This is probably the most thorough coverage on high ankle. Should you use infrared heating? How about infrared heat light?
@SportsInjuryPhysio2 жыл бұрын
At the moment there isn't strong enough evidence to show that any infrared devices really make a difference to healing, but they have also not been shown to do any harm. So I would not spend money on getting that treatment but if you have it available already, then it may be OK to use.

Hi, Firsly, your video is amazing and the best video regarding high ankle sprain on youtube with such detail. I had a question regading ankle dorsiflexion mobilisation exercises while sufffering with high ankle sprain. It has been suggested that initially dorsiflexion should be avoided after high ankle sprain. Hi i suffered a ankle sprain while i was at work it has been hurting since last week i went to the ER i was told that it was a ankle sprain i have a hard time walking on it and i was given a boot to wear and crutches so i need to know how long i must wear the boot i have been doing rest and ice and partial weight bearing by the use of crutches how long will it take for this to heal?
@SportsInjuryPhysio4 күн бұрын
We can't really know how long you should wear the boot because it depends on the type of ankle sprain - the treatment is very different for a high ankle sprain vs. a lateral ankle sprain. A physiotherapist can assess it for you and give you the correct treatment advice. This is something that our team of physios can assess and help you with via video call. I have a less severe high ankle sprain on my left ankle. I was wondering if walking with a stabilising ankle brace is ok, as long as I dont get any pain signals. As for now (5 days after the sprain, but also immediately after that) I can bear all my weight on my left foot, I can make all kinds of rotations and I dont feel any pain. I only feel some kind of shivers here and there, but def. Not pain. I just dont want to do something wrong, but I feel I can walk.
@SportsInjuryPhysio10 ай бұрын
You need to check with your doctor - with this type of injury it is better to be too conservative at the start because you usually only have one chance to get it to heal conservatively and if you are too aggressive with your weightbearing you may end up interfering with your healing. So follow their advice.

A week ago my 9 year old daughter who is a competitive gymnast fell off the bar forward and we believed she got a high ankle sprain with a squeeze test at day 5. Initially she has 0 pain walking, level 2-4 pain doing tippy toe, jumping and running. We did taping and lace up brace. She still went to her gym training and did only conditioning. But in the past few days she developed walking pain level 1-2 from 0 at the time of injury. Shall I use Air Walking Boot instead of the brace to speed up the healing? How long do you think she can recover from a mild High Ankle Sprain? She is at her competition season is missing her competitions. Thank you for your video!

Can you also get a high ankle sprain by rolling your ankle forward (where you basically land on the tip of your toe and fall forward, causing hyperflexion in the front of your foot & ankle)? I know that that can obviously cause fractures, but could it just be a sprain if you landed with less force?
@SportsInjuryPhysio10 ай бұрын
This is the typical way in which you sustain a high ankle sprain or syndesmosis sprain (what this video is about)
@DIMI50346 ай бұрын
I can see that you also answered to some recent comments so I will try my luck. On the DF to outside test, if i feel like only dorsiflexion causes pain what should be the problem? Is it still a high ankle sprain or it can be something else? I was thinking it's a grade 1
@SportsInjuryPhysio6 ай бұрын
You can't make the diagnosis from just one test - you have to combine the history of how it happened + how your symptoms react to movements + the tests. So I really can't provide a safe guess at what may be your diagnosis.
@DIMI50346 ай бұрын
@@SportsInjuryPhysio basically the only problem is the DF. As for an injury moment, I don't really have an exact moment. I remember landing badly after a jump but the pain was only for a couple of seconds and I continued playing. I didnt have any problems with it afterwards except for the fact that I had the impression of the ankle cracking more often when moving it when I was laying down. After like 2 weeks from the incident it cracked harder one time (when laying down and just moving it) and after like 2 days I noticed having problems with it. There was no swelling, no nothing, just pain in DF movement. Now I am resting it and it seems better but I am not sure if resting is enough (I had an acl tear in my knee last year and I didn't know that yet, but after some longer rest I taught it's nothing serious and it's gonna get better and it was clearly false. I just don't know if that applies here)

I have 2 bruises on the back of the ankle, on either side of what I thought was Achilles tendon. Maybe 2 inches above ankle bone that sticks out on the side. Could that be the posterior ligament you mention in this video? It is the last longest lasting bruise. Others went away after 6 weeks

xray can show it because the bones moves further away from one another if it is a high ankle sprain.

