Use CLR in the heater core to dissolve the crap in it. Started at Chrysler in the 90s, the design and materials they used across the board in the heater core would collect all the scale and plug. Blow out the core with some air, use hoses to fill core with CLR and let sit about 30 minutes, then flush both directions with clean water.

Did you need to drain the coolant first before detaching the heater core hoses at the firewall?

I did not. I expected some to leak out when I removed the hoses but it didn't really leak at all.
