who take care of women with bio-identical hormones, in a professional way, with everything every woman needs for her own individual needs. I am from Europe and we have great doctors like Thierry Hertoghe who are specialized and highly professional, enabling women (and men too) to get the exact treatment they need.

How can they leave out two of the biggest side effects of menopause which are lack of energy and no libido…maybe that’s what we mean when we say we “Don’t feel like ourselves.” She never addressed those two and the use (or lack there of) with testosterone. (Compounded, of course, because that’s all women can get!)

I could not even finish this episode. Sorry Mel, usually love you and your guests and learn so much but I found this episode to be annoying. The amount of time she used trashing bio identical hormones is crazy. I myself, along with so so so many women I know have a life changing experience on bio identical HRT. My mother-in-law, who is 80, has been on bio identical HRT for the last 35 years and looks and feels younger than 65 year olds that I know. It’s fine to express your opinion but leave it at that, in your opinion….