I still wore the moon boot 2 more weeks adjusting to weight bearing again. now ive transitioned to walking 3+ weeks after. Still doing it tough though including pain and swelling..... How important is it to goto physio after? can i practise movements on my own?
@SportsInjuryPhysio4 жыл бұрын
You can absolutely practise movements on your own, but a physio can help you to choose the right exercises and make sure that you don't do too much too early. There is really no need to see a physio too often. All you need is rehab. I usually see my patients once for a 45 min session where we design their starter programme. Then I follow them up via email until they are ready to progress. For this type of injury I usually only see them every 3 to 4 weeks to update their rehab and if the patient progresses well and understands how to do their rehab they often only need to see me 3 to 4 times. So don't get sucked into seeing someone every week for x number of weeks!

AVOID DORSIFLEXION. I was 5 weeks into a grade 2-3 high ankle sprain recovery process. I did a bit of dorsiflexion and the ankle acted up and swelled for about 2 days. Does that mean the AITFL ligament is torn again and I have to restart the process all over again back to day 1?
@SportsInjuryPhysio2 жыл бұрын
It likely just means that you've irritated it rather than torn it. But you will have to adapt your exercises to suit your current symptoms (so make them lighter or ease off fully for a few days) but you may find that once it is calmed down again it reacts a bit quicker than what it did initially so it isn't totally back to square 1.

Why is ice still recommended when the inventor of the rice acronym retracted his statement in 2013 because 'ice does not help with healing' which is obvious. It dramatically decreases blood flow. Other than that, thank you for your help
@SportsInjuryPhysio9 ай бұрын
Because, like with most medical things, it all depends on the context. I agree that routine icing for days on end is not useful, but in some cases it is important to limit the internal bleeding and swelling immediately after the injury because if that is excessive it cuts off the circulation and you end up with a secondary injury. I discuss it in detail here: www.sports-injury-physio.com/post/using-ice-for-sports-injuries

Please help :( I have so much anxiety. I fell playing "hockey" in the living room and xrays showed a nondisplaced distal fibula fracture. I was put in a cast for 4 weeks. My ortho took more xrays, said that the fracture was fully healed, took me off the cast, and told me to start walking on a moon boot immediately. He gave me a theraband and isometric dorsiflexion stretches along with tracing ABCs to alleviate stiffness. My foot was still swollen and VERY bruised, and there was no assessment on my ligaments or consideration for an ankle sprain. He said I should be off the boot in ~2wks because I was "young and healthy". I walked on the boot without crutches that day (a few days ago) with minimal discomfort. The next day I woke up and it was very swollen, and I can barely place any weight on it. I'm back on the boot with crutches and have been doing dorsiflexion exercises (until I came across this video). I'm 99% sure I have at least a grade II syndesmotic sprain, but really want an MRI to confirm the extent of injury and make a recovery plan. I'm really afraid that I re-injured it and that he's not gonna take my concerns seriously. What should I do? Should I stop any DF stretches until I get an MRI?

Great work, thank you! But I have a question , as you know the RICE regime has changed again and now it is PEACE&LOVE. What do you think about the loading gradually in the acute phase? And now also the ice has been removed so what would you do instead? Thank you for taking your time and sharing + helping us!
@SportsInjuryPhysio8 ай бұрын
The advice about loading remains the same - this is a serious ligament injury and if you do strong dorsiflexion or too much weightbearing you will likely end up with it not healing and need surgery. As for ice - it still has a place during acute management but it should be used only when needed e.g. to stop internal bleeding and reduce excessive swelling - I discuss my opinion about it here: kzbin.info/www/bejne/gZjMh3evrZamo6s
