I noticed in the DEXA video there were no positioning aids. Maybe they are there somewhere... . At the research institute I worked for, we would use positioning aids for longitudinal consistency and the scans would also be done in underwear. Consistent hydration and some other factors when taking longitudinal scans is very important.

Please yell at me if I have this wrong! At the end of the day, isn't it correct that if you have the willpower to modify your diet and exercise, that these drugs are not needed to obtain the same results?

I agree with your comment. But for those of us who were or are obese, the advantage of these meds come in the form of shutting off the “food noise” and allowing us to make the safe transition to more exercise and life style choices that work. I lost 130lbs on Mounjaro with setting up and maintaining a healthy diet and exercise schedule without that “noise”. I’ve been off Mounjaro for over six months and have continued to loose weight as well as moving off 7 meds and no longer insulin resistant or type two diabetic. I have cycled though many weight loss and weight gain as have most overweight people. I literally see Mounjaro as a miracle drug!!❤

The problem is that it’s a massive “if” you have the willpower. You’re going up against millions of years of hormone development to prevent humans starving. Read the book “The Hungry Brain”.

Evidence for this is all the natural bodybuilders out there. So its very possible. However once you have the weight on it becomes very difficult to lose and keep it off.

​@@jamesgilmore8192I am not certain that I would use bodybuilders, or those in weight controlled sports as success cases. The rates of eating disorders amongst those engaged in such practices is very high. It is quite possible that GLP-1 antagonists could help reduce those adverse sequelae.

You'll need to have a standardised procedure, such as hydration/food intake/exercise on the day before and day of and use the same machine to have greater confidence in the VAT measurement. In VAT precision studies (duplicate scans one after another) the %CV is around 5-8%.

I've been on Ozempic for nearly 2 years. I have not lost any weight, but my body composition totally changed in the right direction. I do functional fitness with weights (crossfit) 4x per week. I make a point to eat enough protein, thanks to Fairlife dairy products. I also take bi-weekly Rapamycin. For sure anyone taking GLP-1 agonists should be supplementing protein. It's really easy not to enough otherwise.

About the visceral fat and diet I would recommend watching the interviews on the subject on the Proof Podcast with Simon Hill. I'd say there is a lot of good research on the topic by now. Here's just a part of a longer interview focused on fatty liver and insulin resistance: kzbin.info/www/bejne/mXzUlnStl76mkK8si=3EXA8XHGD9KFVgE9 To my knowledge there are a large amount of epidemiological data showing correlation between saturated fat and visceral fat, and substitution analysis showing that a switch to polyunsaturated fats would reduce it. In an of itself that's weak evidence, but with the RCTs showing the same, I'd say that is the strongest link between diet and visceral fat. I'd say the research is showing high confidence of improvements from reducing intake of refined sugar and increasing antioxidant intake. A lot of people are talking about animal vs plant protein, but apart from the benefits of decreasing processed red meat I think the benefits likely come from increasing intake of polyunsaturated fats, antioxidants and fibers. For example, going from a daily dose of whey isolate to pea isolate probably doesn't do anything...

I hope studies will be coming out soon showing use of glp-1 agonists in individuals that are performing resistance training and/or eating higher protein diets because right now there’s a lot of, what I would consider, fear mongering about the use of these drugs as if the loss in lean mass and bone density are directly caused by the drugs, rather than the caloric deficit. Maybe it’s less well known to the general public, but anyone that knows anything about fitness has always known that significant caloric deficits cause muscle loss unless you’re eating sufficient protein and resistance training, and if significant enough of a deficit even that won’t be enough to maintain all of your muscle mass, so it really seems like a no-brainer to me that if you take a sedentary obese person with a typical diet and then significantly decrease portion size without changing anything else, they will lose muscle mass alongside their fat loss, so it makes sense that this is what you see in people taking ozempic and these other drugs. I’ve not looked for them, but I’d be interested to see studies on the body composition changes in sedentary people that lose weight solely through reducing portions of their normal diet compared to sedentary people that achieved similar caloric deficits from glp-1 agonists. Maybe my intuition on this is wrong and there is some yet to be discovered mechanism that causes targeted autophagy of muscle tissue, but I would be very surprised if there was much of a significant difference in the loss of lean mass. What did surprise me though was that your guest gained so much lean mass while losing a fair bit of weight at the same time, especially if I’m remembering/heard right that he gained 7lbs of lean mass while only does weight training about once per week and rock climbing a few other days? Those would be very good gains even for a beginner half his age in the same scenario.

Doctors finally have a way to induce calorie restriction with a pill. Prior to that few physicians really gave little effort to diet change.

When characterising weight loss/body composition its good to also include skinfolds and circumferences, e.g. Waist/Hip/Thigh Circumference, Triceps/Subscapular Skinfold. See the appendix 2. Yes its definitely undesirable. At nearly 100g it may well be a real effect as the dexa has uncertainty for VAT. Next stage would be to try and reduce that as well, maybe cortisol is high overnight.

What is the minimum requirement for visceral adipose? It’s needed to protect organs right?

bmi=bodymass/height^2 and you can calculate it in reserve.
