using the wrong shout in skyrim, i wanted to use the shout that makes you transparent since it removes fall damage so i could jump down a cliff as a short cut....i used the whirlwind sprint shout instead and launched myself far past the cliff and falling to my death

Also, there was one time I thought I still had my Histskin abbility selected, but when I wanted to use it for some quick healing after a battle, I turned into a werewolf. JUST AFTER KILLING SAHLOKNIR!!! Seriously, that sucked. Also, on my second go, I figured out how much easier it is to kill a dragon when using the Thu´um. :)

I died in Fallout 4's settlement mode once. I wanted to know how many stories high I could build on one plot, built a bunch of stairs to get my answer (it was 11). Then I accidentally put away the stairs that I was standing on, all 11 stories up.

The lamest way to die is to defeat a boss that killed you 20 times and then his last attack,poison or minion kills you before the kill counts. The boss is reset and you have to do all over again. Yes Dark Souls, I´m looking at you!!

I could swear blind that there was no grenade coming at me when I quick saved but when I loaded, there's a grenade just about to hit me. I finally got around it by very quickly going into my inventory and doing a load of drugs to absorb the splash damage as best as possible. This irritation was further compounded by FA4 not seeming to accept movement inputs immediately after loading...what's that all about, gotta wait about 1.5 seconds before the character will move.

Actual lag or losing connection. Losing connection has to be yours, but sometimes other people's lag can kill you. You can be shooting at a person lagging while standing still and they just won't take damage as everyone else proceeds to move around them. Heres one: getting outplayed, when your opponent makes it a sport to kill you in the most ridiculous fashion possible. For example: death by foot-stooled in Super Smash Bros, getting killed by the flashbang grenade launcher in Battlefield 4, or getting Taunt-killed in Team Fortress 2. Its as if your weakness or unfamiliarity with the game is publicly_GOD DAMMIT MARK! I'M TRYING TO LEARN HOW TO PLAY, AT LEAST LET ME LEARN THE CONTROLS!!!

I have another: In many FF games, casting Thundaga (or similarly-powerful spells) on your entire team while under the effects of Reflect will bounce all those spells onto the boss, inflicting at least double damage compared to casting normally. Imagine doing that only to realise Reflect just wore off and you've just nuked your own team...

I'm an old school gamer so cliffs are a major killer. Back in the day, there were secret ledges. You never knew where they were so in order to find them, you just had to jump off cliffs and see if there was one for you to fall onto. Many a death came from that tactic.

My younger brother had one of the worst possible scenarios for "wandering into a high level area". See, he decided he wanted to go exploring right at the start of FF2, and would regularly save it. So imagine how much horror, misery and frustration he felt when he accidentally saved it in the middle of Werewolf country, and died countless horrible deaths trying to get back to the start, only to reload right back where he started in the middle of what was effectively at that point a killzone.
